2 ENGLISH 1. INTENDED USE Enzyme immunoassay for the qualitative and quantitative determination of IgM antibodies to measles virus in human serum and plasma. 2. SUMMARY AND EXPLANATION Measles is a highly contagious viral disease characterized by a clinically distinct prodrome of fever, coryza, conjunctivitis, cough and a pathognomic exanthem (Koplik`s spots). The rash of Measles appears after a 3- to 5-days prodrome, some 14 days after exposure and may be associated with edema of the skin. The disease is the result of infection with the Measles Virus, genus Morbillivirus of the family Paramyxoviridae. Complications are: otitis media, pneumonia and encephalitis. Measles have a more severe expression in younger or undernourished children with a higher incidence of hemorrhage Measles, with 5% to 10% of lethal cases. Measles are one of the most contagious infectious diseases. The virus spreads through droplets emanating from the respiratory tract of infected persons or by direct contact. The incidence of Measles has declined since the introduction of vaccination programs. 3. TEST PRINCIPLE µ-capture enzyme immunoassay (ELISA). The wells of the microtiter strips are coated with anti-human IgM. IgM contained in the sample binds to the IgM class specific antibodies on the wells and measles-specific IgM is detected by an enzyme-conjugated antigen. After the substrate reaction the intensity of the color developed is proportional to the amount of IgM-specific antibodies detected. Results of samples can be determined directly using single-point calibration or via determination of a cut-off-value (COV). 4. WARNINGS AND PRECAUTIONS 1. For in-vitro diagnostic use only. For professional use only. 2. Before starting the assay, read the instructions completely and carefully. Use the valid version of the package insert provided with the kit. Be sure that everything is understood. 3. In case of severe damage of the kit package please contact IBL or your supplier in written form, latest one week after receiving the kit. Do not use damaged components in test runs, but keep safe for complaint related issues. 4. Observe lot number and expiry date. Do not mix reagents of different lots. Do not use expired reagents. 5. Follow good laboratory practice and safety guidelines. Wear lab coats, disposable latex gloves and protective glasses where necessary. 6. Reagents of this kit containing hazardous material may cause eye and skin irritations. See MATERIALS SUPPLIED and labels for details. Material Safety Data Sheets for this product are available on the IBL- Homepage or upon request directly from IBL. 7. Chemicals and prepared or used reagents have to be treated as hazardous waste according to national biohazard and safety guidelines or regulations. 8. Avoid contact with Stop solution. It may cause skin irritations and burns. 9. All reagents of this kit containing human serum or plasma have been tested and were found negative for HIV I/II, HBsAg and HCV. However, a presence of these or other infectious agents cannot be excluded absolutely and therefore reagents should be treated as potential biohazards in use and for disposal. 5. STORAGE AND STABILITY The kit is shipped at ambient temperature and should be stored at 2-8 C. Keep away from heat or direct sun light. The storage and stability of specimen and prepared reagents is stated in the corresponding chapters. The microtiter strips are stable up to the expiry date of the kit in the broken, but tightly closed bag when stored at 2 8 C. Version / 5
