3 Introduction The German Cochrane Centre (GCC) is the representative of the international Cochrane Collaboration (CC) in Germany. Furthermore, it is the reference Centre for the German speaking part of Switzerland, for Austria, Liechtenstein, the Czech Republic, Slovakia and Hungary (for branches s. page 17). The CC is an international not for profit organisation with the vision that healthcare decision making throughout the world will be informed by high quality, timely research evidence (www.cochrane.org). To achieve this, more than 31,000 people from over 120 countries work together in preparing, updating and promoting systematic reviews about health care interventions. So far, over 6,000 Cochrane Reviews are published in the Cochrane Library (www.thecochranelibrary.com). The national centres are responsible for the following core functions in their countries: 1. To promote and represent The Cochrane Collaboration. 2. To serve as a source of information about The Cochrane Collaboration. 3. To provide or facilitate training and support for review authors, editors, handsearchers and other contributors to The Cochrane Collaboration. 4. To support regional editorial bases of Review Groups, Methods Groups and Fields by: assistance to find funding and mediating conflicts, either between Cochrane entities or between individuals and entities. 5. To contribute to improving the quality of Cochrane reviews by performing, supporting or promoting methodological research. 6. To promote accessibility to The Cochrane Library to healthcare professionals, patients and others, e.g. by pursuing national subscriptions and translations where necessary. 7. To handsearch general healthcare journals in the linguistic area of the Centre and to submit the search results to the Collaboration s trial database. The GCC was formally registered within The Cochrane Collaboration in Up to 2004 it received funding from the German Federal Ministry of Education and Research, which was complemented by the Medical Center of the University of Freiburg and the Medical Faculty of the University of Freiburg. Since January 2005 the German Federal Ministry of Health has sponsored the GCC and will continue till the end of The University of Freiburg s Institute for Medical Biometry and Medical Informatics hosts the Centre and funds the position of the director and one administrator. 4 In addition to the core functions as a centre of the CC, the German centre acts as a national hub for the implementation of international scientific and strategic developments related to knowledge transfer, the concept of evidence based medicine and the for role systematic reviews can play for both. The centre supports the implementation of methods related to the concepts of evidence based health research and the grading of evidence according to the GRADE approach. Another scientific focus is the investigation of the extent of publication bias in medical research and how it affects the reliability of the results of research syntheses. The GCC was involved in several high rank scientific projects (s. p. 4 ff) in 2012 and 2013, and presented the results of its work in publications and a large number of presentations (s. p. 21ff and 311ff). The GCC s peer reviewed publications add up to 68 Impact Factor points in 2012 and 98.5 in In 2012, a highlight of the centre s work was the organisation and realisation of the Scientific Symposium including the 4th Annual Lecture held by John Ioannidis of the Network for Enhancing the Quality and Transparency of Health Research (EQUATOR Network). The Network embraces worldwide activities to improve the quality of scientific reporting by developing and implementing reporting guidelines (www.equator network.org). The symposium was a cooperative project between the EQUATOR Network and the GCC and attracted the attention of nearly 300 participants and was reviewed very positively. Furthermore, in 2012 the Faculty of Medicine of the University of Freiburg honored the persistent and outstanding efforts of Gerd Antes relating to the Cochrane Collaboration (founding the GCC), the concept of evidence based medicine, transparency in medical research (co founder of the German Register for Clinical Trials) and knowledge translation in Germany by awarding him an honorary professorship. In 2013 the German Ministry of Health commissioned an expertise about the GCC. Last but not least: In 2014 the Medical Center of the University of Freiburg will make the GCC a central institution for an initial period of five years with prospects of ongoing funding afterwards. 5 Objectives in 2012 and 2013 The objectives of the GCC in 2012 and 2013 were: 1. Fulfilling the Core Functions of a Cochrane Centre 2. Supporting the activities of the GRADE Working Group 3. Recruiting and supporting reviewers / review groups 4. Participating in systematic review activities, evidence assessments and HTA reports 5. Developing and conducting education and training, workshops and courses related to evidence based medicine, systematic reviews and the GRADE methodology 6. Improving the quality of publications of clinical trials in German journals by promoting publication guidelines 7. Information for users of evidence for different target groups (science journalists, health system professionals, consumers and the public) 8. Maintaining information structures 9. Searching German printed publications for non published clinical trials (Handsearching) 10. Establishing a stable funding for