2 1. Verwendungszweck Für die in-vitro Diagnostik. RIDA GENE EAEC ist eine real-time PCR zum direkten qualitativen Nachweis von enteroaggregativen E. coli (EAEC) in humanen Stuhlproben. 1,2 Die RIDA GENE EAEC real-time PCR soll die Diagnose einer durch enteroaggregative E. coli. verursachten Gastroenteritis unterstützen. 2. Erläuterung Escherichia coli (E. coli) sind gram-negative, durch peritriche Begeißelung bewegliche, fakultativ anaerobe Stäbchenbakterien und gehören zur Familie der Enterobacteriaceae. E. coli ist Bestandteil der normalen Darmflora des Menschen, aber auch vieler landwirtschaftlicher Nutztiere und ist in der Regel apathogen. Einige Stämme von E. coli sind durch den Erwerb von bestimmten Pathogenitätsfaktoren (z.b. Toxingene) humanpathogen. Die sechs bekannten darmpathogenen E. coli: enterohämorrhagische E. coli (EHEC), enteropathogene E. coli (EPEC), enterotoxische E. coli (ETEC), enteroinvasive E. coli (EIEC), enteroaggregative E. coli (EAEC) und diffus adhärente E. coli (DAEC) lassen sich durch die spezifischen Pathogenitätsfaktoren differenzieren. 3 Enteroaggregative E. coli (EAEC) wurden erstmals 1987 im Stuhl eines Kindes aus Chile beschrieben. 4 Charakteristisches Merkmal der EAEC ist die aggregative Adhärenz (AA). Im Goldstandard HEp-2 Zelladhärenz Assay adhärieren EAEC an die Epithelzellen in einem sogenannten "stacked-brick" Muster (gestapelte Backsteine). Definiert werden EAEC als E. coli, die keine hitzelabile (LT) oder hitzestabilen (ST) Enterotoxine sezernieren und sich an HEp-2 Zellen in einem AA-Muster adhärieren. 5 Die meisten EAEC-Stämme beherbergen ein Virulenzplasmid (paa), auf dem eine Vielzahl von Virulenzgenen (z.b. aggr, agga, aafa, agg3 and aata) lokalisiert sind, die mit der AA in Verbindung gebracht werden. 6,7,8 Das aata Gen (Anti-Aggregationsprotein-Transportergen, früher EAEC-Sonde bzw. CVD 432 genannt) sowie das aggr Gen (Hauptregulator der EAEC-Plasmid-Virulenzgene) werden zum EAEC-Nachweis mittels PCR verwendet. 8,9,10,11 EAEC-Stämme, die das paa exprimieren, werden als typische EAEC, solche die es nicht exprimieren, als atypische EAEC bezeichnet. 12 Die häufigste klinische Manifestation einer EAEC-Infektion ist wässriger Durchfall. Weniger häufige klinische Symptome sind leichtes Fieber, Übelkeit, Erbrechen, Bauchschmerzen und das Vorhandensein von Blut im Stuhl, Schleim oder Leukozyten. Die Inkubationszeit reicht von 8 bis 18 h. In einer Studie mit gesunden Probanden verursachte die orale Gabe von EAEC c.f.u. Durchfall. 2 RIDA GENE EAEC
3 Durch die hohe erforderliche Infektionsdosis wird eine fäkal-orale Übertragung von EAEC durch Lebensmittel oder Wasser vermutet. EAEC sind die Ursache von akuten und chronischen (> 14 Tage) Durchfällen bei Kindern, Erwachsenen und HIVinfizierten Personen, sowohl in Entwicklungs- als auch in Industrieländern. 8 Es sind nach ETEC die zweithäufigsten Erreger einer Reisediarrhoe bei Reisenden in Entwicklungsländern wie Mexiko, Indien und Jamaika. 13 EAEC Ausbrüche werden meist mit dem Verzehr von kontaminierten Lebensmitteln in Verbindung gebracht. 14 EAEC sind bei 2% - 68% der Patienten mit Durchfall und bei 0% bis 15% der Kontrollen aus Indien, Südamerika, Europa und dem Nahen Osten isoliert worden. 13 In einer Meta-Analyse von veröffentlichten Studien wurde gezeigt, dass EAEC für 15% aller akuten Diarrhoen bei Kindern in Entwicklungsländern und für 4% in Industrieländern verantwortlich ist. 15 In Deutschland wurden EAEC in 2% der pädiatrischen Patienten mit Durchfall isoliert. In der gesunden Kontrollgruppe konnten keine EAEC nachgewiesen werden. 12 Bei Schweizer Kindern wurde EAEC in 10,2% der Kinder mit Durchfall, verglichen mit 2,2% der Kinder ohne Durchfall isoliert. In den USA wurde EAEC in 4,5% der Patienten mit Diarrhoe und in 1,7% der asymptomatischen Patienten nachgewiesen Testprinzip RIDA GENE EAEC ist eine real-time PCR zum direkten qualitativen Nachweis enteroaggregativer E. coli (EAEC). Nach der DNA-Isolierung wird (falls vorhanden) die für EAEC spezifischen Genfragemente (aata, aggr) amplifiziert. Die amplifizierten Zielsequenzen werden mit Hydrolyse-Sonden, die an einem Ende mit dem Quencher und am anderen Ende mit einem Reporter-Fluoreszenzfarbstoff (Fluorophor) markiert sind, nachgewiesen. In Gegenwart einer Zielsequenz hybridisieren die Sonden mit den Amplikons. Während der Extension trennt die Taq-Polymerase den Reporter vom Quencher. Der Reporter emittiert ein Fluoreszenzsignal, das durch die optische Einheit eines real-time PCR-Gerätes detektiert wird. Das Fluoreszenzsignal steigt mit der Menge der gebildeten Amplikons an. Der RIDA GENE EAEC Test enthält eine Internal Control DNA (ICD), um die Probenpräparation und/oder eine potentielle PCR Inhibition kontrollieren zu können. RIDA GENE EAEC

