Zusammenfassung Die Option der Bypass Operation bleibt unverzichtbar. Die Entscheidung zur Bypass Operation individualisiert sich immer mehr. - Abhängig vom OP Risiko - Abhängig von der Komplexität - Abhängig vom Willen des Patienten - Abhängig von der Compliance des Patienten Die Hauptstammstenose ist nicht länger eine Operationsdomäne
Schlussfolgerungen Ad hoc Interventionen sollten vermieden werden. Der postoperative Patient ist genauso zu behandeln wie der Patient nach Intervention. Neue Plättchenfunktionshemmer und PCI Techniken ( IVUS / Läsionspräperation / neue Stents / CTO techniken) könnten die Intervention sicherer und effektiver machen.
