2 1. VERWENDUNGSZWECK Der MutaREAL Helicobacter pylori (Clarithromycinresistenz) real time PCR-Test dient dem Nachweis von Helicobacter pylori im real time PCR-Kapillarsystem (z.b. LightCycler, Roche). Gleichzeitig wird der Resistenzstatus des Keims gegen das Antibiotikum Clarithromycin bestimmt. 2. EINLEITUNG Eine der Hauptursachen für chronische Gastritis und peptische Magenulcerationen ist die Besiedlung des Magens mit dem gram-negativen Mikroorganismus Helicobacter pylori. Die Therapie umfasst die Gabe verschiedener Antibiotika, eines der wichtigsten von ihnen ist das Clarithromycin. Die Resistenz von Helicobacter pylori gegenüber Clarithro-mycin ist somit eine häufige Ursache für erfolglose Eradikations-therapien (1). Es wurde nachgewiesen, dass Mutationen im 23-S rrna Gen (A-G Austausch an Nukleotidposition 2142 und 2143) für diese Resistenz verantwortlich sind (1-4). Die Erfolgsrate der Clarithromycinbehandlung von Helicobacter pylori bei Stämmen mit einer einzelnen Mutation liegt lediglich bei 40%, während die Erfolgsrate bei Wildtypstämmen bei 85% liegt (5). Mehrere molekularbiologische Tests sind in den letzten Jahren entwickelt worden, welche die Mutation im 23S rrna-gen von Helicobacter pylori nachweisen. Diese Tests basieren zumeist auf einer PCR mit anschließendem Restriktionsendonukleaseverdau. Die dabei resultierenden DNA-Fragmente werden elektrophoretisch (auf Agarose- oder Polyacrylamidgelen) aufgetrennt und die eventuelle Anwesenheit einer Mutation durch RFLP- Analyse festgestellt (5,6). Diese Nachweismethoden sind jedoch sehr zeitaufwändig und benötigen optimierte PCR-Bedingungen, da eine Kontamination mit Amplifikaten aus vorausgegangenen Experimenten zu falsch-positiven Ergebnissen führt. Ein schnellerer und verlässlicherer Nachweis der A2142G- und A2143G- Mutationen im 23-S rrna-gen von Helicobacter pylori, welcher aus Antrum- oder Corpus- Magenbiopsien (auch nach durchgeführtem CLO-Test) bzw. aus Stuhlproben isoliert wurde, kann mit Hilfe des MutaREAL Helicobacter pylori) - Kits durchgeführt werden, welcher im geschlossenen real time PCR-Kapillarsystem durchgeführt wird. Dabei werden die Mutationen im 23S rrna-gen in Echtzeit über Schmelzkurvenanalysen detektiert (innerhalb einer Stunde nach Testbeginn). Außerdem kann die Clarithromycinsensitivität, auch bei Doppelinfektionen mit clarithromycin-resistenten und clarithromycinsensitiven Helicobacter pylori Stämmen bestimmt werden. Der Kit enthält Materialien zur Untersuchung von 24 bzw. 96 Proben (nach DNA-Isolierung aus Magenbiopsien bzw. aus Stuhlproben). TESTPRINZIP Der MutaREAL Helicobacter pylori (Clarithromycinresistenz) - Kit enthält spezifische Primer und Hybridisierungssonden, sowie zusätzliches Material für die Untersuchung des 23S rrna-gens von Helicobacter pylori. Die an Position 2142 und 2143 vorhandene Base (je nach Mutation oder Wildtyp) ist durch die Bindungsaffinität der Hybridisierungssonden an das DNA-Template während der Schmelzkurvenanalyse charakterisiert. Die Identifikation der unterschiedlichen Genotypen in jeder einzelnen Probe wird im real time PCR-Kapillarsystem (z.b. LightCycler, Roche) ermöglicht. 