2 Verwendungszweck: Der hier beschriebene Assay ist für die Bestimmung von 1,25-(OH) 2 Vitamin D aus Serum und Plasma. Nur zur in vitro Diagnostik. Einleitung: Vitamin D wird entweder in der Haut (unter Einfluß von UV-Licht) gebildet oder aus der Nahrung aufgenommen. In der Leber entsteht die Speicherform des Vitamin D, das 25-hydroxyvitamin D. In der Niere wird in einem 2. Hydroxylierungsschritt die Hormonform des Vitamin D, das 1,25- dihydroxyvitamin D (D-Hormon) gebildet. Das dafür verantwortliche Enzym, die 1α-Hydroxylase der Niere, unterliegt einer strengen Kontrolle durch Hormone (insbesondere Parathormon) und wird in seiner Aktivität auch durch die Serumkonzentrationen von Calcium und Phosphat beeinflusst. Die Serumkonzentration von 1,25-dihydroxyvitamin D richtet sich also normalerweise nach den Erfordernissen des Stoffwechsels. Abweichungen des 1,25-dihydroxyvitamin D von der Norm müssen also immer im Kontext mit den übrigen Parametern des Calciumstoffwechsels interpretiert werden. Erst bei ausgeprägtem Vitamin D-Mangel wird auch die Serumkonzentration von 1,25-dihydroxyvitamin D absinken, zur Diagnostik des Vitamin D-Mangels sollte man deshalb den Vorläufermetabolit, das 25- hydroxyvitamin D messen. Ursachen für einen unphysiologischen Mangel an 1,25-dihydroxyvitamin D können jedoch Metabolisierungsstörungen entweder aufgrund genetischer Defekte der 1α-Hydroxylase (selten) oder aufgrund von Nierenfunktionsstörungen (häufiger) auftreten. Bereits bei leicht eingeschränkter Nierenfunktion kommt es zu einem Abfall der 1,25- dihydroxyvitamin D-Konzentration (Rickers et al. 1985). Da 1,25-dihydroxyvitamin D wichtige Funktionen im Calciumstoffwechsel hat und insbesondere auch die Parathormonsekretion in den Nebenschilddrüsen supprimiert, kommt es mit zunehmender Niereninsuffizienz zur Ausbildung der renalen Osteopathie, die durch Mineralisierungsstörungen (Osteomalazie) und fibröse Veränderungen (Osteitis fibrosa) gekennzeichnet ist. Die Behandlung der renalen Osteopathie besteht in der Gabe von 1,25- dihydroxyvitamin D (Calcitriol) oder des Prohormons 1α-hydroxyvitamin D. Erniedrigte oder relativ zu niedrige 1,25-dihydroxyvitamin D Spiegel findet man bei renalen Tubulusfunktionsstörungen (z.b. Phosphatdiabetes, Fanconi-Syndrom). Eine unphysiologische Überproduktion von 1,25- dihydroxyvitamin D tritt bei granulomatösen Systemerkrankungen (z.b. 1

3 Sarkoidose) auf, wo eine extrarenale 1,25-dihydroxyvitamin D Synthese stattfindet. Diese kann zur Hypercalciämie führen. Auch bei der idiopathischen Hypercalciurie findet man relativ zu hohe 1,25- dihydroxyvitamin D Spiegel. Reaktiv findet man erhöhte Konzentrationen von 1,25-dihydroxyvitamin D bei Störungen des Vitamin D-Rezeptors (selten), bei calciumarmer Ernährung, sowie bei Parathormonüberschuß (primärer Hyperparathyreoidismus) und bei manchen Tumorarten (infolge Sekretion von parathormonähnlichem Peptid, PTHrP). Indikationen: Nierenfunktionsstörungen Chronische Niereninsuffizienz Hämodialyse Nach Nierentransplantation Renale Osteopathie Osteomalazie bei V.a. gestörten Vitamin D-Metabolismus Nierentubulusfunktionsstörungen (Phosphatdiabetes, Fanconi-Syndrom) Überwachung einer Therapie mit aktiven Vitamin D-Metaboliten Ideopathische Hypercalciurie Hypercalciämie 2

