INTENDED USE Enzyme immunoassay for the in-vitro diagnostic quantitative determination of autoantibodies against surface antigens of spermatozoa in human serum. 2. SUMMARY AND EXPLANATION In Western Society, the incidence of infertile couples is estimated to be 10-15% of the population. The role of anti-sperm-antibodies in infertility remains controversial, due to the different methods of determination. Among the classical methods for detecting anti-sperm-antibodies, the sperm-agglutination test and the sperm-immobilisation test have been widely applied. These tests and other variants of agglutination tests are both time consuming and inconsistent in performance. ELISA-tests for anti-sperm-antibodies have many advantages over conventional methods. The ELISA for spermatozoa-antibodies from IBL combines these advantages with high sensitivity and specificity. The test is easy to perform and enables the screening of large numbers of sera for immunological infertility. 3. TEST PRINCIPLE The test is based on a non-competitive ELISA. Sperm-surface antigens are extracted from a pool by a method related to that of Alexander (1984) and are coated on the wells of microtiter strips. All samples are incubated in duplicate either on coated and also on uncoated wells (yellow plate). The strips are incubated with diluted sera from patients, and after a washing step, are incubated again with peroxidase conjugated anti-human-immunoglobulin (IgA, IgG and IgM). Following a final wash step, enzyme substrate (TMB) is added and color developed is determined using an ELISA reader. The color intensity in the wells is proportional to the bound immunoglobulins. The non-specific binding is determined individually for each serum and subtracted from the total binding. Results are read from a standard curve and are expressed as Units/mL (mu/100µl). 4. WARNINGS AND PRECAUTIONS 1. For in-vitro diagnostic use only. For professional use only. 2. Before starting the assay, read the instructions completely and carefully. Use the valid version of the package insert provided with the kit. Be sure that everything is understood. In case of severe damage of the kit package please contact IBL or your supplier in written form, latest one week after receiving the kit. Do not use damaged components in test runs, but keep safe for complaint related issues. 4. Obey lot number and expiry date. Do not mix reagents of different lots. Do not use expired reagents. 5. Follow good laboratory practice and safety guidelines. Wear lab coats, disposable latex gloves and protective glasses where necessary. 6. Reagents of this kit containing hazardous material may cause eye and skin irritations. See MATERIALS SUPPLIED and labels for details. Chemicals and prepared or used reagents have to be treated as hazardous waste according to national biohazard and safety guidelines or regulations. 8. Avoid contact with Stop solution. It may cause skin irritations and burns. 9. All reagents of this kit containing human serum or plasma have been tested and were found negative for HIV I/II, HBsAg and HCV. However, a presence of these or other infectious agents cannot be excluded absolutely and therefore reagents should be treated as potential biohazards in use and for disposal. 5. STORAGE AND STABILITY The kit is shipped at ambient temperature and should be stored at 2-8 C. Keep away from heat or direct sun light. The storage and stability of specimen and prepared reagents is stated in the corresponding chapters. The microtiter strips are stable up to 6 w in the broken, but tightly closed bag when stored at 2 8 C. Version 1 / / 6