4. KITBESTANDTEILE Jeder Testkit enthält Reagenzien zur Durchführung von 24 bzw. 96 Bestimmungen, sowie eine Gebrauchsanleitung. Ref. HINWEISE UND VORSICHTSMAßNAHMEN Nur zur in vitro Diagnostik verwenden. Dieser Test muss durch speziell geschultes Personal durchgeführt werden. Die klinischen Proben müssen als potentiell infektiöses Material angesehen und entsprechend auch entsorgt werden. Die Regeln der Good Laboratory Practice (GLP) müssen eingehalten werden. Testkit nach Ablauf des Verfalldatums nicht mehr verwenden. AMPLIFIKATION Die PCR-Technologie ist sehr sensitiv, d.h. die Amplifikation eines einzelnen Moleküls generiert Millionen identischer Kopien. Diese Kopien können in Form von Aerosolen entweichen und sich auf Unterlagen festsetzen. Um eine Kontamination von Proben mit Amplifikaten aus vorausgegangenen Experimenten zu vermeiden, sollte in zwei von einander (möglichst auch räumlich) getrennten Bereichen gearbeitet werden und zwar: 1) Bereich, in dem die Proben vorbereitet und die Reagenzien angesetzt werden 2) Bereich für die Amplifikation und deren Detektion Pipetten, Reaktionsgefäße und andere Arbeitsmittel sollten nicht innerhalb der verschiedenen Arbeitsbereiche zirkulieren. 5 8. TESTDURCHFÜHRUNG Die gesamte Durchführung ist in folgende vier Schritte aufgeteilt: A) Probenvorbereitung und DNA-Extraktion aus dem Ausgangsmaterial. B) Herstellung des gebrauchsfertigen Enzym-Mix für die real time PCR. C) Amplifikation und kombinierte Detektion (in Echtzeit) der entstehenden DNA Fragmente. D) Interpretation der Ergebnisse über die Software des real time PCR-Kapillarsystems. A) PROBENVORBEREITUNG UND DNA-EXTRAKTION 1. Verdau der Magenbiopsien mit Hilfe von Proteinase K: die Probe in ein steriles Reaktionsgefäß geben und 40 µl der Proteinase K-Lösung (A8), 40 µl Lysepuffer (A7) und 320 µl dh 2 O dazu pipettieren. Inkubation für 6 h bei 37 C im Thermoblock. 2. Isolierung der DNA mit Hilfe eines handelsüblichen Extraktionskits entsprechend den Angaben des Herstellers. Falls die DNA aus Stuhlproben isoliert wird, kann der erste Schritt weggelassen werden! 3. Falls die real time PCR nicht sofort durchgeführt wird, sollten die extrahierten DNA- Proben bei -20 C aufbewahrt werden. B) VORBEREITUNG DES GEBRAUCHSFERTIGEN ENZYM-MIX 1. Ein Röhrchen des Enzym-Mix (A1a) und ein Röhrchen des Enzympuffers (A1b) kurz bei rpm zentrifugieren, um die Lösungen am Gefäßboden zu sammeln. 2. Enzympuffer (A1b) quantitativ (60 µl) zum Enzym-Mix (A1a) zugeben. 3. Vorsichtig durch Auf- und Abpipettieren mischen (nicht vortexen!). Die nun vorliegende Lösung stellt den gebrauchsfertigen Enzym Mix dar (gekühlt aufbewahren). C) Real time Helicobacter pylori PCR-PROTOKOLL Es ist ratsam, das gesamte Protokoll zu lesen, bevor mit der Durchführung begonnen wird. Der Ansatz einer Gesamtmenge an Master-Mix für die jeweilige Anzahl an Proben und die entsprechenden Kontrollen sollte folgendermaßen erfolgen: 1. Die gebrauchsfertige Enzym-Mix Menge für eine Reaktion mit der Anzahl (N) der durchzuführenden Reaktionen (inklusive Kontrollen A4, A5 und A6) multiplizieren und zum Ausgleich der Pipettierungenauigkeit einen zusätzlichen (virtuellen) Ansatz berechnen. Mit allen Reagenzien in der gleichen Weise verfahren. Reagenzien während der Arbeitsschritte stets geeigent kühlen! Herstellung des Master-Mix: Die folgenden Reagenzien in einem sterilen Reaktionsröhrchen vorsichtig vermischen (NICHT VORTEXEN!): gebrauchsfertigen Enzym-Mix (A1a + A1b), Primer und Sonden-Mix (A2) und MgCl 2 (A3). Diese Mischung stellt den Master-Mix dar - kurz in einer Tischzentrifuge anzentrifugieren µl des Master-Mix mit Hilfe von Mikropipetten und sterilen Filterspitzen in jedes PCR-Reaktionsgefäß (z.b. LightCycler -Kapillare) pipettieren. 