1 Serazym Helicobacter pylori Enzymimmunoassay zum Nachweis von Helicobacter pylori Antigen in Kotproben r E-081 n96 r E n48 v In-vitro- Diagnostikum c Einführung Das humanpathogene Bakterium Helicobacter pylori (H. pylori) ist ein gramnegatives, bewegliches, spiralförmiges Stäbchen. Es wird geschätzt, dass etwa die Hälfte der Weltbevölkerung mit H. pylori infiziert ist, wobei die Infektionsrate in Ländern mit vergleichsweise niedrigem hygienischem Standard bereits im Kindesalter (< 10 Jahre) bei über 70% und in Ländern mit höherem hygienischem Standard bei 25-50% liegt. Für die Verbreitung gelten iatrogene, oral-orale und fäkal-orale Übertragungswege als gesichert. Das Vorkommen tierischer Erregerreservoire konnte bislang nicht eindeutig belegt werden (1). H. pylori ist durch eine starke Ureaseaktivität und bei einigen Stämmen durch die Produktion eines Zytotoxins (Vac A) charakterisiert. Die pathogenetische Bedeutung dieser extrazellulären Produkte liegt in der direkten Epithelschädigung. Darüber hinaus verursacht die Infektion eine chronische Entzündungsreaktion unter verstärkter Bildung von Entzündungsmediatoren wie Interleukin 8 und 1, sowie Tumornekrosefaktor alpha. Den bei H.pylori Infizierten häufig nachweisbaren Autoantikörpern gegen Parietalzellen wird eine mögliche Rolle bei der Entwicklung der atrophischen Gastritis als Vorstufe des Magenkarzinoms zugeschrieben (1, 2). Die primär meist asymptomatisch verlaufende Infektion führt bei etwa 10% der Infizierten zur Ausprägung von H. pylori assoziierten Gastritiden, die Folgeerkrankungen wie chronisch-aktive Gastritis, Ulcus ventriculi, Ulcus duodeni, Magenkarzinom sowie MALT-Lymphom nach sich ziehen können. Patienten mit Magenmalignomen und Ulcus duodeni sind zu fast 100% mit H. pylori infiziert, Patienten mit chronischer Gastritis zu 80% und mit Ulcus ventriculi zu 70%. Beim Magenkarzinom liegt in 60% der Fälle eine Infektion mit H. pylori vor (2). In der Regel beruht die Diagnostik auf der Kombination von Gastroendoskopie und nachfolgender Untersuchung endoskopisch entnommenen Biopsiematerials (Kultur, Histologie und Urease-Schnelltest). 2 Die Erregeranzucht aus Biopsiematerial ist schwierig und gelingt nicht immer. Häufig wird zur Verlaufskontrolle der 14 C-Atemtest durchgeführt, der durch die bakterielle Urease aus radioaktiv markiertem Harnstoff freigesetztes CO 2 in der Atemluft des Patienten nachweist. Dieser Test hat zwar den Vorteil, neben hoher Spezifität und Sensitivität nicht-invasiv zu sein, ist aber an spezielle Messgeräte und die Einnahme radioaktiv markierter Substanzen durch den Patienten gebunden. Inzwischen stehen immunologische Tests auf der Basis spezifischer anti- Helicobacter pylori Antikörper zur Verfügung, die den direkten Nachweis von H. pylori Antigen aus Kotproben ermöglichen und zur therapeutischen Verlaufskontrolle eingesetzt werden können. Der serologische Nachweis von spezifischen Antikörpern hat den Nachteil, dass die im Serum Infizierter lebenslang persistierenden Immunglobuline nach Erreger-Eradikation nicht vollständig verschwinden, so dass eine Therapiekontrolle nur bedingt und unter der Voraussetzung der Untersuchung von Serumpaaren möglich ist (1, 3). J. Clin. Microbiol. 