the GCC 11. Organising the EQUATOR Symposium The objectives were accomplished by the following projects, workshops and events, publications and presentations and related activities: Projects in 2012 and 2013 (selection) In 2012 and 2013 the GCC conducted a total of 16 funded projects with an overall funding volume of more than 1.2 million euros. Briefly, the major projects were: The work of the German Cochrane Centre to support knowledge transfer in health care funded by the Federal Ministry of Health The project Die Arbeit des Deutschen Cochrane Zentrums zur Unterstützung von Wissenstransfer im Gesundheitswesen ( The work of the German Cochrane Centre to support knowledge transfer in health care ), funded by the Federal Ministry of Health, was granted in 2011 and carried on until the end of This project constituted main funding for the GCC in support of its most important goals according to a centres core functions, namely to distribute and promote evidence based medicine through German Cochrane Centre Annual Report 2012/2013 4/46

6 systematic reviews. These are understood as an ongoing process. In 2012 the GCC hosted several workshops to provide training in literature research, writing of systematic reviews and using the GRADE method. Acting on Knowledge funded by the Federal Ministry of Health The Federal Ministry of Health granted further funding in May 2012 targeting special issues for improving knowledge transfer for German Healthcare: ACTING ON KNOWLEDGE Aktivitäten des Deutschen Cochrane Zentrums zur Weiterentwicklung der systematischen Nutzung von Wissen (Knowledge Translation) in der Gesundheitsversorgung in Deutschland (Activities of the German Cochrane Centre to advance the systematic use of knowledge (translation) in healthcare in Germany). The project was granted till the end of 2013 and was approved for expansion till the end of ACTING ON KNOWLEDGE was complementary to the project above and targeted on: An overview and evaluation of the methods for evidence mapping Evidence based methods and training for the non medical health professionals Support of clinical guideline authors by preparing a manual for systematic search of literature [in cooperation with the Institute for Knowledge Management of the German Association of the Scientific Medical Societies (AWMF IMWI) and the Agency for Quality in Medicine (ÄZQ)] Development of a strategic concept for a German National Health Library The EQUATOR Scientific Symposium 2012 OPEN (Overcome failure to Publish negative findings) funded by the European Commission This project, funded by the European Commission, was granted in 2011 and was carried out until October The GCC acted as coordinator of this project which united 11 European partners. Key aims of the project were to diminish publication bias by bringing together opinion leaders, conducting systematic reviews about the topic and developing evidence based recommendations for initiating the implementation of adequate measures to control publication bias and limit its effects on clinical decision making. The Kick off Meeting was held in Freiburg in February 2012 and the Recommendation Workshops in Barcelona and Freiburg were held in Furthermore, various findings which derived from the work packages of the project were published in peer reviewed journals. Source: project.eu RARE Bestpractice funded by the European Commission The GCC is a partner in an international consortium which applied for EU funding for the project Platform for sharing best practices for management of rare diseases: German Cochrane Centre Annual Report 2012/2013 5/46

7 RARE Bestpractices in The grant started in January 2013 and will last until December The platform will be used for a comprehensive understanding of the renewed challenges which are generated by actions and incentives of the European Commission. It intends to address the following critical questions: A) How to take better advantage of scientific progress in biomedical research in ways to translate research results into tangible benefits for rare disease patients in a timely manner and to offer high quality information to these patients and health professionals? B) How to promote and support a consistent level of healthcare services for rare disease patients, thus implementing the EU Directive on the application of patients rights in cross border healthcare? Source: DECIDE funded by the European Commission The GCC is a partner in the international consortium of the EU project DECIDE (Developing and Evaluating Communication Strategies to Support Informed Decisions and Practice Based on Evidence), which aims to improve the dissemination of evidence based recommendations by building on the work of the GRADE Working Group to develop and evaluate methods that address the targeted dissemination of guidelines. The project runs from June 2011 until the end of The DECIDE consortium, which is a composition of members of the GRADE Working Group, will further develop the GRADE approach to ensure effective dissemination of evidence based recommendations targeted at the key stakeholders (healthcare professionals; policymakers and managers; patients and the general public) who determine activities/decisions in clinical practice. The consortium adapts stakeholder input from advisory groups, consultations and user testing. This is done across a wide range of health systems in Europe. The targeted dissemination strategies that are developed, will be evaluated in randomised trials, refined and used and evaluated with real guidelines, developed by the DECIDE partners and other guideline developers, that the project partners support. Source: collaboration.eu ECRAN funded by the European Commission The GCC is partner in the international consortium of the ECRAN project (European Communication on Research Awareness Needs). The project is funded by the European Community in order to improve the EU Citizens knowledge about medical research and support their participation in independent and multinational clinical trials. The project runs from September 2012 to August ECRAN aims to provide clear and reliable explanations of basic and more advanced concepts, practical information, educational contents including guides, tutorials, an German Cochrane Centre Annual Report 2012/2013 6/46