Reagenzien und ihre Lagerung - Alle Reagenzien müssen lichtgeschützt bei -20 C gelagert werden und können bis zum aufgedruckten Verfallsdatum verwendet werden. Nach Erreichen des Verfallsdatums kann keine Qualitätsgarantie mehr übernommen werden. - Vor dem Gebrauch sollten die Reagenzien schonend aufgetaut werden (z.b. im Kühlschrank bei 2-8 C). - Ein wiederholtes Einfrieren/Auftauen bis zu 5 Mal beeinträchtigt die Testeigenschaft nicht (ggf. Aliquots nach dem ersten Auftauen herstellen und die Reagenzien sofort wieder einfrieren). - Alle Reagenzien während der PCR-Vorbereitung geeignet kühlen (2-8 C). 4 RIDA GENE EAEC

Vorsichtsmaßnahmen - Nur für die in-vitro Diagnostik. - Eine räumliche Trennung von Extraktion, PCR-Ansatz und PCR ist zu beachten, um Querkontaminationen zu vermeiden. - Dieser Test ist nur von molekularbiologisch geschultem Laborpersonal durchzuführen. - Die Gebrauchsanweisung zur Durchführung des Tests ist strikt einzuhalten. - Während des Umgangs mit Proben Einmalhandschuhe tragen und nach Abschluss des Tests die Hände waschen. - In den Bereichen, in denen mit Proben gearbeitet wird, nicht rauchen, essen oder trinken. - Klinische Proben müssen als potentiell infektiös angesehen werden und müssen wie sämtliche Reagenzien und Materialien, die mit potentiell infektiösen Proben zusammenkommen, entsprechend entsorgt werden. - Testkit nach Erreichen des Verfallsdatums nicht mehr verwenden. 6 8. Testdurchführung 8.1 Probenvorbereitung DNA-Isolierung aus Stuhlproben Für die DNA-Isolierung aus Stuhlproben wird ein kommerziell erhältliches DNA-Isolierungskit (z.b. RTP Pathogen Kit (STRATEC Molecular)) oder DNA-Extraktionsystem (z.b. Maxwell 16 (Promega)) empfohlen. Die Angaben des Herstellers sind zu beachten. Es wird empfohlen die Stuhlproben vor der Extraktion 1:3 mit Wasser zu verdünnen, stark zu vortexen und 30 sec bei rpm zu zentrifugieren. Aus dem Überstand das entsprechende Volumen nach Angaben des Herstellers verwenden. Der RIDA GENE EAEC Test enthält eine Internal Control DNA (ICD), die entweder nur als Inhibitionskontrolle oder als Extraktionskontrolle für die Probenpräparation und Inhibitionskontrolle verwendet werden kann. Wird die Internal Control DNA (ICD) nur als Inhibitionskontrolle verwendet, muss 1 µl der ICD dem Master-Mix hinzugefügt werden (s. Tab.3). Wird die Internal Control DNA (ICD) als Extraktionskontrolle für die Probenpräparation und als Inhibitionskontrolle verwendet, müssen 20 µl der ICD während der Extraktion eingesetzt werden. Die ICD soll dem Proben-Lysispuffer Mix und nicht direkt dem Probenmaterial zugefügt werden. 8.2 Herstellung des Master-Mix Die Gesamtzahl der für die PCR benötigten Reaktionen (Proben und Kontrollreaktionen) ist zu berechnen. Bei jedem Testlauf muss eine Positiv- und eine Negativkontrolle mitgeführt werden. Es wird empfohlen den Master-Mix mit 10 % zusätzlichem Volumen anzusetzen, um einen Pipettierverlust auszugleichen (s. Tab.2, Tab.3). Vor der Benutzung den Reaction Mix, die Taq-Polymerase, die Positive Control, das PCR Water und die ICD auftauen, durchmischen und kurz zentrifugieren. Reagenzien während der Arbeitsschritte stets geeignet kühlen (2 8 C). 6 RIDA GENE EAEC