2005; 43: Anwendungsbereich Der Serazym Helicobacter pylori ist ein in-vitro vitro-diagnostikum zum Nachweis von Helicobacter pylori Antigen in Kotproben. Testprinzip Der Serazym Helicobacter pylori ist ein indirekter Zwei-Seiten-Assay auf der Basis polyklonaler Antikörper gegen Helicobacter pylori. Verdünnte unbehandelte Kotproben sowie negative und positive Kontrollproben werden zusammen mit biotinylierten, polyklonalen anti-helicobacter pylori Antikörpern in die mit polyklonalen anti-helicobacter pylori Antikörpern beschichteten Vertiefungen der Mikrotitrationsstreifen dosiert. Nach 60 min Inkubation bei Raumtemperatur (RT) werden die ungebundenen Komponenten durch einen Waschschritt entfernt und Streptavidin-poly-Peroxidase (POD)-Konjugat in die Kavitäten dispensiert. Nach erneuter Inkubation für 30 min bei RT wird das ungebundene Streptavidin-poly-Peroxidase (POD)-Konjugat in einem Waschschritt entfernt. Die POD setzt in einem nachfolgenden enzymatischen Reaktionsschritt die farblose Substratlösung in ein blaues Endprodukt um. Diese Reaktion wird nach 15 Minuten durch Zugabe der Stopplösung abgebrochen, wodurch ein Farbumschlag der blauen Lösung zu gelb auftritt. Die bei 450 / 620 nm gemessene Extinktion des Endprodukts ist zur Konzentration der spezifisch gebundenen Helicobacter pylori Antigene direkt proportional. GAL_E-081_ _DE Seite 2 von 16

Wiederholtes Einfrieren und Auftauen der Proben ist zu vermeiden. Vorbereitung und Verwendung Eingefrorene Kotproben zügig auftauen und gut durchmischen. In ein Reaktionsgefäß 500 µl Verdünnungsmedium pipettieren. Bei festen oder halbfesten Kotproben 100 mg (Durchmesser etwa 2-3 mm) mit einem Einmalstäbchen, bei flüssigen Kotproben 100 µl in das Probenverdünnungsmedium überführen und sorgfältig suspendieren. Gegebenenfalls Schwebeteilchen durch Zentrifugation in einer Mikrozentrifuge 1 min bei maximaler Drehzahl sedimentieren. Für die Testdurchführung zusätzlich benötigte Materialien und Hilfsmittel Verstellbare Einkanal-Mikropipette 8-Kanalpipette bzw. 4 Vorbereitung und Lagerung der Reagenzien Testbesteckformat und Haltbarkeit Ein Testbesteck enthält Reagenzien für 96 oder 48 Bestimmungen. Das komplette Testbesteck mit verschlossenen Reagenzflaschen und Mikrotitrationsstreifen ist bei Lagerung bei 2 8 C bis zum aufgedruckten Verfallsdatum stabil. Alle geöffneten Testbesteckbestandteile sind bei ordnungsgemäßer Lagerung bei 2 8 C mindestens 2 Monate haltbar. Die gebrauchsfertig verdünnte Waschlösung ist bei 2 8 C mindestens 1 Monat verwendbar. Vorbereitung und Verwendung Die Mikrotiterplatte mit teilbaren Streifen ist in einem aluminium-beschichteten Beutel zusammen mit Trockenmittel vakuumversiegelt. Öffnen der Verpackung erst nach Erreichen der Raumtemperatur. Nicht gebrauchte Kavitäten vor Feuchtigkeit schützen und zusammen mit dem Trockenmittel in den Beutel zurücklegen und verschließen. Waschpufferkonzentrat (10-fach) mit destilliertem oder deionisiertem Wasser verdünnen. Beispiel: 10 ml Waschpufferkonzentrat (2) + 90 ml destilliertes oder deionisiertes Wasser. Testdurchführung Proben mit Verdünnungsmedium (3) 1 : 6 verdünnen, z.b. 100 mg oder 100 µl Kotprobe + 0,5 ml Verdünnungsmedium (3). Die Reihenfolge der Pipettierschritte und deren Durchführung im Zeittakt sind einzuhalten. Bei größeren Probenserien empfiehlt sich das Pipettieren der Reagenzien aus Flüssigkeitsreservoirs mittels Mehrkanalpipette, um Zeitverzögerungen zu vermeiden. Beim Waschvorgang dispensierte Waschlösung mindestens 5 Sekunden einwirken lassen und Waschlösungsreste durch gründliches Absaugen oder Ausschlagen der Kavitäten entfernen! Substrat vor Licht geschützt aufbewahren! Arbeitsschritte 1. Die Testreagenzien und die benötigte Anzahl an Kavitäten auf Raumtemperatur (RT) erwärmen und alle Reagenzien vor Gebrauch leicht schütteln, Schaumbildung vermeiden Tropfen (oder 120 µl) CONJ BIOTIN Biotin-Konjugat (6/1) pro Kavität. 3. Je 120 µl CONTROL + Positive Kontrolle (4) 120 µl CONTROL Negative Kontrolle (5) 100 µl verdünnte Probe pipettieren, mischen. 4. Platte abkleben und 60 min bei RT inkubieren. 