4 Einleitung Introduction 1. cerv-x ist ein Cage zur zervikalen intersomatischen Fusion bestehend aus röntgenstrahldurchlässigem PEEK Optima LT1. Der cerv-x wird über einen ventralen Zugang in den Zwischenwirbelraum einge bracht. Mit Hilfe des Cages ist eine Über brückung des Zwischenwirbel raumes möglich und damit die Wiederherstellung der Zwischen wirbelhöhe und die Re kon struktion des Wirbel säulen profils. Eine zusätzliche Stabilisierung mit einer zervikalen Platte, z.b. osmium wird dringend empfohlen. Um die korrekte Position des Implantates im Zwischenwirbelraum zu bestimmen, enthält der cerv-x-cage Röntgenmarker. Die Implantate werden einzeln steril ver packt geliefert. Zum Einsetzen der Im plan tate dürfen ausschließlich die dafür vorgesehenen Originalinstrumente des Implantatsystems verwendet werden. Die vorliegende OP-Technik beschreibt das Implantat und die Instrumente sowie die Arbeitsschritte für die Anwendung des cerv-x-cages. Sie ist als alleinige Grundlage für die erfolgreiche An wendung des cerv-x-cages nicht aus reichend. Es wird empfohlen, die Operations technik bei einem erfahrenen Operateur zu erlernen. Bitte beachten Sie die Gebrauchsanweisung für cerv-x Implantate und die Aufbereitungsanweisung für Instrumente. cerv-x is a cage for cervical inter body fusion, consisting of radiolucent PEEK Optima LT1. The cerv-x is inserted into the intervertebral space using an anterior approach. With the cage, it is possible to bridge the intervertebral space, thereby reconstructing the spinal profile and restoring the intervertebral height. An additional stabilization with a cervical plate is re commended, e.g. osmium. For establishing the correct position of the implant in the intervertebral space, the cerv-x cage contains radiographic markers. The implants are single sterile packaged. To insert the implants, only the original instruments from the implant system designated for this purpose may be used. The present surgical technique describes the implant and the instruments as well as the procedures for using the cerv-x cage. It is not sufficient as the sole basis for the successful application of the cerv-x cage. It is recommended to study and learn the operating technique with and from an experienced surgeon. 5 Indikationen und Kontraindikationen Indications and contraindications Indikationen cerv-x ist ein Implantat zur Abstützung des Zwischenwirbelraumes im Bereich der menschlichen zervikalen Wirbelsäule von C3 bis C7 mit dem Ziel der Unterstützung der Spondylodese. Die Implantate können zugleich zur Wiederherstellung des Wirbelsäulenprofils dienen. Eine zusätzliche Stabilisierung wird dringend empfohlen. Fusionsoperationen sind z.b. indiziert für: Degenerative Bandscheibenerkrankung Spondylolisthese und Retrolisthese Grad 1 Degenerative Skoliose Revisionsoperation Pseudarthrose Kontraindikationen Patienten mit Fieber oder Leukozytose Patienten mit Wirbelsäule assoziierten Infektionen (z. B. Spondylodiszitis) Patienten mit nachgewiesener Materialallergie oder Neigung zu Fremdkörperreaktionen Patienten mit einem ungünstigen medizinischen oder psychologischen Allgemeinzustand, der durch den Eingriff weiter verschlechtert werden könnte; bei diesen Patienten ist eine sorgfältige Abwägung durch den behandelnden Chirurg/Arzt vorzunehmen Patienten mit unzureichender Knochenqualität oder -quantität, z.b. schwere Osteoporose, Osteopenie, Osteomyelitis Schwangerschaft Indications cerv-x is an implant for supporting the intervertebral space in the cervical spine in humans from C3 to C7 with the purpose of providing support for spondylodesis. 8 Operationstechnik Surgical technique Patientenlagerung und Zugang Der Patient wird auf den Rücken gelagert. Die Lagerung, die Inzision und der Zugang erfolgen in typischer Weise zur Durchführung einer ventralen zervikalen Diskektomie. Patient positioning and approach The patient is placed in a supine position. The positioning, the incision and the approach are done in the usual manner for anterior cervical diskectomy. Diskektomie Der Zwischenwirbelraum wird dargestellt und das Bandschei ben material, z. B. mit Rongeuren, entfernt. Die Endplatten werden präpariert. Im Anschluss wird das Segment mit einem Spreizer für die zervikale Wirbelsäule distrahiert. Diskectomy The intervertebral space is exposed and disk material is removed with rongeurs for example. The end plates are prepared. Afterwards the segment is distracted by a retractor for the cervical spine. 5. Bestimmung der Implantatgröße Mit Hilfe des Probeimplantats wird die benötigte Implantatgröße bestimmt. Zur Auswahl stehen gewölbte Cages und Cages mit 5 Winkelung in 2 Größen mit Höhen von 4 bis 8 mm. Probeimplantate stehen ab der Höhe 5 mm zur Verfügung. Das Probeimplantat (CS XX, CS 8212-XX, CS 8214-XX bzw. CS 8216-XX) wird auf den Halter (CS 8210) aufgeschraubt. Die Probeimplantate sind farbkodiert. Die entsprechende Farbkodierung findet sich ebenfalls auf der Verpackung der Implantate. Determining the implant size By means of the trial implants the implant size required is determined. Es steht wahlweise ein Einsetzinstrument ohne Tiefenanschlag (CS ) und mit Tiefenanschlag (CS ) zur Verfügung. Der Cage wird auf das Instrument aufgesetzt und festgeschraubt. The safe, stable position of the cage is checked. Afterwards the inserter is removed. Important: Use of a C-arm control is recommended. Positionsbestimmung Mit Hilfe der Marker wird die Position des Cages bestimmt. Eine zusätzliche Stabilisierung mit einer zervikalen, ventralen Platte wird empfohlen.