2 µl der Proben-DNA bzw. der Positiv- und Negativkontrollen (A4, A5 und A6) zu den jeweils korrespondierenden PCR-Reaktionsgefäßen zugeben (es empfiehlt sich zuerst die Negativkontrolle zu pipettieren und die Gefäße jeweils unverzüglich zu verschließen, um Kontaminationen zu vermeiden). Die Reagenzien in den PCR-Reaktionsgefäßen durch kurzes Zentrifugieren am Gefäßboden sammeln (für Kapillaren LightCycler Kapillar-Zentrifuge verwenden). Folgendes real time PCR - Protokoll durchführen: 95 C für 10 min 95 C für 0 sec 45 cycles 55 C für 10 sec ramping time: 20 C/sec aqu. mode: SINGLE 72 C für 15 sec 95 C für 0 sec 50 C für 5 sec ramping time: 0.1 C/sec aqu. mode: CONT 80 C für C) PCR-ANALYSE UND INTERPRETATION DER ERGEBNISSE 1. Durchführung der real time PCR im Kapillarsystem. 2. Schalten Sie den Color Compensation Filter ein, welcher für die gleichzeitige Nutzung der LC Red-640 und LC Red-705 markierten Hybridisierungssonden benötigt wird (durch Aktivierung des Felds Choose CCC File). 3. Das Ergebnis der Helicobacter pylori- Amplifikation wird in den Kanälen F2 und F3 angezeigt (s. Abb. 1 und Abb. 2). 4. Die Melting curve Analyse sollte mit folgenden Einstellungen durchgeführt werden: Digital filter: enabled Degrees to Average: Die Beurteilung der Messwerte und der zugehörigen Tm-Analyse werden nach Ende der PCR mit Hilfe des LightCycler Manual Tm Estimation Report durchgeführt, wobei der daraus resultierende Tm-Wert die Differenzierung zwischen Wildtyp- und Mutant- Genotypen ermöglicht. 6. Das Ergebnis ist für jede Probe in 2 verschieden Kanälen (F2 und F3) sichtbar und kann direkt mit den Ergebnissen der Wildtyp- und Mutant-Kontrolle verglichen werden (s. Abb. 3 und Abb. 4). 7. Vergleichen sie die Schmelzkurven der Proben mit den Schmelzkurven der beiden Kontrollen (Wildtyp und Mutation). Stand:

7 9. EINSCHRÄNKUNGEN Die real time PCR liefert für die Positivkontrollen und die Proben-DNA die im Beispiel angegebenen Schmelzkurven. Proben mit nicht eindeutigen Schmelzkurven (zu flacher Kurvenverlauf) müssen wiederholt werden. Verwenden Sie hierbei zur Orientierung die Amplituden der Kontroll-Schmelzkurven. Gegebenenfalls muss die Proben-DNA erneut getestet bzw. die Analyse mit neu isolierter DNA wiederholt werden. Bei fehlenden Positiv-Kontroll-Schmelzkurven erfolgte keine korrekte DNA- Amplifikation und die gewählten PCR-Bedingungen müssen daher überprüft und korrigiert werden. INTENDED USE The MutaREAL Helicobacter pylori (Clarithromycin resistance) real time PCR Kit is a screening assay for the detection of Helicobacter pylori and the synchronous determination of its resistance status to clarithromycin by real time detection in the capillary system of the LightCycler (Roche). 2. INTRODUCTION One of the major factors for chronic gastritis and peptidic ulcerations of stomach is the colonisation of the stomach by the gram-negative microorganism Helicobacter pylori. The therapy comprises application of certain antibiotics, of which one of the most important is clarithromycin. The resistance of Helicobacter pylori against clarithromycin is the major cause of failure in eradication therapies [1]. It was shown that mutations in the 23-S rrna gene (A-G change at nucleotide position 2142 or 2143) was responsible for the resistance of Helicobacter pylori against clarithromycin [1-4]. The succes rate for the clarithromycin treatment of Helicobacter pylori in strains with a single mutation is only 40%, but 85% in wild type strains [5]. Several molecular biology tests detecting the mutation in the 23-S rrna gene of Helicobacter pylori have been developed during the last few years, mainly as polymerase chain reaction (PCR) tests followed by restriction enzyme digestion. The resulting DNA fragments are separated on agarose or acrylamide gels and present mutation is detected by restriction fragment length polymorphism (RFLP) analysis [5, 6]. These detection methods are time consuming and need an optimization of PCR conditions. Moreover a contamination with amplified product can cause false-positive results. A rapid and reliable detection of the A2142G and A2143G mutations in the 23-S rrna gene of Helicobacter pylori isolated from antrum and corpus stomach biopsies (even after use in the CLOtest) can be performed with the MutaREAL Helicobacter pylori (clarithromycin resistance) kit. Using this LightCycler PCR test (the PCR process is owned by Hoffmann-La Roche Ltd.), the mutations in the 23-S rrna gene are detected in real time by melting curve analysis within only one hour. Also, the status of clarithromycin sensitivity is determined, even if a double infection with clarithromycin-resistant and clarithromycin-sensitive Helicobacter pylori is present. The kit contains materials for determination of 24 or respectively 96 samples (after isolation of DNA from stomach biopsies or also from stool samples). PRINCIPLE OF THE TEST The MutaREAL Helicobacter pylori (clarithromycin resistance) kit contains specific primers, FRET-hybridization probes and additional material for analysis of the Helicobacter pylori 23-S rrna gene. The base present at nucleotide position 2142 and 2143 (mutation or wildtype) is characterized by melting curve analysis determining the binding affinity of the FREThybridization probe to the DNA template. The identification of different genotypes in each single sample is conducted exclusively by real time PCR in the LightCycler. 4. 12 6. STORAGE AND HANDLING All reagents (A1 to A8) should be stored at 20 C. All reagents can be used until the expiration date printed on the labels. Do not freeze and thaw reagents A1, A2, A4, A5, A7 and A8 several times. Use LightCycler Cooling Block (Roche) or cool all reagents during the working steps. The Primer and Probe-Mix should be stored in the dark. 7. WARNINGS AND PRECAUTIONS For in vitro diagnostic use. This assay needs to be carried out by skilled personnel. Clinical samples should be regarded as potentially infectious materials. This assay needs to be run according to GLP (Good Laboratory Practice). Do not use the test kit after expiration date. The LightCycler-PCR process is owned by Hoffmann-La Roche Ltd. AMPLIFICATION The PCR technology is utmost sensitive. Thus, amplification of a single molecule generates millions of identical copies. These copies may evade through aerosols and sit on surfaces. In order to avoid contamination of samples with DNA which previously was amplified, it is important to physically strictly divide sample and reagent preparation units from sample amplification units. Set up two separate working areas: 1) Isolation of the DNA 2) Amplification/ detection of amplification products Pipets, vials and other working materials should not circulate among working units! Use always sterile pipette tips with filters Wear separate coats and gloves in each area Routinely decontaminate your pipettes and the laboratory benches with decontaminant Avoid aerosols 8. PROCEDURE The complete procedure is separated in four steps: A) Sample preparation and DNA extraction B) Preparation of the ready to use Enzyme-Mix C) Amplification and combined detection of DNA templates followed by melting curve analysis using hybridization probes (LightCycler -PCR) D) Interpretation of the genotypes using the LightCycler software and the melting curve analysis values Date: 2006/01/01 4 N:/WINDAT/AL/MB/ADT/eMRHP24-96