4 Testprinzip: Der biologisch wirksame Vitamin D-Metabolit, das 1,25-(OH) 2 D, muß zu diesem Test auf einer Kieselgursäule (Extrelut) aus dem Probenmaterial extrahiert und anschließend mittels Kieselgelmaterial (Silica Kartuschen) von den anderen Vitamin D-Metaboliten (hauptsächlich dem 25-OH-D und dem 24,25-(OH) 2 D) abgetrennt werden. Zur quantitativen Erfassung des 1,25-(OH) 2 D konkurrieren im Radiorezeptorassay 3 H-markiertes 1,25-(OH) 2 D 3 als Tracer und das 1,25-(OH) 2 D aus der Probe um die begrenzte Anzahl von Bindungsstellen des spezifischen Rezeptors für 1,25-(OH) 2 D. Nach Reaktion des Rezeptors mit 1,25-(OH) 2 D wird der freie von gebundenem Tracer mit Aktivkohle abgetrennt. Inhalt der Testpackung: 1,5 ml Standard: NSB, 0, 6.6, 20, 60, 180 pg/ml 1,5 ml Kontrollserum 1,25-(OH) 2 D Rezeptor, lyophilisiert (in 11 ml eiskaltem aqua dest resuspendieren) 3 H-1,25-(OH) 2 D 3, Tracer < 12 kbq 15 ml Assay-Puffer (AP) 14 ml Aktivkohle-Suspension 30 ml Tris/HCl Puffer (TP) 2 ml Ethanol 3

5 Benötigte Ausstattung und Reagenzien: 50 Extrelut-Säulen 50 Silica-Kartuschen Diisopropylether Isopropanol Hexan β-counter Gefriertruhe und Kühlschrank Speedvac oder Stickstoffeindampfer Vorbereitung und Lagerung der Reagenzien: Standard, Kontrollen und NSB vor Gebrauch auftauen. Bei -20 C sind die Standards, Kontrollen und NSB bis zum angegebenen Verfallsdatum haltbar (siehe Etikett) 1,25-(OH) 2 D Rezeptor wird direkt vor dem Gebrauch in 11 ml eiskaltem aqua dest resuspendiert. Bei 2-8 C ist der lyophilisierte Rezeptor bis zum angegebenen Verfallsdatum haltbar (siehe Etikett). Resuspendierter Rezeptor ist nicht haltbar. 3 H-1,25-(OH) 2 D 3, Tracer < 12 kbq gebrauchsfertig. Haltbar bei -20 C bis zum angegebenen Verfallsdatum (siehe Etikett). Assay-Puffer (AP), gebrauchfertig. Haltbar bei -20 C bis zum angegebenen Verfallsdatum (siehe Etikett). Aktivkohle-Suspension, gebrauchfertig. Haltbar bei 4 C bis zum angegebenen Verfallsdatum (siehe Etikett). 4

6 Tris/HCl Puffer (TP), gebrauchfertig. Haltbar bei 4 C bis zum angegebenen Verfallsdatum (siehe Etikett). Ethanol, gebrauchfertig. Haltbar bei RT bis zum angegebenen Verfallsdatum (siehe Etikett). Vorbereitung des Probenmaterials: Plasma/Serum: Das Probenmaterial sollte bis zur Verwendung bei -20 C gelagert werden. Nach dem Auftauen für 10 min. bei 1800 x g zentrifugiert. Testduchführung und Probenvorbereitung. Aufbau der Extraktionsanlage: Extraktionseinheit 1. Säule 2.Säule regenerierbar RIA-Röhrchen 5

