2 Deutsch Anleitung zur Probenentnahme und Versand Anleitung zur Blutentnahme 1. Beschriftung des Entnahmeröhrchens n Name des Patienten n Identifikationsnummer des Patienten n Geburtsdatum ml venöses Blut mittels Venenpunktion in das Entnahmeröhrchen (Streck Cell-Free DNA BCT ) aufnehmen. Weitere Informationen zu den Schritten 2 4 sind in der Gebrauchsanleitung für das Streck Cell- Free DNA BCT ( zu finden. 3. Das Röhrchen vollständig befüllen (bis der Blutstrom zum Stillstand gekommen ist). 8 10x 4. Den Inhalt des Blutentnahmeröhrchens sofort durch leichtes 8- bis 10-maliges Schwenken der Hand mischen. Hinweis: Eine gute Mischung ist nur dann gewährleistet, wenn die Drehung der Hand 180 in jede Richtung beträgt. 5. Befülltes Blutentnahmeröhrchen unbedingt etikettiert und bei Raumtemperatur aufbewahren. Hinweis: Auf keinen Fall kühlen oder einfrieren! 6. Datum und Uhrzeit der Blutentnahme notieren.

3 Deutsch Anleitung für Verpackung und Versand der Probe 1. Das Probenröhrchen in eine der Aussparungen des Absorptionsbeutels einlegen und einwickeln. 2. Das nun im Absorptionsbeutel befindliche Probenröhrchen in den Gefahrgutbeutel (für Biogefahrgut) legen und den Beutel verschließen. 3. Den Gefahrgutbeutel in den Luftpolsterbeutel legen, den Schutzstreifen von der Klebefläche abziehen und den Luftpolsterbeutel verschließen. 4. Den Luftpolsterbeutel auf ein Ende der Gelverpackungs hülle (muss Raumtemperatur haben) legen und einwickeln. 5. Einen Schaumstoffblock (der unbedingt Raumtemperatur haben muss) unten in die Styproporbox legen. Schaumstoffblock Gel-Verpackungshülle Schaumstoffblock Styroporbox 6. Die Gelverpackungshülle mit der darin enthaltenen Probe aus Schritt 4 auf den Schaumstoffblock in der Styroporbox auflegen. 7. Den zweiten Schaumstoffblock nun oben auf die Gelverpackungshülle legen. Hinweis: Alle Verpackungskomponenten (einschließlich Gelverpackungshülle und Schaumstoffblock) immer nur bei Raumtemperatur lagern. Auf keinen Fall kühlen oder einfrieren!

4 Deutsch Styroporbox Versandkarton 8. Die Styroporbox schließen und in den Versandkarton setzen. 9. Den Versandkarton mit einem breiten Klebeband verschließen. Freigestellte Medizinische Proben 10. 5 English Specimen Collection and Shipping Instructions Blood Draw Instructions 1. Fill out label with: n Patient name n Patient identification number n Date of birth 2. Collect 10 ml of blood by venipuncture into the provided Streck Cell-Free DNA BCT (blood collection tube). Consult Instructions For Use for the Streck Cell-Free DNA BCT ( for steps Fill the tube completely (until blood flow stops). 8 10x 4. Immediately mix the tube by gentle inversion 8 to 10 times. Note: One inversion is a complete turn of the wrist (180 degrees) and back. 5. Ensure that tube is labeled and kept at ambient temperature. Note: Do not refrigerate or freeze blood samples. 6. Record date and time of blood draw.

6 English Packaging and Shipping Instructions 1. Place specimen tube inside one of the slots of the absorbent pouch and roll the absorbent material around the tube. 2. Place specimen tube within absorbent pouch into the biohazard specimen bag and seal the bag. 3. Place biohazard specimen bag into the foil bubble pouch and seal the foil bubble pouch after removing the adhesive liner. 4. Place the foil bubble pouch on one end of the ambient gel wrap and wrap it around the foil bubble pouch. 5. Place one ambient foam brick into the bottom of the styrofoam box. foam brick gel wrap foam brick Styrofoam box 6. Place the ambient gel wrap protecting the specimen from step 4 into the styrofoam box on top of the foam brick. 7. Place the second ambient foam brick on top of the ambient gel wrap. Note: Keep gel wrap and foam bricks at ambient temperature. Do not refrigerate or freeze.

7 English Styrofoam box Cardboard box 8. Close the styrofoam box and place it inside the cardboard shipping box. 9. Seal the cardboard shipping box using wide sealing tape. Exempt Human Specimen 10.