It is important to preserve the chemical integrity of a blood specimen from the moment it is collected until it is assayed. Do not use grossly hemolytic, icteric or grossly lipemic specimens. Bidistilled or deionised water 7. Paper towels, pipette tips and timer 9. PROCEDURE NOTES 1. Any improper handling of samples or modification of the test procedure may influence the results. The indicated pipetting volumes, incubation times, temperatures and pretreatment steps have to be performed strictly according to the instructions. Use calibrated pipettes and devices only. 2. Once the test has been started, all steps should be completed without interruption. Make sure that required reagents, materials and devices are prepared ready at the appropriate time. Allow all reagents and specimens to reach room temperature (18-25 C) and gently swirl each vial of liquid reagent and sample before use. Mix reagents without foaming. 3. Avoid contamination of reagents, pipettes and wells/tubes. Use new disposable plastic pipette tips for each reagent, standard or specimen. Do not interchange caps. Always cap not used vials. Do not reuse wells/tubes or reagents. 4. Some components contain 250 µl solution. Take care that the solution is completely on the bottom of the vial before opening. Version 1 / / 6

Use a pipetting scheme to verify an appropriate plate layout. 7. Incubation time affects results. All wells should be handled in the same order and time sequences. It is recommended to use an 8-channel Micropipettor for pipetting of solutions in all wells. 8. Microplate washing is important. Improperly washed wells will give erroneous results. It is recommended to use a multichannel pipette or an automatic microplate washing system. Do not allow the wells to dry between incubations. Do not scratch coated wells during rinsing and aspiration. Rinse and fill all reagents with care. While rinsing, check that all wells are filled evenly with Wash Buffer, and that there are no residues in the wells. 9. Humidity affects the coated wells/tubes. Do not open pouch until it reaches room temperature. Unused wells/tubes should be returned immediately to the resealed pouch with desiccant. 10. PRE-TEST SETUP INSTRUCTIONS Preparation of lyophilized or concentrated Components The volumes stated below are sufficient for one test procedure using 4 coated and 4 uncoated strips (32 determinations). TEST PROCEDURE 1. Secure the coated and uncoated (NSB, yellow) microtiter strips alternately in the holder. For each Standard, Control and sample take 2 of the coated and 2 of the uncoated (yellow) wells. 2. Pipette 100 µl of each Standard, Control and diluted sample into the respective wells. 3. Cover plate with adhesive foil. Incubate 1 h at 37 C. 4. Remove adhesive foil. Discard incubation solution. Wash plate 3 x with 250 µl of diluted Wash Buffer. Remove excess solution by tapping the inverted plate on a paper towel. 5. Pipette 100 µl of freshly prepared Enyzme Conjugate into each well. 6. Cover plate with new adhesive foil. Incubate 1 h at 37 C. Version 1 / / 6

5 Anti Sperm Ab ELISA (RE52029) ENGLISH 7. Remove adhesive foil. Discard incubation solution. Wash plate 3 x with 250µL of diluted Wash Buffer. Remove excess solution by tapping the inverted plate on a paper towel. 8. For adding of Substrate and Stop Solution use, if available, an 8-channel Micropipettor. Pipetting should be carried out in the same time intervals for Substrate and Stop Solution. Use positive displacement and avoid formation of air bubbles. 9. Pipette 100 µl of TMB Substrate Solution into each well. 10. Incubate min at RT (18-25 C). 11. Stop the substrate reaction by adding 50 µl of TMB Stop Solution into each well. Briefly mix contents by gently shaking the plate. 12. Measure optical density with a photometer at 450 nm (Reference-wavelength: nm) within 30 min after pipetting of the Stop Solution. 12. QUALITY CONTROL The test results are only valid if the test has been performed following the instructions. Moreover the user must strictly adhere to the rules of GLP (Good Laboratory Practice) or other applicable standards/laws. All standards and kit controls must be found within the acceptable ranges as stated on the QC Certificate. If the criteria are not met, the run is not valid and should be repeated. Each laboratory should use known samples as further controls. In case of any deviation the following technical issues should be proven: Expiration dates of (prepared) reagents, storage conditions, pipettes, devices, incubation conditions and washing methods. It is recommended to participate at appropriate quality assessment trials. 