7 Der Extraktionsständer besteht aus 2 Plexiglas-Untereinheiten, die übereinander gestellt werden. In den oberen Teil werden die Extrelut-Säulen (Ständereinheit I) eingesetzt und in den untere Teil (Ständereinheit II) die Silica Kartuschen (regenerierbar) eingesetzt. Für die Schritte 1-4. (Auftragen des Probenmaterials auf die Säulen und waschen) wird die Extraktionseinheit (I und II) in eine Auffangschale gestellt. Vor dem Elutionschritt (Punkt 5) werden die Extrelut-Säulen entfernt. Ständereinheit I dient nun zur Aufnahme der Reagenzgläser zum Auffangen des Eluats und wird unter die Ständereinheit II positioniert. Extraktion mit Extrelut undsilica-säulen: 1. Die Extrelut-Säulen (Ständereinheit I) mit je 500 µl Tris-Puffer äquilibrieren µl der Standards, Kontrolle bzw. Proben in Duplikaten auftragen und 10 min. einziehen lassen. 3. Mit 4 x 1 ml Diisopropylether in je 3 min. Abstand das Vitamin D von den Extrelut Säulen in die Silica-Kartuschen extrahieren. (Nach der Extraktion müssen die Extrelutsäulen entfernt werden.) 4. 5 x 2 ml Isopropanol/Hexan (4/96 v/v) und 3 x 2 ml Isopropanol/Hexan (6/94) waschen. (Silica-Säulen Ständereinheit II) 5. Die Elution des 1,25-(OH) 2 D erfolgt mit 2 x 2 ml Isopropanol/Hexan (25/75 v/v). Hinweis: Es ist darauf zu achten, daß die Reagenzgläser (Ständereinheit I) exakt unter den Säulen stehen. 6. Die eluierten Proben werden unter leichtem Stickstoffstrom bei 37 C oder in einer Vakuumzentrifuge eingetrocknet. Der eingetrocknete Proben, Kontrollen und Standards können bei -20 C 24h gelagert werden. 6

8 7. 20 µl Ethanol zu jeder eingetrockneten Proben bzw. jedem Standard und Kontrollen pipettieren und vortexen µl eiskalten Assaypuffer in jedes Röhrchen hinzufügen und vortexen µl des resuspendierten Rezeptormaterial (frisch ansetzen) in jedes Röhrchen pipettieren, vorsichtig vortexen min bei Raumtemperatur inkubieren(20-23 C) µl Tracer zufügen und vorsichtig vortexen min bei RT (20-23 C) inkubieren µl eiskaltes DCC (Dextran coated Charcoal) zufügen, vorsichtig vortexen. 14.Zentrifugation für 10 min bei 2000 x g. 15.Überstand in Szintillationsgefäße dekantieren, 3 ml Szintillator zugeben, schütteln (Bei Verwendung moderner Sicherheitsszintilatoren wie Aquasafe 300 oder HiSafe III ist dem zu messenden Überstand die 1,5-2 fache Szintillator-Menge zuzusetzen). 16.Im β-counter 1 min messen. Regenerierung der Silicakartuschen: Die unteren Säulen werden nach folgendem Schema regeneriert. die Säulen können direkt nach der Extraktion regeneriert werden und müssen vor dem nächsten Gebrauch trocken sein. 2 x 2 ml Methanol auftragen. 2 x 2 ml n-hexan auftragen. Säulen danach unter dem Abzug trocknen lassen. 7