3 ENGLISH 6. SPECIMEN COLLECTION AND STORAGE Serum, Plasma (EDTA, Citrate) The usual precautions for venipuncture should be observed. It is important to preserve the chemical integrity of a blood specimen from the moment it is collected until it is assayed. Do not use grossly hemolytic, icteric or grossly lipemic specimens. Samples appearing turbid should be centrifuged before testing to remove any particulate material. Storage: 2-8 C -20 C Stability: 7 d > 7 d 7. Micropipettes (Multipette Eppendorf or similar devices, < 3% CV). Volumes: 5; 100; 500 µl 2. Calibrated measures 3. Tubes (1 ml) for sample dilution 4. 8-Channel Micropipettor with reagent reservoirs 5. Wash bottle, automated or semi-automated microtiter plate washing system 6. Microtiter plate reader capable of reading absorbance at 450 nm (reference wavelength nm) 7. Bidistilled or deionised water 8. Paper towels, pipette tips and timer 9. PROCEDURE NOTES 1. Any improper handling of samples or modification of the test procedure may influence the results. The indicated pipetting volumes, incubation times, temperatures and pretreatment steps have to be performed strictly according to the instructions. Use calibrated pipettes and devices only. 2. Once the test has been started, all steps should be completed without interruption. Make sure that required reagents, materials and devices are prepared ready at the appropriate time. Allow all reagents and specimens to reach room temperature (18-25 C) and gently swirl each vial of liquid reagent and sample before use. Mix reagents without foaming. 3. Avoid contamination of reagents, pipettes and wells/tubes. Use new disposable plastic pipette tips for each component and specimen. Do not interchange caps. Always cap not used vials. Do not reuse wells/tubes or reagents. 4. Use a pipetting scheme to verify an appropriate plate layout. Version / 5
4 ENGLISH 5. Incubation time affects results. All wells should be handled in the same order and time sequences. It is recommended to use an 8-channel Micropipettor for pipetting of solutions in all wells. 6. Microplate washing is important. Improperly washed wells will give erroneous results. It is recommended to use a multichannel pipette or an automatic microplate washing system. Do not allow the wells to dry between incubations. Do not scratch coated wells during rinsing and aspiration. Rinse and fill all reagents with care. While rinsing, check that all wells are filled precisely with Wash Buffer, and that there are no residues in the wells. 7. Humidity affects the coated wells/tubes. Do not open the pouch until it reaches room temperature. Unused wells/tubes should be returned immediately to the resealed pouch including the desiccant. 8. This assay can be processed on automated systems. See PERFORMANCE for details. 10. Note: Addition of RF-Adsorbens is not necessary, because of the µ-capture principle. 11. TEST PROCEDURE 1. Pipette 100 µl of calibrator and each control and diluted sample into the respective wells of the Microtiter Plate. For a valid assay performance the calibrator can be pipetted 3 times. 2. Cover plate with adhesive foil. Incubate 60 min at 37 C. 3. Remove adhesive foil. Discard incubation solution. Wash plate 3 x with 300 µl/well of diluted Wash Buffer. Remove excess solution by tapping the inverted plate on a paper towel. 4. Pipette 100 µl of working antigen conjugate / enzyme tracer solution into each well. 5. Cover plate with new adhesive foil. Incubate 60 min at 37 C. 6. Remove adhesive foil. Discard incubation solution. Wash plate 3 x with 300 µl/well of diluted Wash Buffer. Remove excess solution by tapping the inverted plate on a paper towel. 7. For adding of Substrate and Stop Solution use, if available, an 8-channel Micropipettor. Pipetting should be carried out in the same time intervals for Substrate and Stop Solution. Use positive displacement and avoid formation of air bubbles. 8. Pipette 100 µl of TMB Substrate Solution into each well. 9. Incubate 30 min at C in the dark (without adhesive foil). 10. Stop the substrate reaction by adding 100 µl of TMB Stop Solution into each well. Color changes from blue to yellow. 11. Measure optical density with a photometer at 450 nm (Reference-wavelength: nm) within 60 min after pipetting of the Stop Solution. 12. QUALITY CONTROL The test results are only valid if the test has been performed following the instructions. Moreover the user must strictly adhere to the rules of GLP (Good Laboratory Practice) or other applicable standards/laws. All Version / 5
