2 HINTERGRUND Fragestellung: Warum ist die Prävalenz von myofaszialen (nicht-spezifischen) Rückenschmerzen soviel größer als die von Schmerzen in Extremitätenmuskeln? Arbeitshypothese: Paraspinale tiefe Rückenmuskeln (bes. M. multifidus (MF)) haben neuroanatomisch stärkere und/oder synaptisch effektivere Verbindungen mit dem ZNS als der M. gastrocnemius-soleus. Schmerzhafte chemische Reizung des M. multifidus erregt mehr Hinterhornneurone als die des M. gastrocnemius-soleus und löst bei wachen Ratten mehr Schmerz aus. Insgesamt hat der MF effektivere Verbindungen mit höheren nozizeptiven Zentren als der GS. Diese Befunde können zumindest teilweise die hohe Prävalenz von chronischen unspezifischen Rückenschmerzen erklären. 9