9 Auswertung: Zur Auswertung des Testes empfehlen wir die 4-Parameter-Funktion. Alternativ kann auch eine Punkt-zu-Punkt-Auswertung oder eine gewichtete Spline-Funktion gewählt werden. Vor jeder automatischen Auswertung sollte stets eine Kontrolle der Doppelwerte auf Plausibilität ( Ausreißerkontrolle ) durchgeführt werden; falls dies nicht durch das verwendete Programm erfolgt, sollte diese der Operator durchführen. 10 Nachweisgrenze: Die Nachweisgrenze dieses 1,25-Dihydroxy-Vitamin D wurde festgesetzt als B0 + 2SD. Sie ist < 2 pg/ml. Normbereich (Plasma oder Serum): Gesunde Erwachsene Jahre: Kinder bis 12 Jahre: Schwangere ( Gestationswoche): Personen über 70 Jahre: pg/ml bis zu 40% höhere Werte bis zu 60% höhere Werte bis zu 40% niedrigere Werte Es treten keine jahreszeitlichen Schwankungen auf. Radio Immuno-Assays zeigen in direkten Vergleichsstudien gegen Radio-Rezeptor-Assays bei bestimmten Patientenkollektiven bis zu 50% höhere Werte 4),5). Wir empfehlen jedem Labor einen eigenen Normwertbereich zu etablieren. Qualitätskontrolle: Kontrollen: 1,25-(OH)2 D Kontrollserum: Der Konzentrationsbereich ist dem beigefügtem Datenblatt entnehmen Linearität: Der lineare Bereich erstreckt sich von pg/ml. Die Linearität dieses Testes ist gegeben bei einem Probenvolumen von 100 µl bis 500 µl. 9

13 Allgemeine Hinweise zum Test: Dieser Kit wurde nach der IVD Richtlinie 98/79/EG hergestellt und in den Verkehr gebracht. Falls für die Herstellung der Testkomponenten Humanseren verwendet wurde, sind diese auf HIV und Hepatitis B getestet und für negativ befundet worden. Jedoch sollten die Testkomponenten als Vorsichtsmaßnahme immer wie potentiell infektiöses Material behandelt werden. Die Testkomponenten enthalten zum Schutz vor bakteriellen Kontaminationen Natriumazid. Natriumazid ist giftig. Substrate für enzymatische Farbreaktionen sind als giftig und karzinogen beschrieben. Jeder Kontakt mit der Haut oder der Schleimhaut ist zu vermeiden. Alle im Test enthaltenen Reagenzien dürfen ausschließlich zu wissenschaftlichen Zwecken oder, wenn vermerkt, zur in vitro Diagnostik eingesetzt werden. Die Reagenzien dürfen nach Ablauf des auf der Verpackung angegebenen Datums nicht mehr verwendet werden. Einzelkomponenten verschiedener Chargen dürfen nicht gemischt oder ausgetauscht werden. Für die Qualitätskontrolle sind die, für medizinische Laboratorien erstellten Richtlinien zu beachten. Die charakteristischen Testdaten wie Inkubationszeiten, Inkubationstemperaturen und Pipettiervolumina der verschiedenen Komponenten wurden firmenintern festgelegt. Nicht mit dem Hersteller abgesprochene Veränderungen in der Testdurchführung können die Resultate beeinflussen. Die Firma Immundiagnostik GmbH übernimmt für die hierdurch entstandenen Schäden und Folgeschäden keine Haftung. Bei Gewährleistungansprüchen ist das beanstandete Material mit schriftlicher Erklärung innerhalb von 14 Tagen zum Hersteller- der Immundiagnostik GmbH zurück zu senden. 12