13 A) SAMPLE PREPARATION AND PREENRICHMENT 1) Digestion of stomach biopsies with proteinase K: transfer the sample into a sterile microtube and add 40 µl of proteinase K solution (A8), 40 µl lysis buffer (A7), and 320 µl DNAse-free water. Incubate for 6 h at 37 C in the thermoblock. 2) Extract genomic DNA by use of a commercial DNA isolation kit. DNA extraction from stool samples is directly made with these kits (skip first step). 3) If the LightCycler PCR is not performed immediately, store extracted DNA at < -20 C. B) PREPARATION OF THE READY TO USE ENZYME MIX 1) Centrifuge shortly one vial enzyme mix (A1a) and one vial enzyme buffer (A1b) at rpm in the table centrifuge. 2) Transfer 60µl of enzyme buffer (A1b) into the enzyme mix (A1a). 3) Mix carefully by pipetting (do NOT vortex!). This is the ready to use Enzyme Mix. C) Real time Helicobacter pylori PCR PROTOCOL Please carefully read the manufacturer`s instructions before starting the procedure! The PCR Master Mix volume for the respective number of samples and controls should be pipetted as follows: 1) The ready to use Enzyme Mix volume per reaction and sample (n) should be multiplied with the number of samples to be performed, including controls A4, A5 and A6. For reasons of unprecise pipetting, add an extra (virtual) sample. Proceed in the same manner with all additional reagents! Reaction volume Master Mix Volume 2 µl Enzyme Mix (ready to use) (A1a + b) 2 µl x (N+1) 14.4 µl Primer and Probe Mix (A2) 14.4 µl x (N+1) 1.6 µl MgCl 2 (A3) 1.6 µl x (N+1) 2) Mix gently (do NOT vortex!) the following reagents in a sterile tube: Enzyme Mix (ready to use; A1a + A1b), Primer and Probe Mix (A2) and MgCl 2 (A3). This mixture is the Master Mix. Spin down briefly in a table centrifuge. 3) Pipet 18 µl of Master Mix using micropipets with sterile filter tips in each of the LightCycler capillaries. Add 2 µl of the DNA sample or positive and negative controls (A4, A5 and A6) to each of these capillaries (it is recommended to pipet the negative control first to avoid contamination). 14 3) Perform the following LightCycler PCR protocol: 95 C for 10 min 95 C for 0 sec 45 cycles 55 C for 10 sec ramping time: 20 C/sec aqu. mode here: SINGLE 72 C for 15 sec 95 C for 0 sec 50 C for 5 sec ramping time: 0.1 C/sec aqu. mode: CONT 80 C D) ANALYSIS OF GENOTYPES AND INTERPRETATION OF RESULTS 1) Perform the LightCycler PCR. 2) Switch on the colour compensation filter (required because of the simultaneous use of LC Red-640 and LC Red-705 labelled hybridization probes) 3) After the run the quantification result is obtained in channels F2 and F3 (see Fig. 1 and 2). 4) The melting curve analysis should be done with following settings: Calculation method: Polynomial Digital filter: enabled Degrees to Average: ) The measured values are calculated after the end of the PCR and the subsequent Tmanalysis by the LightCycler Manual Tm Estimation Report. The resulting Tm allows the differentiation of wildtype and mutants 6) The result can be viewed in two different channels (channel F2 and F3) for each sample with the wildtype as well as the mutation control as index (see Fig. 3 and 4) 7) Compare melting curves of the samples with the melting curves of both controls (wild type and mutation) Samples with ambiguous melting curves (flat curvature) should be repeated.
