37 Kapitel 4 Diagnose: Bildgebung Statement Besides history and physical examination, transabdominal sonography has first preference. In the case of non conclusive signs of pancreatitis (inhomogeneous organ, main pancreatic duct not dilated) but presence of clinical suspicion, endosonography should be performed. Endosonography guided fine needle puncture (EUS-FNP) enables cytological and histological evaluation of focal lesions. CT and MRT as well as MRCP are complementary procedures in non conclusive findings of sonography and endosonography. MRCP is the preferred method for evaluation of pancreatic ducts. Furthermore, the disease is associated with an increased risk of pancreatic cancer. Thus surveillance and regular clinical follow up is recommended after making primary diagnosis. [evidence, 4, grade of recommendation A, agreement] Statement Occurence of endocrine insufficiency leads to late diabetes related complications in cases of insufficient control of metabolism. Diabetes due to a loss of pancreatic parenchyma is associated with an increased risk of hypoglycemia. Hypoglycemia is associated with increased mortality. Statement Weight loss, which was not intended by the patient, newly diagnosed diabetes mellitus, changes of the characteristics of pain, newly diagnosed cholestasis without concomitant pain, or relapsing pancreatitis without an underlying obvious cause, should lead to a further diagnostic work up.