5. Dekantieren und 5 mal mit 300 µl Waschlösung (verdünnt aus (2 ) waschen. Restflüssigkeit durch Ausschlagen auf Zellstoff entfernen Tropfen (oder 120 µl) CONJ STREPT Streptavidin-poly-POD-Konjugat (6/2) pro Kavität. 7. Platte abkleben und 30 min bei RT inkubieren. 8. Dekantieren und 5 mal mit 300 µl Waschlösung (verdünnt aus (2 ) waschen. Restflüssigkeit durch Ausschlagen auf Zellstoff entfernen 9. 3 Tropfen (oder 120 µl) SUBSTR TMB Substrat (7) pro Kavität min lichtgeschützt bei RT inkubieren Tropfen (oder 120 µl) STOP Stopplösung (8) pro Kavität, kurz schütteln 13. 5 Berechnung der Ergebnisse Qualitative Evaluierung Cut-off Bestimmung: OD Negative Kontrolle + 0,10 Proben mit OD Werten, die oberhalb der errechneten Grenzwert OD liegen sind als positiv, Proben mit OD Werten unterhalb der oder gleich der errechneten Grenzwert OD sind als negativ im Serazym Helicobacter pylori zu bewerten. Referenzwert Serazym Helicobacter pylori Positiv > Cut-off Negativ Cut-off Aufgrund von Unterschieden in der Population wird empfohlen, dass jedes Labor seine eigenen pathologischen und normalen Referenzbereiche bestimmen sollte. Die genannten Werte sind deshalb nur als Empfehlung zu werten. Testvalidierung Der Test kann ausgewertet werden, wenn: OD-Mittelwert der negativen Kontrolle 0,20 (manuelle Abarbeitung) 0,30 (automatische Abarbeitung) OD-Mittelwert der positiven Kontrolle 1,20 Sind die o. g. Gültigkeitskriterien nicht erfüllt, muss der Testansatz wiederholt werden. Stellen Sie sicher, dass die Abarbeitung strikt gemäß Testanleitung erfolgt (korrekte Reagenzienvorbereitung, korrekte Inkubationszeiten und -temperaturen, sorgfältiges Waschen, Mischen nach Zugabe von Kontrollen und Proben zur Biotin-Konjugatlösung im ersten Inkubationsschritt). Sollten die Gültigkeitskriterien auch nach wiederholtem Testansatz nicht erfüllt sein, kontaktieren Sie bitte den Hersteller. Grenzen der Methode Der qualitative enzymimmunologische Nachweis von Helicobacter pylori in Kotproben lässt keine Korrelation zwischen gemessener Extinktion und Schweregrad der Infektion zu. Die Extinktion der Proben darf auch nicht mit der Extinktion der positiven Kontrolle in Korrelation gesetzt werden. Kreuzkontaminationen der Testbesteckreagenzien und Proben können zu falschen Ergebnissen führen. Unkorrekte Verdünnung, ungenügende Homogenisierung der Proben sowie nicht sedimentierte Festbestandteile in zentrifugierten Proben können sowohl zu falsch positiven als auch zu falsch negativen Ergebnissen führen. Automatische Abarbeitung Bei Abarbeitung des Serazym Helicobacter pylori auf einem Mikrotitrationsplatten-Vollautomaten (wie z.b. DS2 oder DSX) können in Abhängigkeit vom verwendeten Gerät und von den individuellen Geräteeinstellungen im Vergleich zur manuellen Bearbeitung höhere OD-Werte gemessen werden. In diesen Fällen ist die Erhöhung des max. zulässigen Grenzwertes für die OD der Negativ-Kontrolle auf 0,3 zulässig. Für den Waschprozess ist die Einprogrammierung von Waschpuffer-Einwirkzeiten (mind. 10 sec pro Streifen und Waschschritt) gefolgt von einem Waschschritt mit destilliertem oder demineralisiertem Wasser und 10 sec Einwirkzeit zu empfehlen. Gegebenenfalls kann die Anzahl der Waschschritte von 5x auf 7x bis 8x erhöht werden. GAL_E-081_ _DE Seite 5 von 16