5 ENGLISH kit controls must be found within the acceptable ranges as stated on the vial labels. If the criteria are not met, the run is not valid and should be repeated. Each laboratory should use known samples as further controls. In case of any deviation the following technical issues should be proven: Expiration dates of (prepared) reagents, storage conditions, pipettes, devices, incubation conditions and washing methods. 13. CALCULATION OF RESULTS The evaluation of the test can be performed either quantitatively or qualitatively. For a qualitative evaluation, a cut-off value is determined using a lot-specific factor (LSF) which is given on the QC certificate. Quantitative results are achieved using a single-point calibration software. This software is either installed on the ELISA reader used or can be downloaded from our homepage ( IBL software module single-point calibration ). Upon request, IBL can also provide you with the software module directly. The software module works with Excel 2000 or superior Quantitative evaluation using single-point calibration: Quantitative determination of results using single-point calibration is performed via a sigmoid standard curve described by a 4 parameter curve fit. The four parameters A, B, C and D given on the QC certificate of the kit are fed into the single-point calibration software of the ELISA reader or into the IBL software module single-point calibration. A = lower limit (OD) B = turning slope C = turning point (conc.) D = upper limit (OD) In order to adjust the OD values of samples and controls for each test run the target value of the calibrator given on the QC certificate of the kit has to be entered into the software. Using the target value the test specific correction factor F is calculated. F is multiplied with each single OD value measured for the samples and the controls. A possible outlier should be eliminated. The concentration of samples and results is calculated by the software using the corrected OD values and the four parameters and the following formula: Concentration Sample = C OD A D Sample adjusted D The sample dilution of 1:101 has already been reconsidered in the calculation procedure. If higher dilutions are used, results have to be multiplied by the additional dilution factor. They have to be correlated to other clinical observations and diagnostic tests. 15. LIMITATIONS OF THE PROCEDURE Specimen collection has a significant effect on the test results. See SPECIMEN COLLECTION AND STORAGE for details. For cross-reactivities, see PERFORMANCE. Azide and thimerosal at concentrations > 0.1 % interfere in this assay and may lead to false results. 7 DEUTSCH 1. ZWECKBESTIMMUNG Enzymimmunoassay zur qualitativen und quantitativen Bestimmung von IgM Antikörpern gegen das Masernvirus in humanem Serum und Plasma. 2. KLINISCHE BEDEUTUNG Masern stellen eine stark ansteckende Viruserkrankung dar, die klinisch durch ein ausgeprägtes prodromales Fieber, Schnupfen, Konjunktivitis, Husten und ein pathognomisches Exanthem (Koplik`sche Flecken) gekennzeichnet ist. Die Krankheit entsteht durch eine Infektion mit dem Masernvirus, das zu dem Genus Morbillivirus und der Familie Paramyxoviridae gehört. Zehn bis zwölf Tage nach der Infektion erscheinen die sehr deutlichen und charakteristischen prodromalen Symptome: Schnupfen, persistierender bellender Husten, Keratokonjunktivitis, oft verbunden mit Photophobie und Fieber. Im allgemeinen treten auch