1 Projekt Kinderhospital in Sincelejo/Kolumbien Das anliegende Projekt wurde an den Foundationbeauftragten des RC Apolda-Weimarer Land, Freund Eckart Maaß, von der in des Distrikts herangetragen. Freund Maaß reicht die Bitte aus Kolumbien um Beteiligung an dem vorgesehenen Grant an unseren weiter und schreibt: hier habe ich gleich noch ein Anliegen: Wir hatten ja ein Matching Grant mit Kolumbien. Die dortige in fragt jetzt wegen eines Partnerclubs für ein neues Projekt bei uns an. Es geht offenbar um Röntgentechnik für ein Universitätskrankenhaus, dem so etwas noch fehlt (aus deutscher Sicht fast undenkbar!). Der MG-Antrag ist schon fertig und war/ist beigefügt; den Schriftverkehr finden Sie unten. Der Gesamtaufwand sind US$. Gladys sucht einen Club mit 5780 $ und einen Distrikt mit 6760 $. Aufgrund unserer finanziellen Lage sind wir derzeit nicht imstande, zu helfen. Falls Sie einen Club wissen, geben Sie die Information doch bitte weiter. 2 Matching Grant Application The Rotary Foundation (TRF) PROJECT DESCRIPTION Explanation: Matching Grants support the humanitarian service projects of Rotary clubs and districts. In this section, describe in detail the humanitarian need your project will address, the intent of the project, how the project will be implemented, and how Rotarians will be directly involved in the project. Involvement is required of both the host and international partners. Please provide the name of the project site, the city or village, state or province, and country. List multiple locations, if applicable. Service of Pediatrics, University Hospital of Sincelejo (HUS) operates in the sub-site located in the center of the city of Sincelejo in Mother and Child Unit (IMU), which currently does not have an X-ray equipment in operation. This service creates a monthly average of 160 requests for radiographic studies of both hospitalization and emergency department, which must move children to the headquarters of HUS in the chaos of city traffic, compromising the physical integrity of children, plus the ability to destabilize its clinical picture, while he performed the procedure on other delayed by distance. 4 PROJECT FINANCING Explanation: Clearly list all financing in U.S. dollars (please use whole dollars as TRF does not match cents) noting which funds will be contributed in cash and which will be contributed from Designated Funds (DDF). Use of DDF must be authorized by the district Rotary Foundation committee chair and the district governor. TRF matches US$0.50 for every $1 cash contribution and $1 for every $1 contribution from DDF. The primary host club or district must provide at least $100 in either DDF or cash. NOTE: No funds should be sent to TRF prior to official Trustee approval. Upon approval, a letter will be sent to the sponsors notifying them of approval and providing detailed instructions on how and where to submit their contributions. Submitting cash contributions to the Foundation for a Matching Grant can significantly slow down the payment process and the contributions will only receive a 50% match as opposed to a 100% match for DDF. Speak with your district leaders to see if there is a way to utilize DDF as opposed to cash contributions for your application.