8 Allgemeine Hinweise und Vorsichtsmaßnahmen Dieses Testbesteck ist nur zum in-vitro Gebrauch bestimmt und darf nur von geschultem Laborfachpersonal durchgeführt werden. Die Arbeitsanleitung ist strikt einzuhalten. Das Testbesteck oder seine geöffneten Reagenzien sind nur innerhalb der angegebenen Haltbarkeitsfristen zu verwenden. Das Mischen von Testbesteckkomponenten verschiedener Chargen ist nicht erlaubt mit Ausnahme von Verdünnungsmedium, Waschpuffer, TMB/Substratlösung und Stopplösung. Die Komplettierung eines geöffneten Testbestecks mit Reagenzien anderer Hersteller ist nicht erlaubt. Einige Reagenzien (2, 3, 4, 5, 6, 7) enthalten geringe Mengen Kathon (1,0% v/v) und Thimerosal (0,01% w/v) als Konservierungsmittel. Reagenzien nicht verschlucken und Kontakt mit Schleimhäuten vermeiden. Die Lagertemperatur der Reagenzien bis zur Wiederverwendung beträgt 2 8 C. Beim Umgang mit den Komponenten des Testbestecks sowie mit den Patientenproben und Kontrollen sind die Vorschriften zur Unfallverhütung beim Umgang mit potentiell infektiösem Material und gefährlichen Chemikalien zu beachten. Insbesondere sind folgende Regeln einzuhalten: Nicht essen, trinken oder rauchen! Nie mit dem Mund pipettieren! Handschuhe zur Vermeidung von Kontakt mit den Reagenzien und Proben tragen! Sicherheitshinweise zu den einzelnen Testkomponenten beachten! 9 Serazym Helicobacter pylori Enzyme immunoassay for detection of Helicobacter pylori antigen in faecal samples r E-081 n96 r E n48 v In-vitro- diagnostic device c Introduction The human pathogen Helicobacter pylori (H. pylori) is a gram-negative spiral shaped bacterium that has been found in the stomachs of humans in all parts of the world. In developing countries, 70 to 90% of the population carries H. pylori mostly already acquired before the age of 10 years. In developed countries the prevalence of infection ranges from 25 to 50%. H. pylori infections are transmitted via the oral-oral, the faecal-oral route or iatrogenic. A substantial reservoir for H. pylori aside from the human stomach has not been confirmed so far (1). H. pylori is characterized by a strong urease activity and some strains additionally produce a cytotoxin (Vac A). These extracellular products contribute to the pathogenesis by direct damage of the gastric epithelium accompanied by a chronic inflammation with enhanced levels of inflammation mediators like interleukin 8, interleukin 1 or tumor necrosis factor alpha. Patients infected withh. pylori often develop autoantibodies to parietal cells. The key-role of these autoantibodies in the development of atrophic gastritis as precursor of gastric cancer has been reviewed (1, 2). About 10% of the infected persons develop H. pylori associated gastritis and secondary diseases like chronic active gastritis, Ulcus ventriculi, Ulcus duodeni, gastric cancer and MALT lymphoma. Patients suffering from gastric malignomas and Ulcus duodeni are infected with H. pylori in nearly 100% of the cases. About 80% of patients with chronic gastritis, 70% of patients with Ulcus ventriculi and 60% of patients with gastric cancer are infected with H. pylori (2). Diagnosis is usually based on gastroendoscopy combined with the detection of the pathogen in biopsy material by culture, histology and rapid urease test. Culture from biopsy material is difficult and not always successful. The 14 C breath test which is often performed as follow up test, detects CO 2 released by the bacterial urease from radioactive labeled urea in patients breath. They enable the direct detection of H. pylori antigens from faecal specimens and may be used for therapeutic surveillance. Specific antibodies to H. pylori may show a lifelong persistence and even after eradication therapy they are usually still detectable. Therefore therapeutic monitoring by detection of specific antibodies is of limited meaning and does only make sense when paired serum samples are investigated (1, 3). Principle Of The Test Serazym Helicobacter pylori is an indirect two-site-immunoassay for the qualitative determination