Negativkontrolle: Je 5 µl PCR Water zum vorgelegten Master-Mix als Negativkontrolle pipettieren. Hinweis: Wir empfehlen bei Verwendung der ICD als Extraktionskontrolle für die Probenpräparation und die Inhibitionskontrolle je 1 µl der ICD zum PCR-Mix der Negativkontrolle zu pipettieren. Proben: Positivkontrolle: Je 5 µl DNA-Extrakt zum vorgelegten Master-Mix der Probenreaktionen pipettieren. Je 5 µl Positive Control zum vorgelegten Master-Mix in die dafür vorgesehenen Reaktionsgefäße pipettieren. Hinweis: Wir empfehlen bei Verwendung der ICD als Extraktionskontrolle für die Probenpräparation und die Inhibitionskontrolle je 1 µl der ICD zum PCR-Mix der Positivkontrolle zu pipettieren. 8 Reaktionsgefäße bzw. Platte verschließen, mit wenigen Umdrehungen pro Minute kurz abzentrifugieren und in das real-time PCR-Gerät überführen. Die PCR entsprechend der Geräteeinstellung starten (s. Negativ- und Positivkontrollen müssen die korrekten Ergebnisse zeigen (s. Abb.1). Die Positivkontrolle liegt in einer Konzentration von 10 3 Kopien / µl vor. Sie wird in einer Gesamtmenge von 5 x 10 3 Kopien in jedem PCR Lauf eingesetzt. RIDA GENE EAEC

Eine Inhibierung der PCR-Reaktion bzw. ein Fehler im Extraktionsverfahren kann durch die Detektion der Internal Control DNA (ICD) ausgeschlossen werden. 10 RIDA GENE EAEC
11 Eine Probe wird positiv bewertet, wenn die Probe eine Amplifikation im Nachweissystem und in der dazugehörigen Internal Control DNA (ICD) zeigt. Eine Probe wird positiv bewertet, wenn die Probe eine Amplifikation im Nachweissystem, jedoch keine für die Internal Control DNA (ICD) zeigt. Der Nachweis der Internal Control DNA (ICD) ist in diesem Fall nicht notwendig, da hohe Konzentrationen des Amplikons zu einem schwachen oder fehlenden Signal der Internal Control DNA (ICD) führen können. Eine Probe ist nicht auswertbar, wenn die Probe und die Internal Control DNA (ICD) im Nachweissystem keine Amplifikation zeigen. In der Probe sind PCR-Inhibitoren vorhanden bzw. es trat ein Fehler im Extraktionsverfahren auf. Die extrahierte Probe sollte 1:10 mit PCR Wasser verdünnt und erneut amplifiziert werden oder es sollte die Isolierung und Reinigung der Probe verbessert werden. 10. Testmerkmale 10.1 Analytische Sensitivität Die RIDA GENE EAEC real-time PCR hat eine Nachweisgrenze von 5 DNA-Kopien / Reaktion (s. Abb.2). 12 10.2 Analytische Spezifität Die RIDA GENE EAEC real-time PCR ist spezifisch für enteroaggregative E. coli. Es wurden keine Kreuzreaktivitäten zu den folgenden Spezies festgestellt (s. 13 11. Grenzen des Verfahrens 1. Das Ergebnis der molekularbiologischen Untersuchung sollte nicht allein zur Diagnose führen, sondern immer im Zusammenhang mit der Anamnese und Symptomatik des Patienten betrachtet werden. 2. Dieser Test ist nur für Stuhlproben validiert. 3. Unsachgemäße Probenentnahme, -transport, -lagerung und -handhabung oder eine Erregerlast unterhalb der analytischen Sensitivität des Tests können zu falsch negativen Ergebnissen führen. 4. Die Anwesenheit von PCR-Inhibitoren kann zu nicht auswertbaren Ergebnissen führen. 5. Mutationen oder Polymorphismen in den Primer- oder Sondenbindungsregion können den Nachweis neuer oder unbekannter Varianten beeinträchtigen und mit RIDA GENE EAEC zu falsch negativen Ergebnissen führen. 6. Wie bei allen auf PCR basierenden in-vitro-diagnostischen Tests können äußerst niedrige Konzentrationen der Zielsequenzen, die unter dem Detektionslimit (LoD) liegen, nachgewiesen werden. Die erhaltenen Ergebnisse sind nicht immer reproduzierbar. 7. Ein positives Testergebnis zeigt nicht notwendigerweise die Anwesenheit lebensfähiger Organismen an. Ein positives Ergebnis deutet darauf hin, dass enteroaggregativer E. coli (aata, aggr) vorhanden ist. 12. Literatur 1. Müller et al. 15 1. Intended use RIDA GENE EAEC real-time PCR For in vitro diagnostic use. RIDA GENE EAEC is a real-time PCR for the direct, qualitative detection of enteroaggregative E. coli (EAEC) in human stool samples. 1,2 RIDA GENE EAEC real-time PCR is intended for use as an aid in diagnosis of gastroenteritis caused by enteroaggregative E. coli. 2. Explanation of the test Escherichia coli (E. coli) are gram negative, facultatively anaerobic rod bacteria which move by peritrichal flagellation and belong to the Enterobacteriaceae family. E. coli are part of the normal intestinal flora of humans and many farm animals and are generally nonpathogenic. Some E. coli strains are pathogenic to humans through the acquisition of certain virulence factors (e.g. genes for toxins). The six known intestinal pathogenic E. coli: enterohämorrhagic E. coli (EHEC), enteropathogenic E. coli (EPEC), enterotoxigenic E. coli (ETEC), enteroinvasive E. coli (EIEC), enteroaggregative E. coli (EAEC) und diffusely adherent E. coli (DAEC) can be differentiated by the virulence factors. 