Lymphadenopathie und Splenomegalie auf. Während dieser Phase erscheinen Koplik`sche Flecken auf der Wangenschleimhaut, die sich schnell ausbreiten. Diese Flecken sind in der Regel verschwunden, wenn die Hautrötung ihren Höhepunkt erreicht. Die Hautrötung tritt nach einem 3-5 tägigen Prodromalstadium auf, etwa 14 Tage nach der Ansteckung. Der Ausschlag entwickelt sich makulopapulär und breitet sich innerhalb der nächsten 3 Tage schnell über Gesicht, Hals, Rumpf und Extremitäten aus. Auf dem Höhepunkt zeigt sich eine tiefrote Farbe, die mit einem Ödem der Haut assoziiert werden kann. Komplikationen bei Masern können eine Folge der Virusreplikation oder von sekundären bakteriellen Infektionen sein. Die häufigsten Komplikationen sind: Otitis media, Pneumonie und Enzephalitis. Komplikationen durch Masern sind bei jüngeren oder unterernährten Kindern ausgeprägter; hier treten auch hämorrhagische Bilder mit 5% bis 10% an letalen Fällen auf. Masern sind eine der ansteckensten Infektionskrankheiten. Das Virus breitet sich über Tröpfchen aus dem Respirationstrakt von infizierten Personen oder durch direkten Kontakt aus. Das Vorkommen von Masern hat seit der Einführung von Impfprogrammen abgenommen. 3. TESTPRINZIP µ-capture Enzymimmunoassay (ELISA). Die Wells der Mikrotiterstreifen sind mit anti-human- IgM Antikörper beschichtet. Die IgM- Antikörper der Probe binden an die IgM- klassenspezifischen Antikörpern in den Wells und werden von dem enzymmarkierten Antigen detektiert. Die Intensität der durch das Enzym in der Substratreaktion gebildeten Farbe ist proportional zur IgM-Konzentration. Die Ergebnisse der Proben können direkt anhand der Ein-Punkt-Kalibration bestimmt werden oder über den Cut-Off-Wert (COV). 4. WARNHINWEISE UND VORSICHTSMASSNAHMEN 1. Nur zum In-vitro-Gebrauch. Nur für den Gebrauch durch Fachpersonal. 2. Vor der Testdurchführung sollte die Arbeitsanleitung vollständig und sorgfältig gelesen werden und verstanden worden sein. Die gültige Version aus dem Kit verwenden. 3. Im Falle einer erheblichen Beschädigung der Testpackung ist IBL bzw. der jeweilige Lieferant innerhalb einer Woche nach Empfang der Ware schriftlich zu benachrichtigen. Beschädigte Komponenten dürfen nicht zur Testdurchführung verwendet werden, sondern sollten solange aufbewahrt werden, bis der Transportschaden endgültig geregelt ist. 4. Chargen-Nummer und Verfallsdatum beachten. Es dürfen keine Reagenzien aus unterschiedlichen Chargen in einem Test verwendet werden. Verfallene Reagenzien dürfen nicht verwendet werden. 5. Gute Laborpraxis und Sicherheitsrichtlinien beachten. Je nach Bedarf sollten Laborkittel, Einmal- Latexhandschuhe und Schutzbrillen getragen werden. 6. Reagenzien dieses Kits, die Gefahrstoffe enthalten, können Reizungen der Augen und der Haut hervorrufen. Siehe Angaben in KOMPONENTEN DES KITS und auf den Etiketten. Sicherheitsdatenblätter für dieses Produkt sind auf der IBL-Homepage zum Download verfügbar oder auf Anfrage direkt von IBL erhältlich. 7. Chemikalien und vorbereitete oder gebrauchte Reagenzien sind unter Beachtung der jeweiligen nationalen Bestimmungen als Gefahrstoffabfall zu entsorgen. 8. Kontakt mit Stopplösung vermeiden. Kann Hautreizungen und Verätzungen hervorrufen. 9. Alle Reagenzien dieses Kits, die humanes Serum oder Plasma enthalten, ergaben bei der Prüfung auf HCV, HBsAg bzw. Antikörper gegen HIV I/II-Virus ein negatives Ergebnis. Trotzdem kann das Vorhandensein solcher infektiöser Erreger nicht mit absoluter Sicherheit ausgeschlossen werden. Die Reagenzien sollten deshalb wie potentiell infektiöses Material behandelt werden. Version / 6
