3 6 Summary The development of acquired resistance during the course of treatment of even ini tially sensitive tumors is a common occurrence that constitutes a major obstacle to the curative use of chemotherapeutic agents. DNA mismatch repair (MMR) proteins (MSH2, MSH3, MSH6, MLH1, PMS2) play a critical role in maintaining genomic stability. The MMR proteins are widely and concordantly expressed with an exclusively nuclear localization (see Publication I). A cell normally makes a number of mistakes during the replication of DNA which create mis matched bases. These types of errors are repaired by the MMR system. Therefore, loss of MMR results in a genome-wide increase in the rate of mutation, and such losses appear to occur frequently in sporadic and inherited human cancers as evidenced by the appearance of microsatellite instability. Microsatellites are stretches of DNA in which the same se quence of two to four bases is repeated many times. We demonstrated that, in addition to being involved in tumorigenesis, loss of MMR activity is of concern with respect to the use of chemotherapeutic agents to treat established tumors. Loss of MMR results in drug resistance directly by impairing the ability of the cell to detect DNA damage apoptosis, and indirectly by increasing the mutation rate throughout the genome. In the case of cisplatin and carboplatin the effect of loss of MMR activity and activate on sen sitivity to a panel of analogues was tested using different pairs of cell lines deficient and proficient in this function. The MMR-deficient cells were in all cell systems about two fold more resistant to cisplatin and carboplatin than the repair-proficient cells. In contrast, no difference was observed between MMR-deficient and -proficient cells with respect to sensitivity to oxaliplatin, a platinum compound which forms slightly different adducts in DNA. The ability of MMR proteins to recognize different types of platinum adducts was examined with mobility shift assays using nuclear extract from a human ovarian cancer cell line proficient in MMR. The formation of protein complexes on platinated DNA was demonstrated by gel mobility shift assay using a double-stranded radiolabeled oligo nucleotide which was platinated with either cisplatin or oxaliplatin, and the presence of hmsh2 and hmlhl proteins in the complexes was documented by supershift using anti bodies directed against hmsh2 or hmlhl. When an oxaliplatinated probe was used, dif ferent complexes were formed and no supershift was produced by anti-hmsh2 or antihmlhl antibody, indicating that the MMR complex that forms on cisplatin adducts does not form on oxaliplatin adducts. This specificity parallels the fact that loss of MMR result ed in resistance to cisplatin but not to oxaliplatin, suggesting that the components of the MMR system responsible for the difference in sensitivity are quite specific in their ability to discriminate between different types of closely related DNA adducts (see Publication II).

4 7 Unfortunately, the studies of identifying the proteins participating in the complex which binds to the DNA after cisplatin or oxaliplatin injury failed due to the fact that our laboratory had no pure MMR proteins to work with. It is not clear yet how exactly the platinum adducts are recognized by the MMR proteins. It is possible that cisplatin adducts distort the DNA in a manner that mimics the presence of either a single-base mismatch or an insertion/deletion mispair. The hypothesis is that the MMR system serves as a detector for cisplatin-damaged DNA; resistance is thought to result from failure of the cell to recognize the adducts and to activate signaling pathways that trigger apoptosis. If this hypothesis is correct, then the detector must be able to initiate activation of signaling pathways. Indeed, we demonstrated that cisplatin activates JNK1 by a PAKp65 and MKK4-independent mechanism more efficiently in MMR-proficient than -deficient cells, and that cisplatin MMR-proficient cells, whereas this response is completely activates c-abl kinase in the absent in MMR-deficient cells. This reveals that activation of JNK1 and c-abl by cisplatin is in part dependent on the integrity of the MMR function, suggesting that these kinases are part of the