14 Die chronische Wunde ist ein spezielles Krankheitsbild welches einer systematischen krankheitsspezifischen Diagnostik und Therapie bedarf Die Vorstellung, dass chronische Wunden lediglich eine Variante akuter Wunden seien gilt als überholt!

20 Wachstumsfaktoren als Externa applizieren? Topische Applikation rekombinant hergesteller Wachstumsfaktoren - konnten zwar bei normaler Wunden die Heilungsgeschwindigkeit verbessern - deren Einsatz bei Patienten mit Wundheilungsstörungen (und lokal erhöhter Proteaseaktivität) dagegen verlief eher enttäuschend Einzig Platelet Derived Growth Factor-BB (PDGF-BB) steht aktuell als definiertes Präparat zur Therapie von diabetischen Fussulzera zur Verfügung. Auch bei Dekubitalgeschwüren scheint PDGF-BB, topisch appliziert, vorteilhaft zu sein.

21 Aus chronischen Wunden isolierte Fibroblasten zeigen vermindertes Ansprechen auf exogene Applikation von Wachstumsfaktoren => GROWTH TRAP THEORY Fibriogen und Fibrin sind in chronischen Wunden oft massenhaft vorhanden möglicherweise binden diese Makromoleküle Wachstumsfaktoren wodurch jene ihre Wirkung nicht entfalten können

23 After skin wounding, factors involved in - hemostasis - coagulation - inflammation may create a local (stress) environment that favors Nox (NADPH oxidase) activation Nox-derived ROS have been implicated in promoting keratinocyte proliferation (Nakai et al., 2008) Nox activation has a positive role in regulating proliferation of collagen-producing cells, including fibroblasts (Chan et al., 2009)

24 NOX = NADPH oxidases are important enzymatic sources of ROS From findings of the recent years it must be concluded that the activity control of Nox enzymes is much more complex than anticipated Moreover, depending on the cellular activity state, Nox enzymes are selectively activated or inactivated redox-balance

25 Redox modulation of transcriptional factors by ROS While ROS are known to function as harmful products of aerobic metabolism (when present at high concentrations) low levels of these prooxidant molecules have more recently been discovered to modulate transcription factor activation

26 Redox modulation of transcriptional factors by ROS (2) A growing number of molecules in the wide range of signal transduction pathways such as - many kinases - phosphatases - transcription factors are thought to be modulated by intracellular redox status Moreover, a few transcription factors (such as the small GTP-binding protein Rac) are known to activate ROS-generating enzymes (e.g. NADPH oxidase) and produce ROS as a modulator of downstream molecules

34 Protein tyrosine phosphatases (PTPs) are another family of intracellular targets of Nox-derived ROS. PTPs have an essential role in down-regulating receptor tyrosine kinase (RTK)-mediated signaling by dephosphorylating RTKs. Receptor tyrosine kinases (RTKs) are the highaffinity cell surface receptors for many polypeptide growth factors, cytokines, and hormones Based on these data, we suggest that modulation of RTK signaling via PTP inactivation by Nox-derived ROS may have a significant contribution in mediating the activation of ERK MAPK pathway observed in various cellular stress responses

42 Schlussfolgerungen: 1. Ox Stress und resultierende Störungen in Signal-Kaskaden dürften (auch in unterschiedlichen Lebensstadien - jung, mittleres Alter, Senium) unterschiedliche physiologische Auswirkungen haben 2. Den Veränderungen der Redox-Balance (an redoxsensiblen Signalkaskaden) infolge variierendem ox Stress dürften grössere Bedeutung zukommen als der blossen Anhäufung von oxidativ geschädigten Molekülen und Zellbestandteilen 3. Veränderungen des ox Stress und des Redox-Status in den verschiedenen Zellkompartimenten (Zytosol, Mitochondrien, Nukleus) haben unterschiedliche Auswirkungen auf Pathophysiologie und den Alterungprozess