of Helicobacter pylori antigen based on polyclonal antibodies. Biotinylated polyclonal anti-h. pylori antibodies are dispensed simultaneously with specimens and the positive and negative control into the wells of the microplate coated with anti-h. pylori antibodies (polyclonal, rabbit). After 60 minutes at room temperature (RT) unbound material is removed by a washing step. Subsequently solid phase captured antigen-antibody-biotin complexes specifically react with streptavidin-horseradish peroxidase (HRP) during a second incubation period of 30 min at RT. Unbound material is separated by a washing step. HRP converts the subsequently added colourless substrate solution of 3,3,5,5 -tetramethylbenzidine (TMB) into a blue product. The enzyme reaction is terminated by sulphuric acid dispensed into the wells after 15 min incubation at RT turning the solution from blue to yellow. The absorbances of the solution read at 450 / 620 nm is directly proportional to the specifically bound amount of Helicobacter pylori antigen. Longer storage is possible at -20 C. Repeated freezing and thawing of samples should be avoided. Preparation Quickly thaw frozen samples, warm to room temperature and mix well. Pipette 500 µl of sample diluent into a clean tube. Using a disposable stirring rod tansfer about 100 mg (diameter about 2-3 mm) of faeces if solid or pipette 100 µl if liquid into the tube and suspend thoroughly. If necessary spin down floating particles in a micro centrifuge at maximum speed for 1 min. 12 Preparation And Storage Of Reagents Kit size and expiry One kit is designed for 96 or 48 determinations. The expiry date of each component is reported on its respective label and that of the complete kit on the outer box label. Upon receipt, all test components have to be kept at 2 8 C, preferably in the original kit box. After opening all kit components are stable for at least 2 months, provided proper storage. The ready to use wash solution can be used for at least one month when stored at 2 8 C. Reagent preparation Allow all components to reach room temperature prior to use in the assay. The microtitration plate is vacuum-sealed in a foil with desiccant. The plate consists of a frame and strips with breakable wells. Allow the sealed plate to reach room temperature before opening. Unused wells should be stored refrigerated and protected from moisture in the original cover carefully resealed. Prepare a sufficient amount of wash solution by diluting the 10-fold concentrated wash buffer with distilled or deionized water. For Example: 10 ml wash buffer concentrate (2) + 90 ml distilled or deionized water. Assay Procedure Dilute samples with sample diluent (3) 1 : 6, e.g. 100 mg or 100 µl faeces ml sample diluent (3). Avoid any time shift during dispensing of reagents and samples. Make sure the soak time of the wash solution in the wells is at least 5 seconds per wash cycle and that the remaining fluid is completely drained in every single wash cycle! Avoid light exposure of the TMB substrate solution! Working steps 1. Warm all reagents to room temperature (RT) before use. Mix gently without causing foam. 2. Dispense 3 drops (or 120 µl) CONJ BIOTIN biotin-conjugate (6/1) per well. 3. Pipette: 120 µl CONTROL + positive control (4) 120 µl CONTROL negative control (5) 100 µl diluted faecal specimen and mix gently. 4. Cover plate and incubate for 60 min at RT. 5. Decant, then wash each well 5x with 300 µl wash solution (diluted from (2 ) and tap dry onto absorbent paper. 6. Dispense 3 drops (or 120 µl) CONJ STREPT streptavidin-poly-hrp-conjugate (6/2) per well. 7. Cover plate and incubate for 30 min at RT. 8. Decant, then wash each well 5x with 300 µl wash solution (diluted from (2 ) and tap dry onto absorbent paper. 