13. CALCULATION OF RESULTS Correct the OD of the coated well for each standard, control and sample by the corresponding OD of the NSB well. Calculate the mean value of duplicates. The obtained OD of the standards (y-axis, linear) are plotted against their concentration (x-axis, logarithmic) either on semi-logarithmic graph paper or using an automated method. A good fit is provided with cubic spline, 4 Parameter Logisitcs or Logit-Log. For the calculation of the standard curve, apply each signal of the standards (one obvious outlier of duplicates might be omitted and the more plausible single value might be used). The concentration of the samples can be read directly from the standard curve. Due to the dilution of samples the values obtained have to be multiplied by the factor 0.5 to receive the concentrations in U/mL in the undiluted sample. In case of diluted samples the values have to be multiplied with the corresponding dilution factor. Samples showing concentrations above the highest standard have to be diluted as described in PRE-TEST SETUP INSTRUCTIONS and reassayed. Typical Calibration Curve (Example. They have to be correlated to other clinical observations and diagnostic tests. Apparently healthy subjects show the following values: < 150 mu/100 µl It is recommended that each laboratory establishes its own range of normal values. 15. ZWECKBESTIMMUNG Enzymimmunoassay für die quantitative in vitro-bestimmung von Autoantikörpern gegen Oberflächenantigene von Spermatozoen in humanem Serum. 2. KLINISCHE BEDEUTUNG Etwa % aller Paare mit Kinderwunsch in der westlichen Gesellschaft leiden unter einer Fertilitätsstörung. Die Rolle von Spermatozoenantikörpern wird in diesem Zusammenhang kontrovers diskutiert, nicht zuletzt aufgrund der vielfältigen Methoden ihrer Bestimmung. Die klassischen Methoden zum Nachweis von Spermatozoenantikörpern beruhen auf deren biologischen Auswirkungen wie Agglutination oder Immobilisation. Der Nachteil dieser Verfahren besteht neben einem hohen Zeitaufwand in der subjektiven Auswertung und der fehlenden Quantifizierung. Die genannten Nachteile gelten nicht für den IBL-Spermatozoen-Antikörper-ELISA, der die Vorteile des direkten Antikörpernachweises (Spezifität) mit der Quantifizierbarkeit der Ergebnisse verbindet. Durch die Verwendung einer repräsentativen Mischung aller Oberflächenantigene des Spermiums wird zudem eine hohe Sensitivität erreicht. Der Test ist leicht durchzuführen und erlaubt das Screening großer Probenanzahlen auf immunologische Infertilität. 3. TESTPRINZIP Das Testprinzip basiert auf einem nichtkompetitiver ELISA. Oberflächenantigene von Spermien, die durch ein spezielles Extraktionsverfahren (nach Alexander, 1984) gewonnen werden, werden in den Wells von Mikrotiterplatten immobilisiert. Verdünntes Patientenserum wird jeweils in einer beschichteten - und für die Bestimmung der unspezifischen Bindung - in einem unbeschichteten Well (gelbe Mikrotiterplatte) inkubiert. Nach einem Waschschritt werden gebundene Antikörper mit einem zweiten, Peroxidase-konjugiertem antihuman IgA-, IgG- und IgM-Antikörper nachgewiesen. Es folgt ein weiterer Waschschritt. Nach Zugabe der Substratlösung (TMB) und später der Stopplösung, entwickelt sich proportional zu der Menge der gebundenen Antikörper ein gelber Farbstoff, dessen Lichtabsorption photometrisch gemessen wird. Die unspezifische Bindung in den unbeschichteten Wells der gelben Mikrotiterplatte wird von der optischen Dichte in den entsprechenden beschichteten Wells abgezogen. Die Differenz der optischen Dichte ist proportional zu der Menge spezifisch gebundener Antikörper. Die Antikörperkonzentration in Units/mL (mu/100µl) wird an einer Eichkurve abgelesen. 