Each type of reporting system has its advantages and disadvantages. 2. A mandatory system should be accompanied by regulations on sanctionsfree reporting and clear rules on confidentiality. Frontline personnel in hospitals and in the primary care sector are obligated to report adverse events to a national reporting system 2. Patients and relatives may report adverse events 3. Hospital owners are obligated to act on the reports 4. The National Board of Health is obligated to communicate the learning nationally 15

19 Beispiel UK: Never events policy The aim of this policy is to reduce the incidence of never events to zero. They are intolerable and inexcusable. Never Events are serious, largely preventable patient safety incidents that should not occur if the available preventative measures have been implemented. 2.9: Failure to report a never event is simply unacceptable and a sign of real cultural and safety failings in an organisation. 2.10: Reporting serious incidents is a legal requirement under CQC regulations. Never events are clearly defined as serious incidents. 6.1: Failure to report a never event is an extremely serious failing on the part of the staff involved as well as the organisation. 24 Fazit Mandatorisches Reporting nimmt zu, in allen Typen von Gesundheitssystemen Verschiebung von "Lern-Perspektive" zu mehr Verantwortlichkeit und Transparenz Fokussierung auf Ereignisse mit schwerem Schaden (minimum safety standards) Chance zur Monitorisierung von "harm" Verbesserungsarbeit zielgerichtet nachzuverfolgen Auch bei mandatorischen Systemen gibt es ein (massives?) under-reporting Qualität und Detailgrad der Meldungen unterschiedlich Mandatorisches Reporting meist verbunden mit Verpflichtung zur Aufarbeitung Zum Teil Gegenstand von Leistungsaufträgen und Zielvereinbarungen Freiwillig vs. Mandatorisch ist nie "entweder-oder" sondern "sowohl-als auch" 24