3 Enteroaggregative E. coli (EAEC) were first identified and described in the stool of a child from Chile in The defining feature of EAEC is its characteristic aggregative adherence (AA) phenotype. In the gold standard HEp-2 cell adherence assay EAEC adhere to the epithelial cell surface in a stacked-brick formation. EAEC are defined as E. coli that do not secrete heat-labile or heat-stable enterotoxins and adhere to HEp-2 cells in an AA pattern. 5 Certain EAEC strains carry a high molecular weight plasmid (paa) associated with AA, on which a number of virulence genes (e.g. aggr, agga, aafa, agg3 and aata) are located. 6,7,8 Important virulence genes for EAEC detection by PCR are the aata gene (anti-aggregation protein transporter gene, referred to as CVD432 or EAEC probe) and the aggr gene (master regulator of the EAEC plasmid virulence genes). 8,9,10,11 EAEC strains that carry paa are regarded as typical EAEC while strains that lack paa are regarded as atypical EAEC. 12 The most common clinical manifestation of EAEC Infection is watery diarrhea. Less common associated clinical symptoms are low-grade fever, nausea, vomiting, abdominal pain and the presence of fecal blood, mucus or leukocytes. The incubation ranges from 8-18 h. A voluntary study with an inoculum of c.f.u. of EAEC caused diarrheal illness. Due to the high required infective dose suggests a fecal-oral transmission of EAEC by food or water. RIDA GENE EAEC
16 EAEC is the cause of acute and chronic (> 14 days) diarrhea among children, adults and HIV-infected persons, in both developing and industrialized countries. 8 It is the second most common cause of travelers diarrhea after ETEC among travelers to developing countries, such as Mexico, India and Jamaica. 13 Outbreaks of EHEC diarrhea have been reported and linked to the consumption of contaminated food. 14 EAEC has been isolated from 2% - 68% of patients with diarrhea and from 0% to 15% of controls from India, South America, Europe and the Middle East. 13 In a meta-analysis of published studies, EAEC was a cause of acute diarrhea in a median of 15% of children living in developing countries and 4% of children living in industrialized countries. 15 EAEC were isolated from 2% of pediatric patients with diarrhea in Germany compared with none of healthy controls. 12 In Swiss children EAEC was isolated in 10.2% of children with diarrhea compared with 2.2% of children without diarrhea. In the US EAEC was isolated 4.5% of case patients versus 1.7% of controls Test principle The RIDA GENE EAEC is a real-time PCR for the direct, qualitative detection of enteroaggregative E. coli (EAEC). After DNA-isolation, amplification of the gene fragments specific for EAEC (aata and aggr, if present) occurs. The amplified targets are detected with hydrolysis probes, which are labeled at one end with a quencher and at the other end with a fluorescent reporter dye (fluorophore). In the presence of a target the probes hybridize to the amplicons. During the extension step the Taq-polymerase breaks the reporter-quencher proximity. The reporter emits a fluorescent signal which is detected by the optical unit of a real-time PCR instrument. The fluorescence signal increases with the amount of formed amplicons. The RIDA GENE EAEC assay contains an Internal Control DNA (ICD) as an internal control of sample preparation procedure and to determine possible PCR-inhibition. 16 RIDA GENE EAEC

Precautions for users - For in vitro diagnostic use only. - Extraction, PCR preparation and the PCR run should be separated in different rooms to avoid cross-contaminations. - This test must only be performed by laboratory personnel trained in molecular biology methods. - Strictly follow the working instructions. - When handling samples, wear disposable gloves. After finishing the test, wash your hands. - Do not smoke, eat or drink in areas where samples or test reagents are being used. - Samples must be treated as potentially infectious as well as all reagents and materials being exposed to the samples and have to be handled according to the national safety regulations. - Do not use the kit after the expiration date. 18 RIDA GENE EAEC