8 DEUTSCH 5. LAGERUNG UND HALTBARKEIT Der Kit wird bei Umgebungstemperatur angeliefert und sollte bei 2-8 C gelagert werden. Vor Hitze und direkter Sonneneinstrahlung schützen. Hinweise zur Lagerung und Haltbarkeit der Proben und vorbereiteten Reagenzien sind den entsprechenden Kapiteln zu entnehmen. Die Mikrotiterplatte ist auch nach dem Öffnen der Verpackung bis zum Verfallsdatum des Kits haltbar, wenn der Beutel sorgfältig wieder verschlossen und bei 2-8 C gelagert wird. 6. PROBENGEWINNUNG UND -AUFBEWAHRUNG Serum, Plasma (EDTA, Citrat) Die üblichen Vorsichtsmaßnahmen bei der Blutabnahme sind einzuhalten. Die chemische Integrität der Blutproben muss vom Zeitpunkt der Blutabnahme bis zur Testdurchführung erhalten bleiben. Keine hämolytischen, ikterischen oder lipämischen Proben verwenden. Getrübte Proben sollten vor der Testdurchführung zentrifugiert werden, um Partikel zu entfernen. Lagerung: 2-8 C -20 C Haltbarkeit: 7 d > 7 d 7. 9 DEUTSCH 9. HINWEISE ZUR TESTDURCHFÜHRUNG 1. Fehler bei der Handhabung der Proben oder Abweichungen von der beschriebenen Testdurchführung können die Ergebnisse verfälschen. Die angegebenen Pipettiervolumina, Inkubationszeiten, Temperaturen und Vorbereitungsschritte sind unbedingt gemäß Arbeitsanleitung einzuhalten. Nur kalibrierte Pipetten und Geräte verwenden. 2. Sobald mit der Testdurchführung begonnen wird, sollten alle Arbeitsschritte ohne Unterbrechung durchgeführt werden. Es ist sicherzustellen, dass alle benötigten Reagenzien, Geräte und Hilfsmittel zur rechten Zeit zur Verfügung stehen. Alle Reagenzien und Proben müssen auf Raumtemperatur (18-25 C) gebracht und vor Gebrauch vorsichtig ohne Schaumbildung gemischt werden. 3. Kontaminationen der Reagenzien, Pipetten und Wells/Röhrchen sind zu vermeiden. Neue Einmal- Pipettenspitzen für jede zu pipettierende Komponente und jede Probe verwenden. Die Deckel der Fläschchen nicht vertauschen. Nicht benötigte Fläschchen immer verschlossen halten. Wells/Röhrchen oder Reagenzien dürfen nicht wiederverwendet werden. 4. Es sollte ein Pipettierschema verwendet werden um die Identifikation der Standards und Proben auf der Platte sicherzustellen. 5. Die Inkubationszeiten beeinflussen die Ergebnisse. Bei jedem Pipettierschritt sollten alle Wells in der gleichen Reihenfolge und im gleichen Zeittakt behandelt werden. Die Verwendung einer 8-Kanal- Mikropipette zum Pipettieren in alle Wells wird empfohlen. 6. Die korrekte Durchführung der Waschschritte ist entscheidend. Ungenügend gewaschene Wells ergeben falsche Ergebnisse. Die Verwendung einer Multikanalpipette oder eines automatischen Waschsystems für Mikrotiterplatten wird empfohlen. Zwischen den Inkubationen die Wells nicht austrocknen lassen. Beim Waschen und Ausschütteln dürfen die beschichteten Wells nicht beschädigt werden. Alle Reagenzien müssen daher mit Vorsicht pipettiert werden. Beim Waschvorgang ist es wichtig, dass alle Wells vollständig und gleichmäßig mit Waschpuffer gefüllt werden und nach dem Ausschütteln kein Rückstand an Flüssigkeit zurückbleibt. 7. Feuchtigkeit beeinflusst die beschichteten Wells/Röhrchen. Verpackung nicht öffnen bevor Raumtemperatur erreicht ist. Nicht benötigte Wells/Röhrchen sofort in den wiederverschließbaren Beutel mit Trockenmittel zurückgeben. 8. Dieser Assay kann auf Automaten abgearbeitet werden. Siehe TESTCHARAKTERISTIKA. 10. 10 DEUTSCH 11. TESTDURCHFÜHRUNG 1. Je 100 µl vom Kalibrator, jeder Kontrolle und jeder verdünnten Probe in die entsprechenden Wells der Mikrotiterplatte pipettieren. Für eine valide Testdurchführung kann der Kalibrator in 3-fach- Bestimmung pipettiert werden. 2. Platte mit Haftklebefolie abdecken. 60 min bei 37 C inkubieren. 3. Folie entfernen. Inkubationslösung verwerfen. Platte 3 x mit je 300 µl/well verdünntem Waschpuffer waschen. Restliche Flüssigkeit auf Papiertüchern ausklopfen µl fertige Antigenkonjugat / Enzymtracer-Mischung in jedes Well pipettieren. 5. Platte mit neuer Folie abdecken. 60 min bei 37 C inkubieren. 6. Folie entfernen. Inkubationslösung verwerfen. Platte 3 x mit je 300 µl/well verdünntem Waschpuffer waschen. Restliche Flüssigkeit auf Papiertüchern ausklopfen. 