9. Dispense 3 drops (or 120 µl) SUBSTR TMB substrate (7) per well. 10. Incubate for 15 min at RT protected from light. 11. Dispense 3 drops (or 120 µl) STOP stop solution (8) per well, mix gently. 14. 13 Result Interpretation Qualitative evaluation Cut-off determination: OD negative control Samples with absorbances higher than the cut-off value are considered positive, samples with absorbances lower than or equal to the cut-off value are considered negative for Helicobacter pylori antigen. Reference Values Serazym Helicobacter pylori Positive > Cut-off Negative Cut-off It is recommended that each laboratory establishes its own normal and pathological reference ranges as usually done for other diagnostic parameters too. Therefore, the mentioned reference values provide a guide only to values which might be expected. Test validity The test run is valid if: the mean OD of the negative control is 0.20 (manual performance) 0.30 (automatic performance) the mean OD of the positive control is 1.20 If the above mentioned quality criteria are not met, repeat the test and make sure the test procedure is followed correctly (incubation times and temperatures, sample and wash buffer dilution, wash steps, mixing after addition of controls and samples to the biotin-conjugate in the first incubation step etc.). In case of repeated failure of the quality criteria contact your supplier. Limitations of the procedure There is no correlation between measured absorbance and seriousness of the infection. It is also not allowed to correlate absorbances of the samples with that of the positive control. Cross contamination of reagents and samples can produce false positive results. Incorrect dilutions, not sufficiently homogenized samples or solid particles after centrifugation of the suspension can cause false negative as well as false positive results. A negative test result in the Serazym Helicobacter pylori does not exclude an infection. Automatic Processing Performing the Serazym Helicobacter pylori on fully automated microplate processors (e.g. DS2 or DSX) may cause elevated absorbances in comparison to the manual procedure due to individual differences concerning wash procedures and general technical specifications of the equipment. In these cases a maximum value of 0.3 absorbance units is permissible for the negative control. It is recommended to use a wash procedure including 10 seconds soak time per strip and wash step followed by a wash step with distilled or deionized water with 10 seconds of soak time after the final wash step of each wash cycle. 16 Common Advices and Precautions This kit is for in-vitro use only. Follow the working instructions carefully. The kit should be performed by trained technical staff only. The expiration dates stated on the respective labels are to be observed. Do not use or mix reagents from different lots except for sample diluent, wash buffer, TMB/substrate solution and stop solution. Do not use reagents from other manufacturers. Avoid time shift during dispensing of reagents. All reagents should be kept at 2 8 C before use. Some of the reagents contain small amounts of Thimerosal (0.01% w/v) and Kathon (1.0% v/v) as preservative. They must not be swallowed or allowed to come into contact with skin or mucous membranes. Handle all components and all patient samples as if potentially hazardous. Since the kit contains potentially hazardous materials, the following precautions should generally be observed: Do not smoke, eat or drink while handling kit material! Always use protective gloves!