Test procedure 8.1 Sample Preparation DNA-Isolation from Stool Samples Use for DNA-isolation of human stool samples a commercially available DNA isolation kit (e.g. RTP Pathogen Kit (STRATEC Molecular)) or DNA extraction system (e.g. Maxwell 16 (Promega)). Extract DNA according to the manufacturer s instructions. We recommend to dilute the stool samples before extraction 1:3 with water. Vortex the diluted stool sample intensely and centrifuge at 3,000 rpm for 30 sec. Use from the supernatant the appropriate volume according to the manufacturer s instruction. The RIDA GENE EAEC assay contains an Internal Control DNA (ICD), which can either be used as PCR inhibition control or as extraction control for the sample preparation procedure and as a PCR inhibition control. If the ICD is used only as a PCR inhibition control, 1µl of the ICD should be added to the Master-Mix (see Tab.3). If the ICD is used as an extraction control for the sample preparation procedure and as PCR inhibition control, 20 µl of the ICD has to be added during extraction procedure. The ICD should always be added to the specimen-lysis buffer mixture and must not be added directly to the specimen. 8.2 Master-Mix preparation Calculate the total number of PCR reactions (sample and control reactions) needed. One positive control and negative control must be included in each assay run. We recommend to calculate an additional volume of 10 % to compensate imprecise pipetting (see Tab.2, Tab.3). Thaw, mix gently and centrifuge briefly the Reaction Mix, the Taq-Polymerase, the Positive Control, the PCR Water and the ICD before using. Keep reagents appropriately cold during working step (2-8 C). 22 9. Result interpretation The analysis of the samples is done by the software of the used real-time PCR instrument according to the manufacturer` s instructions. Positive and negative controls have to show correct results (see Fig. 1). The positive control has a concentration of 10 3 copies / µl. An inhibition of the PCR reaction or a failure in the extraction procedure can be excluded by the detection of the Internal Control DNA (ICD). A sample is evaluated positive, if both, the sample and the Internal Control DNA, (ICD) show an amplification signal in the detection system. A sample is evaluated positive, if the sample shows an amplification signal in the detection system, but the Internal Control DNA (ICD) is negative. The detection of the internal amplification control is not necessary, because high concentrations of the amplicon can cause a weak or absent signal of the internal amplification control. A sample is evaluated invalid, if both, the sample and the Internal Control DNA (ICD) show no amplification signal in the detection system. The sample contained a PCR inhibitor or a failure occurred in the extraction procedure. The extracted sample needs to be further diluted with PCR water (1:10) and re-amplified, or the isolation and purification of the sample has to be improved. 10. Test characteristics 10.1 Analytical sensitivity The RIDA GENE EAEC real-time PCR has a detection limit of 5 DNA copies per reaction (see Fig.2). RIDA GENE EAEC

25 11. Limitations of the method 1. The result of molecular analysis should not lead to the diagnosis, but always be considered in the context of medical history and symptoms of the patient. 2. This assay is only validated for stool samples. 3. Inappropriate specimen collection, transport, storage and processing or a pathogen load in the specimen below the analytical sensitivity can result in false negative results. 4. The presence of PCR inhibitors may cause invalid results. 5. Mutations or polymorphisms in primer or probe binding regions may affect detection of new variants resulting in a false negative result with the RIDA GENE EAEC assay. 6. As with all PCR based in vitro diagnostic tests, extremely low levels of target below the limit of detection (LoD) may be detected, but results may not be reproducible. 7. A positive test result does not necessarily indicate the presence of viable organisms. However, a positive result is indicative for the presence of enteroaggregative E. coli (aata, aggr). 12.
