Zusammenfassung Key Messages Das Druck-Volumen-Diagramm stellt die pathophysiologische Basis für das Verständnis der Beatmung von ARDS-Patienten dar. Die Beatmungsparameter sollen nach atemmechanischen Kriterien eingestellt werden ( Best of Compliance-Strategy). Der Airway Driving Pressure ( P) ist ein Surrogatparameter für Stress and Strain stellt einen valider Outcomeparameter dar. Der transpulmonale Druckgradient (Driving Pressure) ist die entscheidende Druckgröße für Stress der Lunge und somit für die beatmungsassoziierte Lungenschädigung. Die einzelnen Beatmungsparameter (VT, Drücke, Crs) dürfen nicht isoliert betrachtet werden, sondern als Komponenten einer gemeinsamen lungenprotektiven Beatmungsstrategie.
