F7117 Beschreibung Dezember 2016 SureFast MRSA 4plex ist eine real-time PCR zum direkten qualitativen Nachweis und zur Differenzierung von DNA-Sequenzen der Methicillin-resistenten Staphylococcus aureus (MRSA). Es wird die SCCmec/orfX junction, die resistenzspezifische Sequenz meca/mecc und eine Staphylococcus aureus spezifische DNA-Sequenz nachgewiesen. Der Test ist mit einer Extraktionskontrolle (Internal Control DNA) ausgestattet, die gleichzeitig auch als interne Amplifikationskontrolle verwendet werden kann. Das Nachweisverfahren kann mit allen gängigen real-time PCR Geräten, die mindestens vier Reporterfarbstoffe gleichzeitig bei 522 nm, 553 nm, 610 nm und 670 nm (FAM, VIC, ROX und Cy5) detektieren können, verwendet werden. Die technische Gerätevalidierung erfolgte am Agilent Mx3005P, BioRad CFX 96, Roche LightCycler 480 II 1, Applied Biosystems 7500 und Cepheid SmartCycler. Nachweisgrenze Das SureFast MRSA 4plex real-time PCR hat eine Nachweisgrenze von 5 DNA-Kopien. Die Nachweisgrenze des Gesamtverfahrens ist abhängig von Probenmatrix, Prozessierungsgrad, DNA-Präparation und DNA-Gehalt. Zusätzliche benötigte Geräte und Materialien Real-time PCR Gerät mit vier Detektionskanälen (522 nm, 553 nm, 610 nm und 670 nm) Real-time PCR Verbrauchsmaterialien (Platten, Gefäße, Folien, Deckel) Pipetten, Pipettenspitzen mit Filtern Einmalhandschuhe Vortexmischer Mikrozentrifuge mit Rotor für Reaktionsgefäße Protokoll 1. DNA-Präparation Dieser Test enthält eine Internal Control DNA (ICD), die entweder nur als interne Amplifikationskontrolle oder als Extraktionskontrolle für die Probenpräparation und Inhibitionskontrolle verwendet werden kann. Wird die ICD nur als Inhibitionskontrolle verwendet, muss 1 µl der ICD dem Master-Mix hinzugefügt werden. Wird die ICD als Extraktionskontrolle für die Probenpräparation und Inhibitionskontrolle verwendet, müssen 20 µl der ICD während der Extraktion eingesetzt werden. Die ICD soll dem Proben-Lysisbuffer Mix und nicht direkt dem Probenmaterial zugefügt werden. Für Tupfer: In ein Reaktionsgefäß 200 μl Lysis Buffer 1 geben. Den Tupfer in den vorgelegten Lysis Buffer 1 eintauchen und den Stab abbrechen oder abschneiden. Das Reaktionsgefäß dicht verschließen und 60 Sekunden stark vortexen. Danach im Heizblock für 10 Minuten bei 95 C unter Schütteln erhitzen. Anschließend bei rpm 1 min zentrifugieren und den Überstand als Probe einsetzen. Für Abschwemmungen und Anreicherungen: Hierfür wird das SureFast PREP Bacteria Kit empfohlen. Um das Wachstumspotenzial des Bakteriums besser beurteilen zu können, wird empfohlen, die Proben zu Beginn und am Ende der kulturellen Voranreicherung zu analysieren (Wachstum ab einer Cp-Wert Differenz von > 3). 1 Für die Benutzung des Roche LightCyler 480 II ist eine Color Compensation (Farbstoffkalibrierung) notwendig. 2 Seiten SureFast MRSA 4plex (100 Reakt.) Art. Nr. F7117 Dezember Herstellen des Master-Mix Die Gesamtzahl der für die PCR benötigten Reaktionen (Proben und Kontrollreaktionen) ist zu berechnen. Folgende Kontrollen werden empfohlen: Positivkontrolle, Negativkontrolle und Blank der Extraktion. Dieser Test enthält eine Internal Control DNA (ICD), die als Extraktions- bzw. Inhibitionskontrolle eingesetzt werden kann. Es wird empfohlen den Mix mit 10 % zusätzlichem Volumen anzusetzen, um einen Pipettierverlust auszugleichen. Vor der Benutzung die Reagenzien auftauen, vortexen und zentrifugieren. Die Taq Polymerase sollte nicht aufgetaut und nicht im Vortex gemischt werden. F7117 Interpretation der Ergebnisse Dezember 2016 Die Auswertung der Ergebnisse wird mit der Analyse Software der jeweiligen real-time PCR Geräte nach den Angaben des Herstellers durchgeführt. Negativ- und Positivkontrollen müssen die korrekten Ergebnisse zeigen. Die SCCmec/orfX junction wird im FAM-Kanal, Staphylococcus aureus im ROX-Kanal, das meca/mecc-gen im Cy5-Kanal und die interne Amplifikations- bzw. Extraktionskontrolle im VIC/HEX-Kanal nachgewiesen. Eine Probe wird positiv für den jeweiligen Parameter (SCCmec, Staphylococcus aureus, meca/mecc) bewertet, wenn die Proben-DNA eine Amplifikation im jeweiligen Kanal zeigt. Eine Probe wird MRSA positiv bewertet, wenn die Proben-DNA eine Amplifikation für die Parameter SCCmec, Staphylococcus aureus und meca/mecc (FAM/Rox/Cy5) zeigt Eine Probe wird als negativ für den jeweiligen Parameter bewertet, wenn die Proben-DNA keine Amplifikation im jeweiligen Kanal zeigt und die interne Amplifikationskontrolle (VIC/HEX-Kanal) positiv ist. Eine Probe, die negativ für alle Parameter (SCCmec, Staphylococcus aureus, meca/mecc) und negativ im VIC/HEX-Kanal ist, kann nicht bewertet werden. In diesem Fall sind in der Probe PCR-Inhibitoren vorhanden oder die Nukleinsäure-Extraktion hat nicht ordnungsgemäß funktioniert. Die Isolierung und Reinigung der DNA aus der entsprechenden Probe muss verbessert werden. No. F7117 Description December 2016 SureFast MRSA 4plex is a real-time PCR for the qualitative detection and differentiation of methicillin-resistant Staphylococcus aureus (MRSA). The test detects the SCCmec/orfX junction, the meca/mecc-gene and a specific DNA sequence of Staphylococcus aureus. The test contains an Internal Control DNA (ICD) as an internal control of sample preparation procedure and to monitor possible PCR-inhibition. The real-time PCR assay can be used with established real-time PCR instruments, equipped for detection of four fluorescence emissions at 522 nm, 553 nm, 610 nm and 670 nm (FAM, VIC, ROX and Cy5) at the same time. The technical validation of instruments was performed on Agilent Mx3005P, BioRad CFX 96, Roche LightCycler 480 II 1, Applied Biosystems 7500 and Cepheid SmartCycler. Limit of Detection The SureFast MRSA 4plex real-time PCR has a limit of detection of 5 DNA copies. The assay limit of detection depends on sample matrix, processing grade, DNA preparation and DNA content. DNA-preparation The test assay contains an Internal Control DNA (ICD), which can either be used as PCR inhibition control or as extraction control for the sample preparation procedure and as a PCR inhibition control. If the ICD is used only as a PCR inhibition control, 1 µl of the ICD should be added to the Master-Mix. If the ICD is used as an extraction control for the sample preparation procedure and as PCR inhibition control, 20 µl of the ICD should be added during extraction procedure. The ICD should always be added to the specimenlysis buffer mixture and must not be added directly to the specimen. For swabs Add 200 μl Lysis Buffer 1 into a preparation tube. Insert the swab in the pre-pipetted Lysis Buffer 1 and cut or break the swab stem. Cap the preparation tube tightly and vortex strongly for 60 seconds. Heat and shake the preparation tube at 95 C for 10 minutes in a heating block. Centrifuge for 1 minute at 12,000 rpm and apply the supernatant as sample. For avulsions and enrichments Therefore, the use of SureFast PREP Bacteria is recommended. To assess the process of bacterial growth, it is recommended to compare the samples at the beginning and at the end of the culturing (bacterial growth at Cp difference > 3). 1 note: For the use of the Roche LightCycler 480 II a Color Compensation is necessary. Recommended control reactions: negative control, positive control and blank of extraction. The test assay contains an Internal Control DNA (ICD), which can either be used as PCR inhibition control or as extraction control. It is also recommended to prepare the master-mix with 10 % additional volume in order to compensate reagent loss. Allow the reagents to thaw, mix by vortexing and centrifuge before opening and use. Preparation of the PCR-mix Pipette 20 µl of the master-mix into appropriate tubes or wells. For the negative control pipette 5 µl of PCR Water into the designated tubes and close them. No. F7117 Interpretation of results December 2016 The evaluation has to be made according to the usual analysis program recommended by the real-time PCR instrument manufacturer. The control reactions need to give the correct results. A sample is stated positive for the respective parameter (SCCmec, Staphylococcus aureus, meca/mecc), if the sample DNA shows an amplification in the respective channel. A sample is stated positive for MRSA, if the sample DNA shows an amplification in the FAM, ROX and Cy5-channel (SCCmec, Staphylococcus aureus, meca/mecc). A sample is stated negative for the respective parameter (SCCmec, Staphylococcus aureus, meca/mecc), if the sample DNA shows no amplification in the respective channel and the internal amplification control (VIC) of the sample is positive. If the sample DNA and the internal amplification control are negative the sample contains PCR-inhibiting substances or the sample preparation was not successful. Under these circumstances an evaluation of the samples is not possible. DNA isolation and purification for the sample need to be improved.