8 animated film and even a game to make sure the public is well informed about clinical research and participation in clinical trials. Source: EVAS funded by the Academy for Health Service Research of Baden Wuerttemberg The project Analyse der Evidenzbasierung von Ernährungsbroschüren für Schwangere (Analysis: How evidence based are nutrition brochures for pregnancy? ) was funded by the Young Academics Academy for Health Service Research of Baden Württemberg. It started in July 2012 and lasted until the end of The project aimed at being a basis for a new brochure for pregnant women based on the results of the text analyses of existing brochures, websites etc. In 2012 the research started with collecting brochures and processing the actual evidence in those. Moreover, new data was collected by the use of surveys among gynecologists, nutritionists, pregnant women. Review and Evidence Report Projects with the German Federal Ministry of Education and Research (BMBF), the National Association of Statutory Health Insurance Funds (GKV) and the Institute for Quality and Efficiency in Health care (IQWiG). In 2012 and 2013 funding for the conduct of systematic reviews was granted by the German Federal Ministry of Education and Research and the National Association of Statutory Health Insurance Funds. For the IQWiG a small scale project for creating a research strategy on Identification and Evaluation of German reviews began in February 2011 and was successfully finalised in the beginning of The second small project funded by the IQWiG was an update of evidence based health care information for patients which started in October INKA funded by the German Ministry of Education and Research This project (German title: Ein Forschungsprojekt über die Veränderung der Informationsqualität in der Kommunikationskaskade (INKA) von der medizinischen Fachpublikation bis zur Rezeption der Medizinberichterstattung durch Ärzte und Laien ) analyses scientific Information on its way from the medical abstract to mass media contributions and is funded from 2013 until the end of While the project partner at the Technical University Dortmund analyses the loss of correct information from initial scientific reports down to the diffusion into public media, the GCC investigates the information behavior and resources of German Physicians in an exploratory study. International Cochrane Website Development The GCC hosts the Web Team of the Cochrane Collaboration. Through project funding from the Cochrane Collaboration re development of the Collaboration s website has been made possible. In this still ongoing project, improved technologies where German Cochrane Centre Annual Report 2012/2013 7/46

9 introduced such as site search function, persistent navigation features, and a content management system which allows editors and contributors to manage content remotely with a simple web browser. Even more significant, the new site brings together, at a single web address, many online resources which were previously spread across a confusing array of sites. This centralization is the beginning of a broader standardization process for websites across the Collaboration, bringing a new Cochrane style and content management tools to many Cochrane entity sites. The new website of the Collaboration is playing a central role in promoting access and participation in these changes, and providing improved internal support and outreach to the public, the media and policymakers. Joerg Meerpohl was engaged in the GRADE Working Group and subsequently in the WHO Nutrition Guidance Expert Advisory Group. He established a new workshop ( diagnostic interventions ) in Freiburg which was successfully hosted in autumn 2013 and, furthermore, he established and organised a basic review course for authors together with the Austrian and Swiss Cochrane branches. 13 Demonstrations and tutorials of The Cochrane Library Courses in literature search and methodology in the context of Systematic Reviews In October 2012 the DCZ, in cooperation with the EQUATOR Network, was responsible for the organisation of the annual symposium of the EQUATOR Network (Enhancing the QALity and Transparency Of health Research). The conference, funded by the German Ministry of Health, brought together 300 participants from all over the world under the theme ACT NOW Accuracy, Completeness and Transparency in Health Research Reporting. The annual lecture of the Network was held by John Ioannidis: Reporting and reproducible research: salvaging the self correction principle of science. 