signal transduction pathway activated when MMR proteins recognize cisplatin adducts in DNA (see Publication III). To determine the ability of cisplatin to enrich for MMR-deficient tumor cells dur ing growth in vitro, MMR-deficient cells were molecularly engineered to constitutively express the green fluorescent protein, and changes in the fraction of these MMR-deficient cells were monitored by flow cytometric analysis. Treatment with cisplatin resulted in a gradual enrichment of MMR-deficient cells, whereas treatment with oxaliplatin did not. Likewise, isogenic MSH2~/_ embryonic stem cells grown as xenografts were significantly less responsive to treatment with a single LDio dose of cisplatin than MSH2+/+ tumors, whereas there was no difference in sensitivity to oxaliplatin. These results demonstrate that the degree of cisplatin resistance conferred by loss of MMR is sufficient to produce both enrichment of MMR-deficient cells during treatment in vitro and a large clinical responsiveness in vivo (see Publication IV). difference in We compared the effect of loss of MMR on sensitivity to a panel of commonly used chemotherapeutic agents as measured by clonogenic assay with its effect on the abil ity of the same drag to enrich for MMR-deficient cells in a proliferating tumor cell popula tion. We identified in addition to cisplatin and carboplatin the topoisomerase etoposide and doxorubicin as agents II inhibitors for which loss of MMR causes low-level resistance. Likewise, treatment with these drugs resulted in enrichment for MMR-deficient cells (see Publication V). How loss of MMR produces resistance to the topoisomerase II inhibitors is not clear yet. A high mutation rate due to loss ofmmr could also cause drag resistance by altering the key cytotoxic target of the drag, and this may be the basis for the observa tion that MMR-deficient cells demonstrate low-level resistance to the topoisomerase II in-

5 8 hibitors, since both topoisomerase genes are rich in sequences known to be at high risk for mutagenesis in MMR-deficient cells. We found the frequency of positive immunohistochemical staining for hmlhl in 66% to be reduced in ovarian tumor samples obtained after primary chemotherapy with a cisplatin or carboplatin-based regimen compared to samples of the same tumor obtained prior to treatment. These results are consistent with the concept that treatment with cispla tin or carboplatin selects for preexisting MMR-deficient cells, and that this contributes to the frequent development of drug resistance (see Publication VI).

6 9 Zusammenfassung Die Entwicklung einer Resistenz gegenuber Chemotherapeutika ist auch bei initial gut ansprechenden Tumoren ein haufiges Problem, welches den kurativen Einsatz dieser Substanzen verhindert. Die DNA Mismatch Repair (MMR)-Proteine (MSH2, MSH3, MSH6, MLH1, PMS2) spielen eine wichtige Rolle bei der Erhaltung der genetischen Stabilitat. Diese Proteine werden im Zellkern von nahezu alien Geweben exprimiert (vgl. Publikation I). Bei der Replikation der DNA konnen Fehler entstehen, welche normalerweise durch das MMR-System korrigiert werden. Daher fuhren Defekte im MMR-System zu einer Erhohung der Mutationsrate und finden sich gehauft sowohl bei sporadischen als auch bei familiaren Karzinomen. Charakteristisch ist dabei, dass Mutationen vor allem in kurzen repetitiven DNA-Sequenzen zu finden sind, was zur sogenannten Mikrosatelliten-Instabilitat fuhrt. Wir konnten zeigen, dass diese MMR-Proteine neben ihrer Rolle in der Tumorentstehung auch bei der Chemotherapie von Karzinomen von Bedeutung sind. De fekte im MMR-System konnen einerseits direkt, d. h. durch ein vermindertes Erkennen des DNA-Schadens und damit durch eine verminderte Aktivierang der Apoptose zu einer Chemoresistenz fuhren. Andererseits kann es durch eine erhohte Mutationsrate auch indirekt zu einer Resistenzentwicklung kommen. Anhand von MMR-defizienten und -intakten Zellinien haben wir die Auswirkung des Verlustes der MMR-Aktivitat auf die Sensitivitat gegenuber Cisplatin, Carboplatin und weiteren Platinverbindungen untersucht. Dabei waren die MMR-defizienten Zellen in alien untersuchten Zellsystemen ungefahr zweimal resistenter gegenuber Cisplatin und Carboplatin als die MMR-intakten Zellen. Bei Behandlung mit