2 Entwicklung des SSI-Protokolls In Verwendung derzeit das Protokoll in Version 1.02 (HAI-Net SSI Protocol, version 2.2; May 2012) im Q fand eine Pilot-Erhebung zur Machbarkeitsprüfung des überarbeiteten HAI-Net SSI Protokolls Anschließend, wurde im Mai 2017 die finale Version des Protokolls vom ECDC publiziert (HAI-Net SSI Protocol, version 2.2; May 2017) 2

3 Neuerungen im SSI-Protokoll Die wichtigsten Änderungen umfassen: Struktur- und Prozessindikatoren (SPIs) zur Datenerfassung auf Krankenhaus- / Abteilungsebene wurden hinzugefügt; die Verwendung der neuen Variablen wird für alle teilnehmenden Länder und Krankenhäuser empfohlen Die Nachbeobachtungszeit für tiefe und Organ/ Körperhöhle-WI, wenn ein Implantat vorhanden ist, wurde von einem Jahr auf 90 Tage verkürzt Es wurde eine neue Variable für das Implantat hinzugefügt, die an die Länge des zu verwendenden Follow-up-Zeitraums gekoppelt ist 3

4 Neuerungen im SSI-Protokoll neue Variable auf der Infektionsebene, um die mit postoperativen WI assoziierte höheren Mortalität besser beurteilen zu können neue Variable für mehrere durch denselben Schnitt durchgeführten chirurgischen Eingriffe, in derselben operativen Sitzung auch Herzoperationen (CARD) und Operation am Rektum (REC) melden andere Kodierungen für chirurgische Verfahren als ICD-9-CM erlaubt, falls ICD-9-CM nicht berichtet werden kann 4

5 Neuerungen im SSI-Protokoll Die ASA-Klassifikation des präoperativen Gesundheitszustands wurde gemäß der Version 2014 aktualisiert: die Leistungsminderung incapacitating als Unterscheidungsmerkmal zwischen ASA-Klassen 3 und 4 ist weggefallen: z.b. Patient mit Myokardinfarkt, der weniger als 3 Monate zurückliegt, sollte als ASA-Klasse 4 klassifiziert werden, auch wenn er gut belastbar ist Wichtige Definitionsänderung der ASA-Klasse 5: lt. ehemaliger Klasse hätte auch die Operation nichts an der Prognose des Patienten, innerhalb von 24 h zu versterben, geändert (die Operation hätte als Kontraindikation gesehen werden können). 5
