Vorwort Liebe Kolleginnen und Kollegen, die von Prof. Gerhard Bueß entwickelte und eingeführte TEM (Transanale Endoskopische Mikrochirurgie) war das erste endoskopische Verfahren in der Viszeralchirurgie. Bereits seit 1983 ist die TEM in der klinischen Anwendung. Klinische Ergebnisse sind wesentlich besser, als nach anderen lokal-chirurgischen oder flexibel endoskopischen Techniken. Die Indikation hat sich in der Anfangsphase auf große breitbasige Adenome konzentriert, heute ist aber auch die Entfernung der T1 low-risk Karzinome eine Regel indikation mit exzellenten Ergebnissen. Wesentliche Aspekte der derzeitigen klinischen Forschung sind die Vermeidung methodenbedingter Komplikationen und die Frage, ob auch die Behandlung von T2 low-risk Karzinome nach neoadjuvanter Radiochemotherapie vertretbar oder sogar besser ist. In den vergangenen Jahren wurde in Zusammenarbeit mit dem Städtischen Klinikum Karlsruhe und der Firma Erbe Elektromedizin die "Waterjet unterstützte ESD-TEM" in chromoendoskopischer Technik entwickelt. Dabei handelt es sich um ein komplikationsarmes Verfahren zur Entfernung von großen flachen Rektumadenomen mit sehr niedriger Rezidivrate. Auf Grund des anspruchsvollen Verfahrens ist ein intensives Training für die TEM Voraussetzung, um mit dem Instrumentarium umgehen zu können und die Operationstechnik sicher zu beherrschen. Bereits 1985 wurde der erste Kurs unter wissenschaftlicher Leitung von Prof Bueß in Köln durch geführt und folgende Prinzipien entwickelt: Ausbildung in kleinen Gruppen Sukzessive Steigerung des Schwierigkeitsgrades Fokus auf das Training der manuellen Geschicklichkeit und der methodischen Beherrschung des Verfahrens Verwendung von speziellen Trainings systemen mit integrierten Tierorganen In Zusammenarbeit mit Lilo Mailänder wurde das Schulungskonzept mit anatomischen Trainingsmodellen, wie dem Tübinger MIC Trainer und Verwendung optimal geeigneter Kolonteilen vom Rind schrittweise weiterentwickelt und verbessert. Auch die Perfusion des Kolonpräparats mit heparinisiertem Blut ermöglicht das Erlernen der Hämostasetechniken unter äußerst realistischen Bedingungen. Preface Dear colleagues, the TEM-technique (Transanal Endoscopic Microsurgery) developed and introduced by Prof. Gerhard Bueß was the first endoscopic procedure in visceral surgery. Already since 1983 TEM is in clinical application. Today TEM is world wide accepted as the most precise technique for endorectal operations and clinical results are far superior to any other local treatment or endoscopic technique. Indications were in the starting phase focussed on large sessile adenomas, today early cancer (T1 low risk) also shows better results after TEM. An important aspects of the current clinical research are how to avoid method-linked complications and the question if a treatment of T2 low-risk carcinomas after neoadjuvant radio chemotherapy is acceptable or even better. In the recent years the "Waterjet assisted ESD-TEM" in chromoendoscopic technique has been developed in cooperation with the Staedtisches Klinikum in Karlsruhe and ERBE Elektromedizin, Tuebingen. It is a procedure with a low complication rate and with a very low recurrence rate for the removal of big and flat rectal adenomas. Due to the complexity of TEM needs intensive training, to master the handling of the instruments and to perform a safe procedure when starting clinical application. Already in 1985 the first TEM-course was held under the scientific direction of Prof. Bueß in Cologne following these specific principles: Teaching in small groups Increasing complexity of the operative steps Focus on training of manual dexterity and mastership of the procedure Use of anatomical training boxes with integrated animal organs This procedure was gradually progressed and developed in cooperation with Lilo Mailänder on the basis of anatomical training models with integration of bovine sections of colon ideally suited to this purpose. Perfusion of the colon dissection with heparinized blood allows the hemostatic techniques to be learnt so that training procedures can be implemented under highly realistic conditions in the Tübingen MIS Trainer. TEM Intensivkurs zur Transanalen Endoskopischen Mikrochirurgie Bei der TEM wird mit dem Operationsrektoskop nach Bueß und speziell entwickelten Instrumenten durch den Anus hindurch operiert. Es können so Rektumadenome aber auch kleine Rektumkarzinome operativ entfernt werden. Der Kurs vermittelt in einer Mischung aus intensiven praktischen Übungen mit ansteigenden Schwierigkeitsgraden, theoretischen Unterrichtseinheiten und OP- Hospitationen alle wichtigen Kenntnisse. Durch diese komplexe Fortbildung wird der Teilnehmer optimal vorbereitet, um die Operationstechnik sicher ausführen zu können. TERMINE 2016 11. - 14. April 2016 27. - 30. Juni 2016 Neben der eigentlichen Operationstechnik wird auch auf wichtige spezifische Aspekte für die Diagnose, wie die starre Rektoskopie und Ultraschalluntersuchung sowie die Biopsie eingegangen. Auch die Fragen der richtigen Indikation, besonders im Falle früher Karzinome in welchem Zusammenhang zunehmend auch die Radiochemotherapie zur Anwendung kommt, sind Teilaspekt des Kurses. Weiterhin werden die Vermeidung von Komplikationen durch systematisch, methodisches Vorgehen sowie die Behandlung möglicher Komplikationen geschult. Eine klinische Hospitation, durchgeführt am letzten Kurstag, sowie eine Diskussionsrunde, u.a. mit Fallbeispielen, komplettieren die Veranstaltung. 21. - 24. TEM Intensive course for transanal endoscopic microsurgery During TEM the Bueß rectoscope and a set of specialized instruments are introduced through the anus for operating. In this way rectal adenomas as well as small rectal carcinomas, can be removed operatively. The course, with its unique combination of practical exercises of increasing grades of difficulty, operating theatre visits and theoretical instruction, is de signed to provide the participant with the relevant know-how that will prepare him to carry out "safe operating techniques". The important specific aspects for the diagnosis, as rules for rectoscopy, ultrasound examination and biopsy will be focussed upon. Correct indication, specifically in case of early cancer and for combined treatment will be an important topic, as well as prevention and treatment of complications. Tuebingen MIS Trainer The new standard for endoscopic training on a pelvitrainer A realistic surgical training requires an anatomical model, that allows the easy integration of animal organs. The regular repetition of procedures demands the quick preparation of the training setup. An optimal fixation of the organs is enabled with the help of a metal mesh. The anatomical shape of the mesh is realistic, the frontal abdominal wall is copied from that of the bowel of an insufflated patient.