15 1,25 Dihydroxy-Vitamin D3 Summary and explanation: Vitamin D is either produced in the skin (under the influence of UV light) or taken up from nourishment. The storage type of vitamin D, namely 25- hydroxyvitamin D is formed in the liver. In a second hydroxylation step the hormone version of vitamin D, namely 1,25-dihydroxyvitamin D (D hormone) is formed in the kidneys. The responsible enzyme, 1αhydroxylase of the kidneys, is subjected to a rigid control through hormones (especially parathyroid hormone) and its activity is influenced by the serum concentrations of calcium and phosphate. The serum concentration of 1,25-dihydroxyvitamin D normally readjusts itself to the demands of metabolism. Deviations from the norm of 1,25- dihydroxyvitamin D must therefore always be interpreted in the context of the remaining parameters of the calcium metabolism. The serum concentration of 1,25-dihydroxyvitamin D only falls in extreme cases of vitamin D deficiency. For diagnostics of vitamin D deficiency one should therefore measure the fore-runner metabolite, 25-hydroxyvitamin D. The reason for a non-physiological deficiency of 1,25-dihydroxyvitamin D can be found in metabolic disturbances, caused either by genetic defects in the 1α-hydroxylase (rare) or kidney malfunctions (more common). Even a slightly impaired kidney function can lead to a decrease of the 1,25-dihydroxyvitamin D concentration (Rickers et al. 1985). Since 1,25-dihydroxyvitamin D has important functions in calcium metabolism as well as supplementing secretion of parathyroid hormone from the parathyroid glands, increasing kidney malfunctioning leads to development of renal osteopathy, which is characterized by mineralization disturbances (osteomalacia) and fibrotic alterations (Osteitis fibrosa). Treatment of renal osteopathy consists of the administration of 1,25- dihydroxyvitamin D (Calcitriol) or the prohormone 1α-hydroxyvitamin D. In renal tubules malfunctions one finds lowered or relatively too low levels of 1,25-dihydroxyvitamin D (e.g. diabetes insipidus, Fanconi-Syndrom). A nonphysiological over-production of 1,25-dihydroxyvitamin D arises in granulomatose systems diseases (e.g. sarcoidosis), where extra-renal synthesis of 1,25-dihydroxyvitamin D occurs. This can lead to hypercalcaemia. Also in idiopathic hypercalciuria one finds a relatively too high level of 1,25-dihydroxyvitamin D.As a reaction, one finds increased concentrations of 1,25-dihydroxyvitamin D in disturbances of the vitamin D receptor (rare), in calcium deficient nutrition, as well as from over production of the parathyriod hormone (primary hyperthyroidism) and with some types 1

16 1,25 Dihydroxy-Vitamin D of tumors (which result from the secretion of a peptide similar to parathyroid hormone, PTHrP). Test principle: For this test, the biological effective Vitamin D metabolite 1,25-(OH) 2 D 3, has to be extracted on a diatomaceous column. Following it is necessary to separate the 1,25-(OH) 2 D 3 from other Vitamin D metabolites especially from the 25-(OH) D 3 and the 24,25-(OH) 2 D 3. In the radio receptor assay (), for the quantitative determination of 1,25- (OH)D3, 3 H labeled 1,25-(OH) 2 D 3 tracer and extracted 1,25-(OH) 2 D 3 of the sample compete for a limited number of 1,25-(OH)2 D3 receptors. After reaction of the receptor with 1,25-(OH) 2 D 3 the free tracer is separated by means of an activated charcoal suspension. 18 1,25 Dihydroxy-Vitamin D Reagent preparation and storage: Standards, controls and NSB should be kept at room temperatur before use. The standards, control and NSB are stable at 20 C until the expiry date given on the label. Reconstitute the 1,25(OH) 2 D receptor in 11 ml cold aqua dest. The lyophilized receptor is stable at 2-8 C until the expiry date given on the lable. Reconstitute receptor ist not stabel. The 3 H 1,25(OH) 2 D 3 tracer is ready to use. The tracer is stable at 20 C until the expiry date given on the label. The assay buffer is ready to use and stable at 20 C until the expiry date given on the label. The dextran coated charcoal suspension is ready to use and stable at 4 C until the expiry date given on the label. The Tris/HCL buffer is ready to use and stable at 4 C until the expiry date given on the label. Ethanol is ready to use and stable at room temperatur until the expiry date given on the label. Sample collection and storage: Plasma/Serum If the samples are not used in between 24 h the samples should be stored at 20 C. 4

