4 Einleitung Introduction 1. pezo-a ist ein Implantat zur thorakalen bis lumbosakralen intersomatischen Fusion bei Verwendung eines ventralen Zugangs. Die Cages dienen der Überbrückung und Stabilisierung des Zwischenwirbelraumes mit dem Ziel der Unterstützung der knöchernen Fusion. Die Rekonstruktion des Wirbelsäulenprofils wird u.a. durch die Größe, Höhe und Winkelung der Cages erreicht. Eine zusätzliche Instrumentierung z.b. mit einem dorsalen Pedikelschrauben- Stab-System wie tangors ist notwendig. Die Implantate werden einzeln in den Zwischenwirbelraum eingebracht. pezo-a ist Teil einer universellen Cage- Familie für dorsale und ventrale Zugänge. Ein entscheidender Produktvorteil ist die einheitliche Funktionsweise des Einsetzinstruments für alle Cages. Es bietet eine stabile und sichere Implantat- Instrumenten-Verbindung und erlaubt durch sein schlankes Design eine freie Sicht in den Situs. Durch die einfache Applikation sowie das schlüssige Röntgenmarkerkonzept ist eine zuverlässige Platzierung des Cages möglich. Neben dem optimierten Einsetzinstrument zeichnen sich die Cages durch ihr intelligentes Design aus. Die anatomisch-angepasste Form und durchdachte Zahnung des Implantats ist verantwortlich für das leichte Einsetzen und einen zuverlässigen und dauerhaften Implantatsitz. Die Cages sind befüllbar und bieten einen optimalen Durchbauungsraum. Die OP-Technik ist einfach und reproduzierbar aufgrund eines reduzierten und übersichtlichen Instrumentariums. Die Implantate werden einzeln steril verpackt geliefert. Zum Einsetzen der Implantate dürfen ausschließlich die dafür vorgesehenen Originalinstrumente des Implantatsystems verwendet werden. Die vorliegende OP-Technik beschreibt das Implantat und die Instrumente sowie die Arbeitsschritte für die Anwendung des pezo Systems. Sie ist als alleinige Grundlage für die erfolgreiche Anwendung nicht ausreichend. Es wird empfohlen, die Operationstechnik bei einem erfahrenen Operateur zu erlernen. Bitte beachten Sie die Wiederaufbereitungsanweisung für die Instrumente. pezo-a is an implant for thoracic to lumbosacral interbody fusion using an anterior approach. In addition to the optimized inserter the intelligent design of the cages is distinctive. The easy insertion and reliable and stable fit of the implant are attributable to its anatomically aligned shape and sophisticated toothing. The cages can be filled and provide optimal space for bony fusion. The surgical technique is simple and reproducible due to a set of instruments that is streamlined and easily managed. The implants are delivered in individual sterile packages. Only genuine instruments from the implant system which are exclusively designed for the purpose may be used for inserting the implants. This surgical technique describes the implant, the instruments, and the steps involved in the application of the pezo system. It is not sufficient as the sole basis for a successful application. It is recommended to master the surgical technique with an experienced surgeon. Please refer to the instructions for reprocessing treatment for the instruments. 5 Indikationen und Kontraindikationen Indications and contraindications Indikationen pezo ist eine Implantatgruppe zur Abstützung des Zwischenwirbelraumes im Bereich der thorakalen, lumbalen und lumbosakralen Wirbelsäule mit dem Ziel der Unterstützung der knöchernen Fusion der Wirbelkörper. Die Implantate können zugleich zur Wiederherstellung des Wirbelsäulenprofils dienen. pezo wird einzeln oder paarweise zwischen zwei Wirbelkörper eingebracht. Um eine Fusion zu erreichen, sollte das Implantat immer zusammen mit Knochen oder Knochenersatzmaterial in den Zwischenwirbelraum eingebracht werden. Der Hohlraum der Implantate sollte mit Knochen oder Knochenersatzmaterial befüllt werden. Die zusätzliche stabilisierende Instrumentierung zum Beispiel mit einem Pedikelschrauben-Stab-System ist erforderlich. Fusionsoperationen sind z. B. indiziert für: Degenerative Bandscheibenerkrankung Postdiskektomiesyndrom Revision nach Bandscheibenoperation Degenerative Instabilität Lytische Spondylolisthese Kongenitale Fehlstellung, z.b. Skoliose Spinalstenose mit klinischer Instabilität Pseudarthrose Kontraindikationen Patienten mit Fieber oder Leukozytose Patienten mit Wirbelsäule assoziierten Infektionen (z. B Spondylodiszitis) Patienten mit nachgewiesener Materialallergie oder Neigung zu Fremdkörperreaktionen Patienten ohne adäquate Compliance, die aufgrund ihres geistigen bzw. neurologischen Zustands nicht willens oder fähig sind, sich an die Nachsorgeanweisungen zu halten Patienten mit einem ungünstigen medizinischen oder psychologischen Allgemeinzustand, der durch den Eingriff weiter verschlechtert werden könnte; bei diesen Patienten ist eine sorgfältige Abwägung durch den behandelnden Arzt vorzunehmen Patienten mit unzureichender Knochenqualität oder -quantität, z.b. schwere Osteoporose, Osteopenie, Osteomyelitis Schwangerschaft Indications pezo is a group of implants for support of the intervertebral space in the thoracic, lumbar and lumbosacral spine with the purpose of supporting bony fusion of the vertebral bodies. The implants can also serve in the restoration of the spinal profile. pezo is inserted individually or in pairs between two vertebral bodies. To achieve fusion, the implant should always be inserted with bone or bone graft material into the intervertebral space. Distraktion des Bandscheibenfachs Um den Zwischenwirbelraum zu distrahieren, den Expander mit der flachen Seite in horizontaler Position einbringen und dann drehen. Angefangen mit der kleinsten Größe (Höhe 7 mm) in 1-mm-Abstufungen erhöhen, bis die gewünschte Höhe des Zwischenwirbelraums erreicht ist. Die Expander sind anatomisch geformt und passen sich somit den Endplatten an und können weit nach dorsal platziert werden. Wichtig: Verwendung eines Bildwandlers im seitlichen Strahlengang ist empfehlenswert. Anschliessend den T-Griff (CS 3346) oder den geraden Griff (CS 3347) auf den Halter montieren. Wichtig: Die Verbindung zwischen Probeimplantat und Halter fest fixieren. Montage von Implantat und Einsetzinstrument Die Implantatgröße entsprechend des eingesetzten Probeimplantats wählen. Das Implantat auf das Einsetzinstrument aufsetzen, so dass der erste Pin in die Bohrung greift. Mit Hilfe des Rändelrades den zweiten Pin vorschieben und das Implantat befestigen. Wichtig: Das Rändelrad bis zum Anschlag drehen.
