Was ist Qualität in der gesundheitlichen Versorgung? Qualität ist... the kind of care which is expected to maximize an inclusive measure of patient welfare, after one has taken account of the balance of expected gains and losses that attend the process of care in all its parts (Donabedian 1980) Technical performance: ability to achieve improvements in health status made possible by current knowledge in and technology of healthcare Interpersonal performance: process by which technical performance is implemented the degree to which health services for individuals and populations increase the likelihood of desired health outcomes and are consistent with current medical knowledge (Institute of Medicine 1990) doing the right things (what), to the right people (to whom), at the right time (when), and doing things right first time (Department of Health [UK] 1997)

Anforderungen an Qualitätsindikatoren Annahme: beobachtete Veränderung einer Qualitätsmessgrösse reflektiert eine Veränderung in der zugrundeliegenden Versorgung/squalität Indikatoren müssen bestimmte Grundvoraussetzungen erfüllen, damit Aussagen über Ursache und Wirkung bzw Handlungansweisungen abgeleitet werden können Quelle: Nolte 2010
Kriterien der Indikatorenwahl nach NQF Relevanz (importance) Der Indikator informiert über einen für Entscheidungsträger wichtigen Sachverhalt Wissenchaftlich akzeptabel (scientific acceptability) Der Indikator erfasst die zugrundeliegende/n Annahme/ in valider und wiederholbarer Weise und basiert auf existierender wissenschaftlicher Evidenz oder professionellem Konsens Praktikabel (feasibility) Die dem Indikator zugrundeliegenden Daten lassen sich mit verhältnismässig geringem Aufwand zusammenstellen Nutzen (usability) Der Indikator informiert in einer Weise dass Entscheidungsträger handeln können Quelle: nach National Quality Forum 2009
Indikatorenentwicklung kann problematisch sein Indikatoren sollten auf Basis vorhandener Evidenz entwickelt werden mit Ausnahme einiger (weniger) Bereiche (zb Diabetes) ist diese Voraussetzung aber nicht unbedingt gegeben Auswahl von Indikatoren erfolgt daher oftmals auf Basis von Expertenkonsultation Unterschiede in der Perspektive verschiedener Professionen bezüglich der Definition von Qualität (zb Allgemeinmedizin Orthopädie) Einsatz von formalen Konsultationen (zb Delphi): Zuzammensetzung des Panels and Prozess hat Einfluss auf die letztliche Wahl von Indikatoren Übertragbarkeit von Indikatoren zwischen Gesundheitssystemen Unterschiede in der Interpretation der Evidenz Unterschiede in der verfuegbaren Evidenz (zb Studienergebnisse nur in ausgewählter Sprache verfügbar) Quelle: Nolte 2010

The purpose of this indicator is to discourage unnecessary back surgery people should not have back surgery within a few weeks of onset of pain unless there are imperative clinical reasons ( red flags ) for surgery. This could be regarded as an indicator of Clinical effectiveness: patients should not have treatments which are not clinically indicated Safety: patients are exposed to harm by having unnecessary operations) Efficiency: unnecessary operations waste resources
