No 4 Case Vignette 26 year old male c/o headache, polyuria & polydipsia No headache, no visual disturbances, no Perinaud syndrome Blood sugar and calcium levels were normal Hypernatriemia with inadequately low osmolar urine
No 4 Case Vignette 12/2016 02/2017 Transnasal biopsy: inconclusive, no tumor Endoscopic ventriculoscopy & resection biopsy MIB1 50%, CD117 & OKT 3/4 positive, nuclear SALL4 positive, HCG & AFP negative CK 8a & CD 30 negative, HHV8 & BRAF negative Diagnosis of a germinoma was made
Histology I Pure Seminoma = Germinoma (60%) Less aggressive No other histological elements, never teratoma Never elevations of AFP, occationally HCG (debate) Exquisitely sensitive to radiation & chemotherapy Non-Germinoma and mixed tumors (40%) More aggressive Often associated with teratoma Elevations of AFP & HCG frequent Less sensitive to radiation & chemotherapy
Histology II Teratoma (rare) Least aggressive, very slowly growing Various histological elements May have elevations of AFP, rarely HCG Absolutely insensitive to radiation & chemotherapy Germinoma Yolk sac tumor Embryonal Ca. Chorion Ca. Teratoma Histologies CourtesyPeter Bode, Zürich
Staging Complete workup MRI brain and lumbar spine AFP and HCG in serum Lumbar puncture for cytology & AFP/HCG in cerebrospinal fluid In adolescents/adults: Testicular exam In adolescents/adults: CT thorax and abdomen Surgical biopsy/histology Absolutely required, particularly if AFP & HCG is normal May be omitted if clear cut clinical & radiological diagnosis plus unequivocal elevations of AFP/HCG
Is upfront resection required? Upfront = no No upfront resection due to radio-/chemotherapy sensitivity Rare indication for upfront resection in case of pure teratoma Residual tumor resection = yes, in non-germinomas Indication in non-germinomas with residual tumors after radio-/chemotherapy particularly, if initial teratoma component present No indication for resection in germinomas with residual tumors