14 Contribution to The Cochrane Library : The Cochrane Central Register of Controlled Trials Handsearching means searching manually for trial publications in printed scientific medical journals. Each Cochrane Centre searches medical journals, published in national languages, to identify these reports, which otherwise would not be accessible, because the printed journals are not digitally available or not listed e.g. in the great biomedical literature databases. This effort increases our knowledge base of data from clinical trials and tries to overcome barriers leading to publication bias (by source, medium or language) in systematic reviews. The results of this process are submitted to the Cochrane Central Register of Controlled Trials, which meanwhile contains over citations of controlled clinical trials more than accessible via PubMed or EMBASE. In the last 15 years the GCC identified more than study publications, of which nearly 50 % are not listed in Medline and therefore not easily detectable for scientific research. 15 Cochrane in Germany The efforts of the GCC, the German Review Groups (see below) and the Germanybased Cochrane working groups (www.cochrane.de/de/arbeitsgruppen) as well as the contributing people in Germany lead to the fact that the number of corresponding authors in Germany, contributing to Cochrane Reviews, went from 542 in 2010 up to 691 in 2013 (Search filter: Country: Germany; Reference Centre: GCC; Role: Author). The GCC as reference centre was indexed by persons (Search filter: Reference Centre: GCC; Role: author). 247 of them came from Switzerland, 114 from Austria, 7 from the Czech Republic and 4 from Hungary, none from Liechtenstein (Source: ARCHIE, Cochrane author s database, search from ). 1 Figure 1: Number of Reviews and Protocols with Germany as country of origin since from persons (691 Germany, 247 Switzerland, 114 Austria, 7 Czech Republic and 4 Hungary) remain 637 with residency in other countries. 16 The production of reviews with corresponding authors in Germany shows a continuous increase from 1998 until Figure 2 shows the number of published systematic reviews and protocols from Germany, Switzerland, Austria, Liechtenstein, Hungary and the Czech Republic. The main focus is on leukaemia and lymphoma and the care of those suffering from other haematological diseases like aplastic anaemia is also included in the wider scope of the group. In addition, the group evaluates interventions affecting the haematological cell system like stem cell transplantation and haemotopoetic growth factors. Finally, supportive care and the management of haematological diseases also are tasks of the Cochrane Haematological Malignancies Group. German Cochrane Centre Annual Report 2012/ /46

17 Cochrane Metabolic and Endocrine Disorders Group Prof. Dr. Bernd Richter, Co ordinating Editor Department of General Practice, University Hospital Düsseldorf, Heinrich Heine University, PO Box , D Düsseldorf, Germany E mail : duesseldorf.de Web: endoc.info Since 2000, the CMED has been preparing systematic reviews on the benefits and harms of healthcare interventions for metabolic and endocrine disorders, particularly diabetes mellitus and obesity. The CMED's main area of work is the evaluation of the effects of health care interventions relevant to the prevention, treatment or management, and rehabilitation of metabolic, nutritional and endocrine disorders. The CMED expanded its scope to systematic reviews of diagnostic test accuracy within the same topic areas. The CMED primarily focuses on systematic reviews which evaluate patient orientated outcomes measures. Authors who wish to analyse surrogate outcomes like laboratory parameters may do so but always have to investigate a standard set of patient orientated endpoints. 18 Branches of the German Cochrane Centre Austrian Cochrane Branch The Austrian Cochrane Branch opened in December 2010 as part of the Department for Evidence based Medicine and Clinical Epidemiology of the Danube University Krems. Main tasks are: It provides the basis for evidence based decisions in the Austrian health system. It is supported by Austrian scientists, who provide the means to create sciencebased and independent medical information in the form of Cochrane Reviews that are available to the Austrian health care and the public. It promotes the publication of the Cochrane Collaboration and its importance in the public, aiming at transferring research findings into health care and thereby improving patient information. It organises workshops for those interested, such as (future, potential) authors of Cochrane reviews as well as employees of the Cochrane Collaboration. It is the first point of contact for people interested as well as for employees of the Cochrane Collaboration in Austria. It functions as a contact for employees of the Cochrane Collaboration worldwide, and thus facilitates the communication within the Cochrane Collaboration. 19 Swiss Cochrane Branch The Swiss Cochrane Centre is the branch that is supported by the German, the French, and the Italian Cochrane Centres. It is a national network which is located in Lausanne, Switzerland. The goals of the branch are: Promotion of evidence based medicine and the activities of the Cochrane Collaboration. Cochrane Switzerland supports free access to the Cochrane Library for people in Switzerland Information of health care professionals and a broader audience about the usage and the benefits of systematic reviews Promotion of projects for creation and updating of systematic reviews referring to questions of the effectiveness of medical interventions.