7. Substrat- und Stopplösung möglichst mit einer 8-Kanal-Pipette pipettieren und Substrat- und Stopplösung in denselben Zeitintervallen zugeben. Mit positivem Vorhub pipettieren, um die Bildung von Luftbläschen zu vermeiden µl TMB Substratlösung in jedes Well pipettieren min bei RT (18-25 C) im Dunkeln inkubieren. (Ohne Haftklebefolie.) 10. Die Substratreaktion durch Zugabe von 100 µl TMB Stopplösung in jedes Well stoppen. Die Farbe wechselt von blau nach gelb. 11. Die optische Dichte mit einem Photometer innerhalb von 60 min nach Zugabe der Stopplösung bei 450 nm messen (Referenzwellenlänge: nm). 12. QUALITÄTSKONTROLLE Die Testergebnisse sind nur gültig, wenn der Test gemäß der vorliegenden Arbeitsanleitung abgearbeitet wurde. Ferner muss der Anwender die GLP- Regeln (Good Laboratory Practice) und andere einschlägige Normen und Gesetze beachten. Alle Kit-Kontrollen müssen innerhalb der Akzeptanzbereiche, die auf den Etiketten angegeben sind, gefunden werden. Wenn die Kriterien nicht erfüllt sind, sind die Ergebnisse ungültig und der Test sollte wiederholt werden. Jedes Labor sollte darüber hinaus laborinterne Kontrollen mitführen. Bei Abweichungen sind die folgenden Fehlermöglichkeiten zu überprüfen: Haltbarkeit der (vorbereiteten) Reagenzien, Lagerungsbedingungen, Pipetten, Geräte und Hilfsmittel, Inkubationsbedingungen und Waschmethoden. 13. TESTAUSWERTUNG Die Testauswertung kann wahlweise über eine quantitative Konzentrationsbestimmung mit der Ein-Punkt- Kalibrationssoftware oder qualitativ über die Bestimmung des Cut Off Wertes mit Hilfe des auf dem QC- Zertifikat angegebenen Lot-spezifischen Faktors (LSF auf QC-Zertifikat) erfolgen. Die Ein-Punkt-Kalibrationssoftware ist entweder in der ELISA-Reader Software vorhanden oder kann als IBL-Software Modul Ein-Punkt-Kalibration von der IBL-Homepage herunter geladen werden bzw. wird auf Anfrage kostenlos von IBL elektronisch oder als CD zur Verfügung gestellt. Die Auswertung mit dem IBL- Software Modul Ein-Punkt-Kalibration erfolgt mit Excel 2000 oder höher Quantitative Auswertung über die Ein-Punkt-Kalibration: Die quantitative Konzentrationsbestimmung der Proben mittels der Ein-Punkt-Kalibrationssoftware erfolgt anhand einer sigmoiden Standardkurve, die mit der 4-Parameter-Funktion beschrieben wird. Die 4 Parameter A, B, C und D, die auf dem QC-Zertifikat des Kits angegeben sind, werden in das Ein-Punkt- Kalibrations Programm des ELISA-Readers bzw. des IBL-Software Moduls Ein-Punkt-Kalibraton eingegeben. A = lower limit (OD) B = turning slope C = turning point (Konz.) D = upper limit (OD) Zur Anpassung der OD-Werte von Proben und Kontrollen für jeden Testlauf muss zusätzlich in das Programm der Ein-Punkt-Kalibration der im Kit angegebene OD-Sollwert des Kalibrators eingegeben werden. Mit Hilfe des OD-Sollwertes des Kalibrators wird der testspezifische Korrekturfaktor F berechnet, mit dem alle gemessenen OD-Werte der Proben und Kontrollen in jedem Testansatz korrigiert werden. Die Konzentration der Proben und Kontrollen wird mit den korrigierten OD-Werten und den 4 Parametern nach der folgenden Formel von der Ein-Punkt-Kalibrationssoftware berechnet: A D Konzentration Pr obe C = 1 OD Probe korrigiert D Die standardmäßig durchzuführende Verdünnung von 1:101 ist in dem oben beschriebenen Auswerteverfahren bereits berücksichtigt. Wenn Proben stärker verdünnt wurden, müssen die Ergebnisse mit dem entsprechenden Verdünnungsfaktor multipliziert werden. Die Ergebnisse werden in U/mL angegeben Quantitative und Qualitative Auswertung über den Cut-Off (CO): Für die quantitative und qualitative Auswertung wird in jedem Testlauf der Cut Off (CO) durch Multiplikation des Mittelwertes der OD-Messwerte des Kalibrators mit einem Lot-spezifischen-Faktor (LSF, siehe QC- Zertifikat) ermittelt.
