Primäre Autoimmunneutropenie Typische Erkrankung des Kleinkindesalters Häufigste Ursache für eine chronische Neutropenie im Kindesalter Kann schon intrauterin beginnen Inzidenz 1:100.000, nach Erfahrungen aus unserem Labor deutlich häufiger 2015: 50 Kinder mit Autoantikörpernachweis Mädchen und Jungen etwa gleich häufig betroffen Klinisches Bild: häufig symptomlos typische bakterielle Infektionen: Otitis media, Tonsillitis, Hautabszesse Aphthen
Fallbericht, Teil 1 A 15-month-old girl presented with fever and a subcutaneous abscess of 1,5 cm diameter in the right axilla. Blood count showed haemoglobin 105 g/l, WBC 6,57 10 9 /l, platelets 446 10 9 /l. Differential count revealed neutrophils 6%, eosinophils 5%, monocytes 19% and lymphocytes 79%; absolute neutrophil count (ANC) was 0,39 10 9 /l. C-reactive protein (CRP) was elevated to 98 mg/l. After surgical incision and antibiotic therapy the patient recovered quickly. The family history, past medical history and physical examination of the patient were unremarkable. During the following months the ANC remained low, between 0 and 0,4 10 9 /l; there were no severe infections.. BJH 2008;142:504

Fallbericht, Teil 2 Repeated testing for granulocyte antibodies (PR3, MPO, canca, panca) and antinuclear antibodies (ANA) was negative. After 5 months of persistent neutropenia, bone marrow aspiration and biopsy were performed, which shows a slightly hypercellular bone marrow with normal erythro- and megakaryopoiesis. Granulopoiesis was hyperplastic with a shift to the left, maturing up to the level of band forms.. The age of the patient and the benign course of the disease despite the profound neutropenia make the most likely diagnosis chronic benign neutropenia, which is usually autoimmune in origin, even if circulating neutrophil antibodies are not always detected..