4. WARNHINWEISE UND VORSICHTSMASSNAHMEN 1. Nur zum In-vitro-Gebrauch. Nur für den Gebrauch durch Fachpersonal. 2. Vor der Testdurchführung sollte die Arbeitsanleitung vollständig und sorgfältig gelesen werden und verstanden worden sein. Die gültige Version aus dem Kit verwenden. Weitere Informationen (klinischer Hintergrund, Testcharakteristika, Automatisierung, alternative Anwendungen, Literatur, etc.) finden Sie auf der IBL-Homepage. 3. Im Falle einer erheblichen Beschädigung der Testpackung ist IBL bzw. der jeweilige Lieferant innerhalb einer Woche nach Empfang der Ware schriftlich zu benachrichtigen. Beschädigte Komponenten dürfen nicht zur Testdurchführung verwendet werden, sondern sollten solange aufbewahrt werden, bis der Transportschaden endgültig geregelt ist. 4. Chargen-Nummer und Verfallsdatum beachten. Es dürfen keine Reagenzien aus unterschiedlichen Chargen in einem Test verwendet werden. Verfallene Reagenzien dürfen nicht verwendet werden. 5. Gute Laborpraxis und Sicherheitsrichtlinien beachten. Je nach Bedarf sollten Laborkittel, Einmal- Latexhandschuhe und Schutzbrillen getragen werden. 6. Reagenzien dieses Kits, die Gefahrstoffe enthalten, können Reizungen der Augen und der Haut hervorrufen. Siehe Angaben in KOMPONENTEN DES KITS und auf den Etiketten. Sicherheitsdatenblätter für dieses Produkt sind auf der IBL-Homepage zum Download verfügbar oder auf Anfrage direkt von IBL. 7. Chemikalien und vorbereitete oder gebrauchte Reagenzien sind unter Beachtung der jeweiligen nationalen Bestimmungen als Gefahrstoffabfall zu entsorgen. 8. Kontakt mit Stopplösung vermeiden. Kann Hautreizungen und Verätzungen hervorrufen. 9. Alle Reagenzien dieses Kits, die humanes Serum oder Plasma enthalten, ergaben bei der Prüfung auf HCV, HBsAg bzw. Antikörper gegen HIV I/II-Virus ein negatives Ergebnis. Trotzdem kann das Vorhandensein solcher infektiöser Erreger nicht mit absoluter Sicherheit ausgeschlossen werden. Die Reagenzien sollten deshalb wie potentiell infektiöses Material behandelt werden. Version 1 / / 5

8 anti Sperm Ab ELISA (RE52029) DEUTSCH 5. LAGERUNG UND HALTBARKEIT Der Kit wird bei Umgebungstemperatur angeliefert und sollte bei 2-8 C gelagert werden. Vor Hitze und direkter Sonneneinstrahlung schützen. Hinweise zur Lagerung und Haltbarkeit der Proben und vorbereiteten Reagenzien sind den entsprechenden Kapiteln zu entnehmen. Die Mikrotiterplatte ist auch nach dem Öffnen der Verpackung bis zu 6 w haltbar, wenn der Beutel sorgfältig wieder verschlossen und bei 2-8 C gelagert wird. 6. PROBENGEWINNUNG UND -AUFBEWAHRUNG Serum Die üblichen Vorsichtsmaßnahmen bei der Blutabnahme sind einzuhalten. Die chemische Integrität der Blutproben muss vom Zeitpunkt der Blutabnahme bis zur Testdurchführung erhalten bleiben. Keine hämolytischen, ikterischen oder lipämischen Proben verwenden. Fehler bei der Handhabung der Proben oder Abweichungen von der beschriebenen Testdurchführung können die Ergebnisse verfälschen. Die angegebenen Pipettiervolumina, Inkubationszeiten, Temperaturen und Vorbereitungsschritte sind unbedingt gemäß Arbeitsanleitung einzuhalten. Nur kalibrierte Pipetten und Geräte verwenden. 9 anti Sperm Ab ELISA (RE52029) DEUTSCH 2. Sobald mit der Testdurchführung begonnen wird, sollten alle Arbeitsschritte ohne Unterbrechung durchgeführt werden. Es ist sicherzustellen, dass alle benötigten Reagenzien, Geräte und Hilfsmittel zur rechten Zeit zur Verfügung stehen. Alle Reagenzien und Proben müssen auf Raumtemperatur (18-25 C) gebracht und vor Gebrauch vorsichtig ohne Schaumbildung gemischt werden. 3. Kontaminationen der Reagenzien, Pipetten und Wells/Röhrchen sind zu vermeiden. Neue Einmal- Pipettenspitzen für jedes Reagenz, jeden Standard und jede Probe verwenden. Die Deckel der Fläschchen nicht vertauschen. Nicht benötigte Fläschchen immer verschlossen halten. Wells/Röhrchen oder Reagenzien dürfen nicht wiederverwendet werden. 