Planning target volume (prostate plus all/base of the seminal vesicles dependent on risk criteria with a 1-cm margin plus 7mm for penumbra) with a 4-field conformal technique to a dose of 50 Gy in 16 daily fractions (3.125Gy) over 22 days. When patients were grouped into good (Stage T1/2, PSA <10 ng/ml, and Gleason score <7) (n 181), intermediate (1 raised value) (n 247), or poor (2 or more raised values) (n 277) prognostic groups, the bned was, respectively, 82%, 56%, and 39%. (4 x per week) Fiducial markers for image guidance in 30 patients daily ultrasound 33 pat. Acute Phase Grade 2 and 3 toxicity was 34% and 9% for GU vs. 25% and 10% forgi symptoms. One Grade 4 acute GI toxicity occurred in a patient with unrecognized Crohn s disease. Late Grade 2 and 3 toxicity for GU was 14% and 5%, and GI toxicity was 25% and 3%. One late GI Grade 4 toxicity in a patient with significant comorbidities (anticoagulation, vascular disease). Acute GI toxicity > Grade 2 was shown to be a predictor for late toxicity Grade >2 (p < 0.001). The biochemical disease-free survival at 3 years : 95%. (4 x per week) Gross Tumor Volume (GTV1) and Clinical Target Volume (CTV1) was segmented and encompassed the prostate and the seminal vesicles, whereas GTV2/CTV2 included the prostate only. The Planning Target Volumes (PTV1 and PTV2) incorporated CTV1 and CTV2, respectively, with a uniform 0.75-cm margin. Lokal fortgeschrittenes Prostatakarzinom / S 3 Empfehlung Gesamtdosis und Fraktionierung Die in den Studien untersuchten Dosen betragen mindestens 74 Gy gegenüber einer Standarddosierung von 64 bis < 72 Gy. Da aufgrund der Daten die erforderliche Dosis für das Erreichen eines besseren biochemisch-rezidivfreien Überlebens nicht exakt definiert werden kann, wurde für die Empfehlung ein Dosiskorridor formuliert. Die Standardeinzeldosis der perkutanen Strahlentherapie des Prostatakarzinoms ist 1,8 Gy bis 2,0 Gy. In allen randomisierten Phase III Studien zur Dosiseskalation und zur kombinierten Radiatio mit Hormonentzugstherapie wurden diese Einzeldosen eingesetzt.
Lokal fortgeschrittenes Prostatakarzinom / S 3 Empfehlung Gesamtdosis und Fraktionierung Die hypofraktionierte perkutane Strahlentherapie, eine Behandlung mit höherer Einzel- und geringerer Gesamtdosis, entspricht einer alternativen Fraktionierungsmethode. Zusammenfassend stellt nach derzeitigem Kenntnisstand (auf der Grundlage von 5 RCTs) die perkutane Strahlentherapie mit Hypofraktionierung mit mindestens gleich niedriger Spättoxizität und gleich guter Tumorkontrolle eine valide Alternative zur perkutanen Strahlentherapie mit Standarddosierungen dar. Durch die Erhöhung der Einzeldosen wird die Behandlungszeit für die Patienten verkürzt und der Aufwand für die Therapie verringert, was zu einem Gewinn an Lebensqualität führen kann.
