Hintergrund Die Behandlungsstrategie für Rockwood III-Verletzungen des AC-Gelenks ist umstritten (Larsen et al. JBJS Am 1986, Weinstein et al. AJSM 1995, Smith et al. J Orthopaed Traumatol 2011, Milewski et al. AJSM 2012) Chirurgische Komplikationsraten werden zwischen 18% und 30% angegeben (Weinstein et al. AJSM 1995, Carofino et al. JSES 2010, Salzmann et al. AJSM 2011, Scheibel et al. AJSM 2011, Willimon et al. Phys Sportsmed 2011, Milewski et al. AJSM 2012, Martetschläger et al. Zusammenfassung 1) Ein konservativer Therapieversuch erscheint bei Rockwood III-Verletzungen sinnvoll 2) Gute klinische Ergebnisse können auch für Pat. mit Versagen der kons. Therapie und sekundärer OP erwartet werden 3) Verzögerte Einleitung der Behandlung (> 30 Tage nach Trauma) reduziert die Erfolgswahrscheinlichkeit der kons. Therapie

Beitzel K, Mazzocca AD, Bak K, Itoi E, Kibler WB, Mirzayan R, Imhoff AB, Calvo E, Arce G, Shea K; Upper Extremity Committee of ISAKOS. Arthroscopy. 2014 Feb;30(2):271-8. We suggest the addition of grade IIIA and grade IIIB injuries to a modified Rockwood classification. Grade IIIA injuries would be defined by a stable AC joint without overriding of the clavicle on the cross-body adduction view and without significant scapular dysfunction. The unstable grade IIIB injury would be further defined by therapy-resistant scapular dysfunction and an overriding clavicle on the cross-body adduction view.

Studie über primär kons. Therapie bei Rockwood V-Verletzungen Akute chirurgische Versorgung: binnen 90 d nach Trauma ohne kons. Therapieversuch Versagen kons. Therapie: kein return to full duty oder sekundäre OP. 32 Pat. mit Rockwood V-Verletzungen Primäre Rekonstruktion: 10 Pat. 8 Pat.
