Die neue AHA/ACC-Leitlinie, die statt LDL-Zielwerten prozentuale Cholesterinsenkungen durch Statinbehandlung unterschiedlicher Intensität vorgibt sowie in der Primärparävention einen für europäische Patienten nicht validierten Risikoscore zugrundelegt, ist für deutsche Patienten mit Diabetes wenig geeignet. To match intensity of therapy to absolute risk, that is the answer. 2. Die partielle Übernahme durch die ADA (geringere Betonung des Risikoscores) ändert an dieser Einschätzung wenig. 3. Neue, in den Leitlinien noch nicht berücksichtigte Daten (IMPROVE-IT) zeigen, daß noch niedrigere LDL- Cholesterin Zielwerte tatsächlich noch besser sind. 4. Erweiterte Therapieoptionen: Hochdosis-Statin und Kombination mit Ezetimib bei o Statin-Unverträglichkeit o Verfehlen der Therapieziele Kombination mit Fenofibrat bei o high TG/low HDL (künftig auch PCSK9-Hemmer?) 5. Diabetogener Statineffekt klinisch nicht relevant.