4. Enthalten Komponenten < 250 µl Flüssigkeit, sollte sich vor dem Öffnen der gesamte Inhalt auf dem Boden des entsprechenden Fläschchens befinden. 5. Es wird empfohlen, Doppelbestimmungen durchzuführen, um eventuelle Pipettierfehler zu erkennen. 6. Es sollte ein Pipettierschema verwendet werden um die Identifikation der Standards und Proben auf der Platte sicherzustellen. 7. Die Inkubationszeiten beeinflussen die Ergebnisse. Bei jedem Pipettierschritt sollten alle Wells in der gleichen Reihenfolge und im gleichen Zeittakt behandelt werden. Die Verwendung einer 8-Kanal- Mikropipette zum Pipettieren in alle Wells wird empfohlen. 8. Die korrekte Durchführung der Waschschritte ist entscheidend. Ungenügend gewaschene Wells ergeben falsche Ergebnisse. Die Verwendung einer Multikanalpipette oder eines automatischen Waschsystems für Mikrotiterplatten wird empfohlen. Alle Reagenzien müssen daher mit Vorsicht pipettiert werden. Beim Waschvorgang ist es wichtig, dass alle Wells vollständig und gleichmäßig mit Waschpuffer gefüllt werden und nach dem Ausschütteln kein Rückstand an Flüssigkeit zurückbleibt. 9. Feuchtigkeit beeinflusst die beschichteten Wells/Röhrchen. Verpackung nicht öffnen bevor Raumtemperatur erreicht ist. Kontrolle in jeweils 2 beschichtete und 2 unbeschichtete Wells pipettieren. 3. Platte mit Haftklebefolie abdecken. 1 h bei 37 C inkubieren. 4. Folie entfernen. Inkubationslösung verwerfen. 3 x mit jeweils 250 µl Waschpuffer waschen. Restliche Flüssigkeit auf Papiertüchern ausklopfen µl (frisch verdünntes) Enzymkonjugat pipettieren. 6. Platte mit neuer Folie abdecken. 1 h bei 37 C inkubieren. 7. Folie entfernen. Inkubationslösung verwerfen. 3 x mit jeweils 250 µl Waschpuffer waschen. Restliche Flüssigkeit auf Papiertüchern ausklopfen µl TMB-Substratlösung pipettieren min bei RT (18-25 C) inkubieren. 10. Substratreaktion durch Zugabe von 50 µl TMB-Stopplösung in jedes Well stoppen. Platte kurz schütteln. 11. Kurz schütteln und die Extinktion bei 450 nm (Referenzwellenlänge nm) innerhalb von 30 min. messen. 12. QUALITÄTSKONTROLLE Die Testergebnisse sind nur gültig, wenn der Rest gemäß der vorliegenden Arbeitsanleitung abgearbeitet wurde. Ferner muss der Anwender die GLP- Regeln (Good Laboratory Practice) und andere einschlägige Normen und Gesetze beachten. Alle Standards und Kit-Kontrollen müssen innerhalb der Akzeptanzbereiche, die auf dem QC-Zertifikat angegeben sind, gefunden werden. Wenn die Kriterien nicht erfüllt sind, sind die Ergebnisse ungültig und der Test sollte wiederholt werden. Jedes Labor sollte darüber hinaus eigene bekannte Proben als weitere Kontrollen mitführen. Es wird empfohlen, an den einschlägigen Ringversuchen teilzunehmen. Bei Abweichungen sind die folgenden Fehlermöglichkeiten zu überprüfen: Haltbarkeit der (vorbereiteten) Reagenzien, Lagerungsbedingungen, Pipetten, Geräte und Hilfsmittel, Inkubationsbedingungen und Waschmethoden. 13. TESTAUSWERTUNG Das Signal der beschichteten Wells für jeden Standard, jede Kontrolle und jede Probe um den Wert des entsprechenden NSB-Wells korrigieren. Mittelwerte berechnen. Die erhaltenen OD der Standards (y-achse, linear) gegen deren Konzentration (x-achse, logarithmisch) auftragen, entweder auf semi-logarithmischem Papier oder durch ein entsprechendes Computerprogramm. Bei Verwendung eines Computerprogramms werden die Cubic-Spline-Methode, 4-Parameter-Analyse (linlog) oder Logit-Log-Berechnung empfohlen. Zur Berechnung der Standardkurve sollten alle Werte der Standards verwendet werden (bei Doppelwerten kann ein offensichtlicher Ausreißerwert eliminiert und stattdessen der plausiblere Einzelwert verwendet werden). Die Konzentrationen der Proben können von der Standardkurve abgelesen werden. Wegen der Verdünnung der Proben im Assay müssen die erhaltenen Werte mit dem Faktor 0.5 multipliziert werden, um die Konzentration in U/mL in der unverdünnten Probe zu erhalten.