Oxaliplatin, einem platinhaltigen Zytostatikum, welches mit der DNA andersartige Verbindungen eingeht, konnte hingegen keine Resistenzentwicklung beobachtet werden. Mit Hilfe des Gelshift-Assays wurde anhand eines Zellkernextraktes von MMR-intakten Ovarialkarzinomzellen unter sucht, ob die MMR-Proteine unterschiedliche Typen von DNA-Verbindungen erkennen konnen. Wurde ein DNA-Fragment nach Cisplatinbehandlung mit Zellkernextrakt von MMR-intakten Zellen versehen, so wanderte die Bande in einem Polyacrylamidgel nach Zugabe eines hmsh2- und/oder hmlhl-antikorpers langsamer. Wurde hingegen dasselbe DNA-Fragment mit Oxaliplatin behandelt, so hatten die MMR-Antikorper keinen Einfluss auf das Verhalten im elektrischen Feld. Diese Beobachtung weist darauf hin, dass die MMR-Proteine Cisplatin-DNA-Verbindungen erkennen konnen, hingegen nicht Oxaliplatin-DNA-Verbindungen. Dies steht mit der Tatsache im Einklang, dass Defekte im MMR-System zu einer Resistenz gegenuber Cisplatin aber nicht gegenuber Oxaliplatin fuhren und zeigt, dass die MMR-Proteine unterschiedliche Typen gen unterscheiden konnen (vgl. 7 10 Der Versuch jene Proteine zu identifizieren, welche nach Cisplatin- oder Oxaliplatinbehandlung an der DNA binden, scheiterte, da unser Labor nicht im Besitz der reinen MMR-Proteine war. Es ist noch nicht genau geklart, wie die MMR-Proteine den Schaden an der DNA erkennen. Als Hypothese dragt sich aber auf, dass die MMR-Proteine als eine Art Detektor wirken. Defekte im MMR-System fuhren dazu, dass der Cisplatinschaden an der DNA nicht wahrgenommen wird, was sich phenotypisch als Resistenz zeigt. Sind die MMR-Proteine funktionsfahig, wird nach Erkennen des Schadens an der DNA ein Signal ausgelost, welches zur Apoptose fiihrt. Wir konnten zeigen, dass eine Cisplatinbehandlung in MMR-intakten Zellen zu einer deutlicheren Aktivierung von JNK1 und c-abl fiihrt als in MMR-defizienten Zellen. Diese Beobachtung stiitzt die Hypothese, dass nach der Detektion des Cisplatinschadens an der DNA ein Pathway mit Beteiligung und c-abl ausgelost wird (vgl. Publikation III). von JNK1 Um das Selektionspotential von Cisplatin zu priifen, wurden MMR-defiziente Zel len molekularbiologisch derart verandert, dass sie ein griinfluoreszierendes Protein exprimierten. Der Anteil der MMR-defizienten Zellen in einer Tumorzellpopulation wurde dann mittels Flowzytometrie gemessen. Behandlungen mit Cisplatin fiihrten zu einer Anreicherung der MMR-defizienten Zellen, wohingegen Behandlungen mit Oxaliplatin das Verhaltnis nicht anderten. Ebenso sprachen im Tierversuch MSH2-defiziente Tumore deutlich weniger gut auf eine LDio-Dosis von Cisplatin an als MSH2-intakte Tumore. Bei der Oxaliplatinbehandlung fand sich demgegemiber beziiglich Tumoransprechrate kein Unterschied. Diese Resultate zeigen, dass das Ausmass der Cisplatinresistenz, welche durch den Verlust der MMR-Aktivitat verarsacht wird, ausreicht, um in vitro zu einer Selektion von MMR-defizienten Zellen und in vivo zu einem deutlichen Unterschied beziiglich klinischem Ansprechen zu fuhren (vgl. Publikation IV). Zusatzlich zu den platinhaltigen Substanzen haben wir weitere haufig verwendete Chemotherapeutika beziiglich ihrer Sensitivitat in MMR-defizienten Zellen untersucht. Dabei konnten die Topoisomerase II-Inhibitoren Etoposid und Adriamycin als Substanzen identifiziert werden, die bei MMR-defizienten Zellen eine Resistenz zeigen. Ebenso fiihrt eine Behandlung mit Etoposid oder Adriamycin zu einer Anreicherung von MMR-defi zienten Zellen (vgl. Publikation V). MMR-defiziente Zellen weisen eine erhohte Muta tionsrate in zahlreichen Genen auf, welche ebenfalls zu einer Resistenz gegenuber Che motherapeutika fuhren kann. Topoisomerase-Gene enthalten viele Sequenzen, die gegen uber der Mutagenese anfallig sind. Moglicherweise erklart dies die beschriebene Resi stenzentwicklung. Wir konnten zeigen, dass der Prozentsatz immunhistochemisch hmlhl-positiver Ovarialkarzinomzellen nach cisplatin- oder carboplatinhaltiger Therapie in 66% der Patientinnen tiefer ist als vor der Chemotherapie. Diese Resultate stiitzen ebenfalls die Hy-

8 11 pothese, dass Cisplatin und Carboplatin zu einer Anreicherang von vorbestehenden MMR-defizienten Zellen fiihrt, was zur haufigen Resistenzentwicklung in der Klinik beitragt.