19 1,25 Dihydroxy-Vitamin D Assay procedure and sample preparation: Structure of the extraction unit: The extraction unit consists of two parts, which were put on top of each other. The upper part is used for the Extrelut columns, and the lower part is used for the silica columns. During the apply of the samples and the washing procedure, the whole unit should be put into bowl, to collect the extraction solvents. For the last elution of the silica columns, the RIA-tubes should be placed directly under the columns. Therefore the tubes could be used directly for the test procedure, after evaporation of the solvents. Extraction unit 1. column 2. column RIA-tubes 5

After thawing the sample has to be centrifuged for 10 min. at 1800 x g µl of the Tris Buffer is used to equilibrate the Extrelut columns µl standards, controls and sample (plasma or serum) are applied on the Extrelut-column. They need to be absorbed by the column-material for about 10 minutes. 4. The Vitamin D is extracted from the columns with 4 x 1 ml diisopropylether (3 min for each elution). The eluate is applied directly on a untreated and dry silica cartridge. (After the extraction the columns should be remove.) 5. Afterwards the cartridge is washed with 5 x 2 ml Isopropanol/Hexan (4/96 v/v) and 3 x 2 ml Isopropanol/Hexan (6/94 v/v). 6. The 1,25-(OH) 2 D 3 elution is carried out in giving 2 x 2 ml Isopropanol/Hexan (25/75 v/v) on the column. (Note: Please have attention that the tubes stand directly under the columns.) 7. The eluate is evaporated under a nitrogen stream at 37 C or in a speed vac. Further steps please refer to the assay procedure. The evaporated samples could be stored at 20 C for 24 h. 8..Add 20 µl of ice-cold Ethanol in all tubes (of dried standards, controls and samples). Carry out the test in duplicates 9. Add 250 µl ice-cold assay buffer (4 C) to all tubes and vortex. 10. Add 200 µl ice-cold receptor solution to all tubes also in NSB and vortex carefully. 11. Incubate for 60 min at room temperature (20-23 C max.) 12. Add 20 µl of 3 -H labeled 1,25-(OH) 2 D 3 vortex carefully. 13. Incubate for 60 min at room temperature (20-23 C). 14. Add 200 µl ice cold activated charcoal suspension and vortex carefully. 15. Centrifugation for 10 min at 2000 x g, 4 C. 16. Decant the supernatant into scintillation vials. Add 3 ml scintillation-fluid in the scintillation vials, mix well. Determine 3 H-activity in an ß-counter for 1 min 6

21 1,25 Dihydroxy-Vitamin D Regenerate of the silica columns The silica columns could be regenerated and used several times after the following procedure, after every use: Add 1 x 2 ml Methanol Afterwards, add 1 x 4 ml n-hexan After this procedure, the columns should be dried on air (24-48 hours) Calculation: For the calculation of this test we recommend the 4 parameter logistic. Alternatively one could use the point to point calculation or spline function. Calculation with the spline function requires a control of the duplicates. Example for standard curve: B/Bo pg/ml 7

23 1,25 Dihydroxy-Vitamin D General notes on the test and test procedure: This assay was produced and put on the market according to the IVD guidelines of 98/79/EC. The test components which are made of human serum are tested for Australia antigen and HIV and found to be negative. However, since no test method can offer complete assurance that infectious agents are absent, these reagents should be handled as recommended for any potentially infectious human serum or blood specimen. The normal precautions for laboratory working should be observed. Reagents of the test package contain sodium azide as a bactericide. Contact with skin or mucous membranes is to be avoided. All reagents in the test package are to be used only for in-vitro diagnostics. The reagents should not be used after the date of expiry (see label on the test package). Single components with different lot numbers should not be mixed or exchanged. For quality control, the guidelines for medical laboratories should be observed. The characteristic test data, such as incubation time, incubation temperature and pipetting volumina of the different components are defined by the producer. Any variations of the test procedure, that are not coordinated with the producer, may influence the results of the test.
